This issue will explore the complications, considerations and
consequences of colorectal surgery. Topics will include evaluating
colorectal surgery patients to estimate and minimize morbidity and
mortality, fast-track pathways, intra-operative adjuncts,
colorectal anastomotic complications, proctectomy, stomas, Crohn's
disease, rectal prolapse, pelvic surgery, laparoscopy,
enterocutaneous fistula, and more.
This issue of the Surgical Clinics of North America covers a broad
spectrum of anorectal disease such as basic anatomy, physiology,
and testing of the anorectal region, as well as the most current
diagnostic and management strategies for many of the common
anorectal conditions encountered by surgeons including hemorrhoids,
anal fissures, abscess and fistula.? Also presented are new
techniques in the management of pilonidal disease, perianal
infections including HIV and condyloma, pruritis ani, and anal
stenosis--difficult conditions that often are frustrating for
patient and surgeon alike.? Two articles are dedicated to the
current management of anorectal Crohn's disease and rectovaginal
fistulas, including the evolving multidisciplinary approach.? In
addition, advances in the evaluation and current management of
retrorectal tumors and anal neoplasms including the role of local
excision and transanal endoscopic microsurgery for selected rectal
tumors are covered.? Rounding out this issue are approaches to the
difficult situation of anorectal trauma including foreign body
management, as well as two articles dedicated to the evaluation of
pelvic floor disorders and fecal incontinence.
According to the author, Christianity offers a powerful system
of rewards and incentives to create cultural uniformity. Those who
do not join in this cultural uniformity become anathematized,
oppressed, marginalized, and ultimately removed from the Christian
circle of moral obligation. Using culture studies as a framework
for analysis, Steele investigates the ways in which Christianity
created cultural conditions based on a theology of violence and the
use of sacred violence to foster behaviors that would lead to the
involvement of millions of perpetrators and bystanders during the
many instances of extreme violence used against the Other over the
centuries. As the original Disconfirming Other in the Christian
cultural world, Jews often served as the primary target. Thus,
there was a system of definitions, rewards, incentives, and victims
already in place when the Nazis came to power. Calling for a
re-evaluation of the cultural practices and values that have
developed within Christianity over time, this important new book
helps account for the phenomenon of the Nazi perpetrators and
bystanders during the Holocaust.
Framing the Holocaust as a late but logical development in a
long series of violent responses by Christianity to the
Other--those who stand outside the Christian world, either by
geographical accident, religious tradition, or some other
factor--the author attempts to show how the Holocaust, while not a
specifically Christian event, was nevertheless sanctioned and
conditioned by other events in the history of Christianity. Using
culture studies to frame his analysis, Steele focuses on historical
antecedents that help account for the apathy of bystanders and
point to the preexistence of a moral framework supporting and
empowering the perpetrators of the Holocaust. This unique
perspective concludes that the Nazis invented almost nothing with
regard to the Shoah, and that, instead, a long-standing insistence
on cultural hegemony played a much bigger role in the attempted
destruction of the Jewish community.
Identifying elements of the Christian worldview that have
influenced our theories of tragedy, Steele demonstrates how these
theories fail when applied to Holocaust literature. The challenge
of interpreting Holocaust literature is highlighted by a close
investigation of the extent to which Christian thought, especially
the view of transcendence, has permeated theories of
interpretation. The author appeals for a new theory of tragedy
which would allow an understanding of Holocaust literature without
Christian interpretive biases. This book will be of interest to
scholars of Holocaust literature, religion, and literary criticism.

Troubled by the repression unleashed by World War I, Justice Oliver Wendell Holmes, Jr. insisted that the functioning of the democratic system depended on the right of all Americans to be heard, regardless of how obnoxious their views, provided their words posed no "clear and present danger." Colorectal Surgery has continued to experience tremendous growth in
both the community and academic settings over the past few years.
The recent increase in demand for colorectal specialists has been
fueled by an overwhelming number of applications to fellowship
training programs, resulting in some of the most coveted and
competitive positions. Furthermore, the accumulation of experience,
knowledge, and wisdom from pioneers in the field, combined with
major recent technological advances, has transformed the clinical
management of diseases of the colon and rectum. Colorectal Surgeons
have embraced advances ranging from minimally invasive approaches
for complex problems to novel training methods for future
generations. Additionally, we have spearheaded innovations in the
management of colorectal cancer, pelvic floor disorders,
diverticulitis, inflammatory bowel disease, and anorectal
conditions. Despite these improvements, there remains a seemingly
never-ending mixture of complex patient disease processes and
complications resulting from the care of these patients. Even in
cases where the technical challenges were managed successfully,
complications or poor function may result in dramatic life-long
consequences, reduced quality of life, as well as having economic
implications. The American Society of Colon and Rectal Surgeons
(ASCRS) is the premiere professional organization of Colon and
Rectal Surgeons. Three editions of the ASCRS Textbook of Colon and
Rectal Surgery have been published and have proved to be extremely
valuable for their wealth of general information and knowledge,
providing not only background information, but also specifics
regarding the more complex situations that surgeons who treat
patients with colorectal disease experience on a regular basis. An
ASCRS manual was produced in in 2009 and 2014, each accompanying
their original textbooks. This has been formed by abstracting the
textbook into a bullet format; all figures and most tables were
retained. The 3rd edition of the Textbook (published by Springer)
included completely new chapters and authors. This 3rd edition of
the Manual is indicated to conform to the new edition of the
Textbook and incorporate newer information in the field of colon
and rectal surgery. This Manual will serve as a very useful
resource for physicians and researchers dealing with diseases of
the colon and rectum. It will provide a concise yet comprehensive
summary of the current status of the field that will help guide
education, patient management and stimulate investigative efforts.

Many of the original books in the series have been unavailable for
years, but they are now in print once again. Large cardinal
hypotheses play a central role in modern set theory. One important
way to understand such hypotheses is to construct concrete, minimal
universes, or 'core models', satisfying them. Since Goedel's
pioneering work on the universe of constructible sets, several
larger core models satisfying stronger hypotheses have been
constructed, and these have proved quite useful. In this volume,
the eighth publication in the Lecture Notes in Logic series, Steel
extends this theory so that it can produce core models having
Woodin cardinals, a large cardinal hypothesis that is the focus of
much current research. The book is intended for advanced graduate
students and researchers in set theory.

develop its fine structure theory. This work builds upon the
existing theory of a model of the form L[E], where E is a coherent
sequence of extenders, and relies upon the fine structure theory of
L[E] models with strong cardinals, and the theory of iteration
trees and 'backgrounded' L[E] models with Woodin cardinals. This
work is what results when fine structure meets iteration trees.
The Annual European Meeting of the Association for Symbolic Logic,
generally known as the Logic Colloquium, is the most prestigious
annual meeting in the field. Many of the papers presented there are
