Der Arzneiverordnungs-Report ist seit 1985 eine gemeinsame Publikation von Autoren aus Pharmakologie, Klinik, Praxis, Gesundheitsökonomie und Krankenversicherung. Basis sind die Verordnungsdaten von Arzneimitteln für ambulante Patienten der gesetzlichen Krankenversicherung (GKV). 

Billions of people worldwide take medicines every day to treat important diseases. In many cases, however, neither the doctor nor the pharmacist has the time to explain to the patient why a particular drug should be taken, how the drug works and what side effects to expect. Of course, the patient can find "everything" about a particular drug on the Internet. But how reliable and understandable is this information? In addition, most Internet sources do not point out the interrelationships between different diseases and drug interactions. Written by an experienced and well-known textbook author, this book provides an overview of the most common diseases and the drugs used to treat them. The book is designed for a general audience. It provides patients with essential information about how medications work and what side effects and interactions to expect. Finally, the book gives patients advice on what they can do themselves to improve drug therapy and safety. Summaries, bullet points, tables and diagrams support the information process.

The cyclic purine nucleotides 3',5'-cAMP and 3',5'-cGMP are well-established second messengers. cGMP has recently been covered in a volume of the Handbook of Experimental Pharmacology (volume 191). In addition to 3',5'-cAMP and 3',5'-cGMP, so-called non-canonical cyclic nucleotides exist. These comprise the cyclic pyrimidine nucleotides 3',5'-cCMP and 3',5'-cUMP, the purine nucleotide 3',5'-cIMP, the 2',3'-nucleoside monophosphates and cyclic dinucleotides. In this volume of the Handbook of Pharmacology, word-leading experts in the field summarize our current knowledge on these non-canonical cyclic nucleotides, discuss open questions, future research directions and the pharmacotherapeutic implications. Special emphasis will be given to the emerging roles of 3',5'-cCMP and 3',5'-cUMP as second messengers with regard to generators, effectors, biological functions, inactivation and bacterial toxins. The role of 3',5'-cIMP as potential second messenger will also be critically discussed. Furthermore, we will consider transport of cyclic nucleotides and their potential role as first messengers. The role of the cyclic dinucleotide cGAMP in the immune system will covered, too. Lastly, the book will present important methodological aspects ranging from mass-spectrometric methods for cyclic nucleotide detection to the synthesis of nucleotide analogs as experimental tools and holistic methods for analysis of cyclic nucleotide effects.

The book reviews the literature on the regulation of NADPH oxidase, which is controversial in many areas, and refers specifically to relevant publications from the past 4 years. Main topics are catalytical properties, cofactor requirements and structural components of NADPH oxidase, activation of NADPH oxidase, inhibition of NADPH oxidase, reconstitution and regulation of NADPH oxidase activity in cell-free systems, and pathology of NADPH oxidase.

Basis sind die Verordnungsdaten von Arzneimitteln fur ambulante Patienten der gesetzlichen Krankenversicherung (GKV). Datenbasis des Jahres 2020 sind ca. 820 Mio. Verordnungen von 150.363 Vertragsarzten und 62.876 Vertragszahnarzten fur 73,4 Mio. GKV-Versicherte. Die allgemeine Verordnungs- und Marktentwicklung wird in 6 Kapiteln behandelt, in denen u.a. neue Arzneimittel des Jahres 2020, die Kosten-Nutzen-Analyse von onkologischen Arzneimitteln, Marktsteuerungsmechanismen und Preise von Biologika bzw. Biosimilars sowie die Klassifizierung von Arzneistoffgruppen diskutiert werden. Die Nettokosten fur Arzneimittel in der GKV sind 2020 weiter um 5 % auf rund 46 Mrd. gestiegen, vor allem durch neue, hochpreisige Patentarzneimittel. Hauptkostentreiber waren Onkologika (9,5 Mrd., + 12,4 %) und Antidiabetika (2,8 Mrd., + 16,8 %). Ein wesentlicher Grund fur die Kostenprobleme ist der seit Jahren beobachtete Anstieg der Jahrestherapiekosten neu eingefuhrter Arzneimittel. Dieser Trend zu hochpreisigen Patentarzneimitteln ist durch das Arzneimittelmarktneuordnungsgesetz (AMNOG) sogar 10 Jahre nach seiner Einfuhrung nicht genugend abgeschwacht worden, auch wenn mit der Vereinbarung von Erstattungsbetragen 2020 insgesamt 3,7 Mrd. an Einsparungen erzielt werden konnten.

Fast jeder Bundesburger nimmt Arzneimittel ein, haufig mehrere. Aber oft haben weder Arzt noch Apotheker ausreichend Zeit, dem Patienten zu erklaren, warum er ein Arzneimittel benoetigt, wie es wirkt und welche unerwunschten Wirkungen es hat. Schaut der Patient dann in den Beipackzettel, wird er meist nicht aufgeklart, sondern verunsichert. Das fuhrt dann zu Complianceproblemen in der Arzneitherapie. Haufig gehen Patienten auch zu mehreren AErzten, ohne dass diese davon wissen (AErzte-Hopping). Verschreiben diese AErzte mehrere Arzneimittel, kann es zu gefahrlichen Wechselwirkungen kommen. Der Gesetzgeber hat diese Problematik erkannt und deshalb kurzlich das Gesetz fur mehr Sicherheit in der Arzneimittelversorgung (GSAV) in Kraft gesetzt. Aber fur den Patienten ist es schwierig sich objektiv zu informieren. Zwar findet er im Internet unendlich viele "Informationen", aber Zusammenhange und Gewichtungen lassen daraus meist nicht herauslesen. Am Ende ist die Verwirrung groesser als vorher. Dieses Sachbuch klart auf und liefert die Fakten, die man als Patient wissen muss.
