Belasten Krankenhauser die Umwelt? Wird diese Frage an Mitarbeiter von Krankenhausern gestellt, so antworten 9 von 10 Personen mit ja. Fragt man weiter: "Aber Krankenhauser produ zieren doch nicht und verursachen keine Emissionen ... ?," so rechtfertigt der uberwiegende Teil seine Aussage, indem er auf die im Krankenhaus anfallenden Abfallmengen hinweist. Aus diesem Antwortmuster ist zu schlieBen, daB fUr viele Krankenhausmitarbeiter zwischen Klinikmull und Umweltbela stung eine enge Beziehung besteht. Dies ist eine verstandliche Einstellung, wurde doch dankenswerterweise von einigen Klini ken (z. B. in Freiburg, Straubing und Gunzburgj auf das Problem "Krankenhaus und Umweltschutz" aufmerksam gemacht. Viele Krankenhauser griffen die dort prasentierten Ideen zur Veranderung der Mullproblematik auf und haben in der Zwischen zeit eigene Strategien entwickelt und MaBnahmen eingeleitet. Bemerkenswert an dieser Initiative ist, daB sie ohne wesentliche Anregungen von auBen auskam. Denn im Krankenhaussektor fehlen, wenn man von der Ernennung eines Abfallbeauftragten einmal absieht, Handlungs anst6Be wie - gesetzliche Auflagen, - F6rderungsmaBnahmen des Staates sowie - marktwirtschaftliche Impulse. Sowohl Umweltpolitiker als auch Umweltorganisationen und Beh6rden haben bisher keine oder nur wenige eigene Vorschlage IX zum Thema "UmlM3ltschutz im Krankenhaus" entwickelt. Dieses Defizit ist sicher auch ein Grund fUr die Passivitat der Kassenorga nisationen, die bis heute ebenfalls wenig dazu beigetragen haben, umweltschutzorientierte KrankenhausfUhrung zu unterstUtzen."
Was kann national und international wirklich gegen die dro- hende Klimakatastrophe getan werden? Das Buch enthAlt die BeitrAge der Tagung "Neurowissenschaft und Ethik," die 1986 in Jakobsberg bei Bonn auf Einladung des Bundeskanzlers stattfand und sich mit den ethischen Fragen befaAte, die in den sich rasch entwickelnden Neurowissenschaften und der Neuromedizin eine Rolle spielen und schon jetzt in die Affentliche Diskussion eingetreten sind. Nach einer eingehenden Darstellung der molekularen Biologie, der Struktur und Funktion neuronaler SignalA1/4bertragungsprozesse sowie der Prinzipien neuronaler Netzwerke werden Probleme neurogenetischer Krankheiten, der Intensivmedizin und der Neurochirurgie vorgestellt. Auf dieser Grundlage werden offene ethische Fragen zu dem Problem der Erforschung und Behandlung von Nerven- und Geisteskrankheiten behandelt und in einer abschlieAenden Empfehlung formuliert.
Die Entstehungsbedingungen psychischer Stoerungen im Sinne normabweichender kognitiver und/oder emotionaler Prozesse lassen sich u.a. als Folgen von Uber- oder Unterstimula- tion auffassen, wobei zwischen sogenannten "inneren" und "ausseren" Reizen unterschieden werden muss (STEINGRUBER, 1976). Zu den inneren Reizen gehoeren beispielsweise spezifische Stoffwechselstoerungen, wobei als besonders markantes Bei- spiel die Phenylketonurie (PKU) dienen kann, bei der ein isolierter Enzymdefekt ohne rechtzeitige therapeutische Intervention im Sauglingsalter zu schweren, bleibenden Intelligenzdefekten fuhrt. Als aussere Reize koennen Umwelteinflusse im weitesten Sinne gelten, also sozio-kulturelle, familiare, sowie physiko- chemische Faktoren, deren Einwirken wahrend pra-, peri-, oder fruhen postnatalen Entwicklungsstadien mit langanhalten- den bis irreversiblen Stoerungen der geistig-seelischen Ent- wicklung verbunden sein koennen. Beispielhaft sei hier auf das von SPITZ (1945, 1946) zuerst beschriebene Hospitalis- mus-Syndrom bei maternal deprivierten Kleinkindern, sowie auf die von BRONFENBRENNER (1968) klinisch, von ROSENZWEIG und SENNET (1969) im Tiermodell verifizierte Retardierung der mentalen Entwicklung bzw. der Hirnreifung durch Mangel an sensorischer Stimulation in fruhen Entwicklungsstadien verwiesen.

Als vor langerer Zeit die Verlagsbuchhandlung mit der Auffor derung an mich herantrat, das v. ESMARCHsche Taschenbuch der Hygiene neu zu bearbeiten, habe ich freudig zugestimmt. Einmal schien mir die Neuherausgabe des nlitzlichen und in weitesten Kreisen beliebten Buches eine lohnende Aufgabe zu sein - dann aber glaubte ich auch damit einen Akt der Pietat gegen meinen verehrten Lehrer und Freund erflillen zu sollen. Die von v. ESMARCH dem Buche zugeschriebene Bestimmung: "ein Ratgeber zu sein bei der Ausflihrung hygiimischer MaBnahmen," sollte natlirlich auch fUr die Neuauflage die wichtigste Aufgabe bleiben. Auch der Kreis der Personen, flir die das Buch berechnet war: "Medi zinal- und Verwaltungsbeamte, Arzte, Techniker und Schulmanner," solI im wesentlichen derselbe sein. Es war aber wohl eine Unter schatzung seiner Arbeit, wenn v. ESMARCH glaubte, daB fUr den Hygie niker von Fach das Buch nicht als Hilfsmittel dienen kanne, im Gegen teil: es dlirfte kaum einen Hygieniker geben, der nicht, zum mindesten auf praktischem Gebiete, von dem reichen Wissen und den groBen Erfahrungen des Verfassers Nutzen gezogen hatte. Much of the international cancer literature covers the nature and challenges of the disease as seen in high-income regions of the world, thereby giving a skewed view of the global cancer challenges. As the low- and middle-income regions of the world transition from communicable to non communicable disease patterns, there is a need for a corresponding paradigm shift, with increased emphasis on what the world needs to know about non communicable diseases, including cancer, where the disease is hitherto poorly documented. The main goal of the proposed book is to contribute to this outcomes. |
