Timothy E. Quill, Paul T. Menzel, Thaddeus Pope, Judith K. Schwarz
R1,911 Discovery Miles 19 110 Ships in 12 - 19 working days

In the 21st century, people in the developed world are living longer. They hope they will have a healthy longer life and then die relatively quickly and peacefully. But frequently that does not happen. While people are living healthy a little longer, they tend to live sick for a lot longer. And at the end of being sick before dying, they and their families are frequently faced with daunting decisions about whether to continue life prolonging medical treatments or whether to find meaningful and forthright ways to die more easily and quickly. In this context, some people are searching for more and better options to hasten death. They may be experiencing unacceptable suffering in the present or may fear it in the near future. But they do not know the full range of options legally available to them. Voluntary stopping eating and drinking (VSED), though relatively unknown and poorly understood, is a widely available option for hastening death. VSED is legally permitted in places where medical assistance in dying (MAID) is not. And unlike U.S. jurisdictions where MAID is legally permitted, VSED is not limited to terminal illness or to those with current decision-making capacity. VSED is a compassionate option that respects patient choice. Despite its strongly misleading image of starvation, death by VSED is typically peaceful and meaningful when accompanied by adequate clinician and/or caregiver support. Moreover, the practice is not limited to avoiding unbearable suffering, but may also be used by those who are determined to avoid living with unacceptable deterioration such as severe dementia. But VSED is "not for everyone." This volume provides a realistic, appropriately critical, yet supportive assessment of the practice. Eight illustrative, previously unpublished real cases are included, receiving pragmatic analysis in each chapter. The volume's integrated, multi-professional, multi-disciplinary character makes it useful for a wide range of readers: patients considering present or future end-of-life options and their families, clinicians of all kinds, ethicists, lawyers, and institutional administrators. Appendices include recommended elements of an advance directive for stopping eating and drinking in one's future if and when decision making capacity is lost, and what to record as cause of death on the death certificates of those who hasten death by VSED.

Michael Bennett
R1,051 Discovery Miles 10 510 Ships in 12 - 19 working days

Neuropathic pain is increasingly recognized as a chronic disabling condition. It is frequently thought of as harder to treat than other pain types, and it often results in a poorer quality of life. Around 30% of adults in the UK alone have some type of chronic pain and some estimates suggest that one in five of these will have symptoms of neuropathic pain.
This second edition of the popular pocketbook has been fully updated to include recent developments in the diagnosis and management of neuropathic pain. It includes new chapters on the clinical assessment of neuropathic pain and the use of screening tools in its diagnosis. This edition also includes a new chapter focusing on neuropathic lower back pain, a newly emerging clinical entity which is thought to be the most prevalent type of neuropathic pain in developed countries.

Pathogenesis of Neuropathic Pain - Diagnosis and Treatment (Hardcover, 1st ed. 2022): Daryl I. Smith, Hai Tran Pathogenesis of Neuropathic Pain - Diagnosis and Treatment (Hardcover, 1st ed. 2022)
Daryl I. Smith, Hai Tran
R5,705 Discovery Miles 57 050 Ships in 12 - 19 working days

This comprehensive source on the pathogenic origins of neuropathic pain covers the detailed molecular bases of the currently known neuropathies as classified by their pathogenic origins. Filling a critical need, this book fills the need for a resource on a syndrome that demands improved understanding by clinicians and researchers alike so that treatment options for patients are not categorically limited to a pill or a needle. If the clinician understands the origins of a patients' neuropathic pain, they can work cooperatively toward improving it with tailored therapies that don't create societal diseconomies and that ultimately are effective in helping patients. The book presents in detail the molecular bases of some currently known neuropathies by their pathogenetic origins, allowing clinicians to tailor more specific and more effective treatment regimens for their patients. For basic researchers, this book is a general resource to better direct research on neuropathy-specific molecular mechanisms. The improved understanding of the pathogenesis of neuropathic pain can then be used to develop more specific and more effective manipulations of these pathways.

Beyond mortality, it has a major impact on healthcare resource utilization and a persistent negative effect on patients' long-term reported outcomes. This book is edited by surgeons who are passionate about the topic, and about optimizing the outcomes for polytrauma patients. Grasping the fascinating pathophysiology of MOF is essential for providing patients with quality early care. Each chapter highlights key learning objectives, historical perspectives, diagnostic and therapeutic pearls, and includes a must-know summary, additional reading suggestions and future research directions. Flowcharts, decision-making guides, summary tables, graphics and clinical photographs help to maximize the learning experience and to ensure readers retain what they've learned. The book fills a unique niche area for many specialties dedicated to critical care of polytrauma patients and to their management before and beyond intensive care.

