With the fall in mortality from infectious diseases, the* impact of childhood cancer in the Western World has increased to become the second commonest cause of death in the age group 1-14 years, being surpassed only by accidents. However, even in those countries in which paediatric cancer is attracting increasing interest, the disease is relatively uncommon, and the number of cases seen by any one physician, even in a large general hospital, is often limited to one or two a year. The widely held view by parents and even by many doctors that cancer in childhood is usually untreatable and inevitably fatal is no longer tenable. With improvement in the therapeutic response or in the actual survival rates of children with such lesions as Wilms' tumour, brain tumours, rhabdomyosarcomas, Ewing's sarcoma, retinoblastoma, lym phoma and even leukaemia, there is a real hope of achieving a substantial reduction in the mortality of childhood cancer. Paediatric oncology is, in fact, providing a vigorous stimulus to the much wider field of cancer treatment and research, and is demonstrating the advantages of a multi-disciplinary cooperation in the management of this disease.

In writing this book I have attempted to follow the gen eral pattern of the lectures which I have given to pupil midwives at Worcester Royal Infirmary for the past 21 years. In hospital it has been possible to illustrate what I have had to discuss by reference to cases at present in the wards. This unfortunately is not possible to the same extent in a book, but I have no doubt that my readers will be able to apply the principles described to cases under their care. I am sure that this is a most helpful way of fixing in one's mind the information which is required not only for examination purposes but so much more importantly for the better understanding of the clinical problems with which one is faced, and for the purpose of bringing to them the best possible management. The chapter headings and the arrangement of the chapter content may be regarded as unorthodox, but I have presented the topics in the way in which they seem to me to arise in clinical practice. This, I hope, will make easier the understanding of all aspects of a problem. [ have tried to avoid duplication, and when a topic appears in more than one place, it does so in order to discuss its relevance in a differ ent context. Cross-references have been used throughout the text, in order to facilitate reference to the other facts of importance.

The provision of the best possible anesthetic care to the-greatest number of patients all over the world has always been the main goal of the World Federation of Societies of Anaesthesiologists. In addition to the establishment of Regional Anesthesiology Training Centers and the organization of Regional and World Congresses, the distribution of practical monographs on some important aspects of anesthesiology were considered important for the attainment of this objective. In 1968, on the occasion of the Fourth World Congress of Anesthesiology, held in London, a monograph on "Emergency Resuscitation", compiled by the Committee on Cardiopulmonary Resuscitation, chaired by Professor HENNING POULSEN, was made available to our membership. Subsequent ly, this excellent monograph was translated into Spanish, Russian, Ger man and French and to this date over 100,000 copies of it have been distributed to anesthesiologists and other interested physicians the world over. When looking for a suitable topic for a second monograph to commem orate the 1972 World Congress of Anesthesiology to be held in Kyoto this September, the subject of Obstetric Analgesia and Anesthesia was the first choice of almost everyone consulted. For reasons that are hard to condone, obstetric analgesia and anesthesia has often been the step child among the anesthetic subspecialties.

Parents of children born with mental or physical handicaps, tend to face the physician with questions about the origin of the abnormality concerned and the chance of having another child with the same condition. The physician then finds himself in a difficult situation since the causes of the majority of congenital abnormalities remain as yet unknown. In most cases he has to restrict himself to a mere enumeration of the signs and symptoms encountered or, at best, to a classification of the syndrome at hand. Efforts are frequently made to connect both the somatic aberrations and the mental deficiency with complications during the later stages of pregnancy, at the time of delivery or in the immediate postnatal period. However, a careful search for such items as abnormal dermatoglyphs, or 'degenerative stigmata', and, in particular, a post-mortem examination of the brain, often indicates that the handicap concerned should be ascribed to factors operating long before the time of birth. Theoretically, the causes of abnormalities present at birth can be listed as of a genetic, germinal or peristatic nature. During the past decades special attention has been paid to both genetic and peristatic factors. The purpose of this thesis is to stress the importance of germinal factors, in particular those which, at least in theory, might lead to disturbances in the ripening process of the human egg. We were put on this track by some data concerning the circumstances under which one of our patients presumably had been conceived.
