Until the start of the new century, efforts to strengthen health systems focused solely on the public sector and health programs overseen by public bodies. The private sector was sidelined in certain countries and even banned in others. At the same time, some private-sector stakeholders readily adapted themselves to this special situation so as to avoid becoming part of a structured health system. This volume notes profound changes in health care around the world in two areas. The stakeholders involved in the health sector are increasing in number and diversifying as a result of the development of the private sector. They are also responding to a process of democratization and decentralization. These developments have been paralleled by greater functional differentiation. Various stakeholders are increasingly specializing in particular areas of the health system: service delivery, procurement, management, financing, and regulation. The interdependence of health stakeholders becomes more evident along with the increased complexity of delivery systems as these respond to changing demand. There is a compelling need to forge relationships. Such relationships are in fact emerging in developed countries and, more recently, in developing countries. They may be informal, but are increasingly organized and structured.
This book provides panoramic overviews of critical human service organizational and management practice challenges, as well as new and needed research frontiers. The Future of Human Service Organizational & Management Research: Navigating Complex Frontiers invites researchers, educators, and practitioners to explore: the intersection of the complex environment of public and private human service organizations; and the rise and uncertain effects of new developments in social work, public policy and public management, and other helping professions. Professional burnout is an epidemic in America. Approximately half of physicians and nurses are affected and at risk for themselves and their patients. Much has been written about professional burnout. The term was originally coined in the 1970s by American psychologist Herbert Freudenberger to describe the consequences of severe stress and high ideals experienced by people working in "helping" professions. Since then, many books have been written to address this looming national public health crisis. One in five U.S. adults experiences a mental illness within a given year. With more than 550,000 people working to support this underserved community, the mental healthcare system has grappled with inadequacies and shortcomings in safety, quality, and care delivery. Problems related to the functioning of public healthcare systems encourage the search for alternative solutions, for example to ensure improved access to medical services. However, these proposals also require appropriate theoretical support to better present and apply them. This book draws on Austrian Economics to provide a theoretical framework to support greater involvement of the private sector to improve inefficiencies in public healthcare. The Austrian School of Economics has a solid theoretical output describing and explaining the functioning of many aspects of the market economy (e.g. money, prices, interest rate, or capital). This work applies those principles to a market-based healthcare system and its individual elements, including health insurance. The study in these chapters is divided into two parts. The first part contains the theoretical aspects of the functioning of a complete market system. Particular importance is placed on presenting health insurance as a market institution and exploring its role in the market system. This examination also includes an analysis of alternative forms of financing access to medical services, such as direct payments, medical savings accounts, medical subscriptions, and charity. This open access book is the first compilation that reviews a wide range of social determinants of health (SDHs) for non-communicable diseases (NCDs) and healthy ageing in Japan. With the highest life expectancy and the largest elderly population in the world, Japan has witnessed health inequality by region and social class becoming more prevalent since the 2000s. The first half of this volume describes in detail major NCDs, such as cancers, heart and kidney diseases, diabetes, stroke, and metabolic syndrome. The second half, on the other hand, explores various SDHs relating to healthy ageing. All chapters review and focus on SDHs, particularly health inequality associated with socio-economic status and social capital, which are widely addressed in the field of social epidemiology. The book makes the argument that "Health for All" advocated by the WHO should be implemented based on social justice and benefits for the greater society. Public health researchers and policymakers, both in Japan and other nations, will gain scientific evidence from this book to prepare for the coming era as ageing becomes a global issue.