Mechanical Ventilation - Physiology and Practice (Paperback, 2nd Revised edition): John W. Kreit Mechanical Ventilation - Physiology and Practice (Paperback, 2nd Revised edition)
John W. Kreit; Series edited by John A. Kellum
R2,847 Discovery Miles 28 470 Ships in 12 - 19 working days

Mechanical ventilation is an essential life-sustaining therapy for many critically-ill patients. As technology has evolved, clinicians have been presented with an increasing number of ventilator options as well as an ever-expanding and confusing list of terms, abbreviations, and acronyms. Unfortunately, this has made it extremely difficult for clinicians at all levels of training to truly understand mechanical ventilation and to optimally manage patients with respiratory failure. Mechanical Ventilation was written to address these problems. This handbook provides students, residents, fellows, and practicing physicians with a clear explanation of essential physiology, terms and acronyms, and ventilator modes and breath types. It describes how mechanical ventilators work and explains clearly and concisely how to write ventilator orders, how to manage patients with many different causes of respiratory failure, how to "wean" patients from the ventilator, and much more. Mechanical Ventilation is meant to be carried and used at the bedside and to allow everyone who cares for critically-ill patients to master this essential therapy.

Opioids in Cancer Pain (Paperback): Karen Forbes Opioids in Cancer Pain (Paperback)
Karen Forbes
R1,047 Discovery Miles 10 470 Ships in 12 - 19 working days

The World Health Organization published Cancer Pain Relief in 1990, advocating simplicity in the choice of analgesic and of route of administration. Since that time an increasing number of opioids for moderate to severe pain in an increasing number of formulations have become available, making professionals' choices about cancer pain management more complex.
Part of the Oxford Pain Management Library, this book compares and reviews the current opioids for moderate to severe pain and considers their place in the management of cancer pain, using morphine as the accepted 'gold standard' worldwide. The first sections of the book deal with teh principles of pain management in cancer pain and the range of treatments available for pain due to cancer. Chapters on opioid pharmacology, starting and titrating opioids and managing their adverse effects are followed by discussion about talking to the patient commencing opioids for moderate to severe pain.
Sections also include current evidence around opioid perscribing in patients with liver and renal impairment, respiratory disease or a history of substance misuse. This book presents current evidence in a clinically-based, practical and applicable way, so that the book is useful for all healthcare professionals caring for patients with pain due to cancer.

Pain in Older People (Paperback): Peter Crome, Chris J. Main, Frank Lally Pain in Older People (Paperback)
Peter Crome, Chris J. Main, Frank Lally
R1,311 Discovery Miles 13 110 Ships in 12 - 19 working days

Pain in later life is both quite common and disabling, and it differs significantly in terms of its aetiology, diagnosis and treatment from pain in the general adult population. Concise chapters will summarize up-to-date reserach literature in a practical style that will have direct relevance to busy clinicians. Introductory chapters will include the epidemiology of pain in older people as well as the proper assessment of older patients with pain conditions. Later chapters will focus on specific painful conditions common in the elderly, including arthritis and rheumatism, osteoporosis, abdominal pain, and cancer.
The book will appeal to a wide variety of health care professionals in both primary care and secondary care services such as geriatric medicine, rheumatology, orthopedics, surgery, pain management and palliative medicine. The book will also be of relevance to nurses, physiotherapists, occupational therapists, clinical psychologists, pharmacists and other health care providers.

Cancer-related Bone Pain (Paperback): Andrew Davies Cancer-related Bone Pain (Paperback)
Andrew Davies
R1,306 Discovery Miles 13 060 Ships in 12 - 19 working days

Cancer-related bone pain is experienced by patients with primary bone tumours such as myeloma and osteosarcoma, but is more commonly seen in patients with malignant tumours that have metastasised to bone. Bone pain is one of the most common and severe forms of pain associated with breast, prostate and lung cancer, yet little is known about the underlying mechanisms responsible for the pain. Cancer patients identify bone pain as the most disruptive cancer-related event in terms of their quality of life and daily functioning, and it is also associated with increased incidence of morbidity, depression, and anxiety. Part of the Oxford Pain Management Library, this volume summarises the latest evidence-based and practical information on the management of cancer-related bone pain. Chapters cover the pathophysiology and clinical features of bone pain, general principles of management and the use of opioids and other agents. It will be an invaluable reference for all health care professionals involved in the management of patients with bone pain from various disciplines including palliative care, anaesthetics, oncology and general practice.
