Recent decades have seen a decline in the emphasis on sexuality in psychoanalytic theory, while clinical psychology has become more involved in sexual health issues. However, sexuality remains at the core of human experience, and where there are psychological and psychotherapeutic treatments, there will be sexual issues to be addressed. Sex, Mind and Emotion is a collection of predominantly clinical papers that use a fusion of psychoanalytic, systemic and cognitive theories in conjunction with public service practice. Defining problematic sexual behavior is an issue fraught with difficulty, as acceptable behavior is something affected by social and cultural mores. The authors have defined problematic behavior in various ways, including behavior that has been deemed to be problematic by law, a biostatistical definition of normality and abnormality, and the admission of subjective distress by the patient.The book is divided into three parts: developments in theory, client groups posing new challenges, and innovative therapeutic approaches. It deals with important and relevant topics such as the treatment of sex offenders; the compulsive use of Internet pornography; the psychosexual development of adolescents growing up with HIV; the psychodynamics of unsafe sex; refugees and sexuality; services for people with gender dysphoria; psychological treatment for survivors of rape and sexual assault; and loss of sexual interest. The central tenet is that sexual behavior cannot be divorced from the emotional context in which it occurs and therefore no chapter is about sex without also addressing "mind" and "emotion." Throughout the book, two common themes are the inextricable interrelatedness of biology and psychology, and the importance of a developmental perspective. The contributors also compare clinical evidence with theoretical models and refine both practice and theory accordingly. This book outlines a comprehensive model of risk which draws on mainstream empirical research, threat assessment, developmental psychopathology, attachment theory, and a relational model derived from psychoanalysis. The framework incorporates intrapersonal and interpersonal dimensions and is designed to enhance the reader's capacity to make a thorough risk formulation. The approach highlights the significance of childhood development in understanding violent and sexually violent acts, and the complicated interpersonal processes involved in managing individuals who have a propensity to violent enactment. These dynamic processes between people impact on risk and risk perception, and can distort judgement if not recognised and understood. Assessing Risk will be of practical use in enhancing the skills of professionals to assess and manage risk in a comprehensive and effective way, and will appeal to all those mental health and criminal justice practitioners working with risky individuals.

We are provided with a graphic account of the multiple problems-physical, psychological, social, financial-of someone with severe drug addiction, where sex working and the risks of pregnancy, infection, and assault compound an already challenging presentation. The personal history of trauma and abuse means that the patient requires highly skilled and sensitive management, and adaptations in service provision-such as no morning appointments-that respect the individual's lifestyle. The conflict for professionals is encapsulated in a brief description of the responses of Vanessa Crawford's patient group when asked what messages they would like to be conveyed to future doctors: don't prejudge us, treat us as individuals, give us proper pain control-and "don't trust us". Implicit in this is the recognition that they are in the grip of something that leads them to deceive, probably themselves, but also others-a wish to pervert a relationship to someone who is trying to help. Crawford conveys the importance of being knowledgeable, but not omniscient; of helping the individual to overcome the barrier of shame, which may lead to information being withheld; and the crucial contribution of a collaborative and coherent staff team in containing such challenging patients and in helping them to turn a corner towards recovery.
