This report discusses the June 14, 2003, accident in which the U.S.
small passenger vessel Taki-Tooo capsized while attempting to cross
the bar at Tillamook Bay, Oregon. From its investigation of the
accident, the Safety Board identified the following major safety
issues: decision to cross the bar, Tillamook Bay operations, and
survivability. On the basis of its findings, the Safety Board made
recommendations to the U.S. Coast Guard, the National Marine
Charter Association, and the owners and operators of charter
fishing vessels operating out of Tillamook Bay, Oregon.
