For over 50 years, the mission of the National Institute of Allergy and Infectious Diseases (NIAID) has been to conduct and support basic and applied research to better understand, treat, and prevent infectious, immunologic, and allergic diseases with the ultimate goal of improving the health of individuals in the United States and around the world. Despite the availability of an effective vaccine, there are still 400 million people, worldwide who are chronically infected with hepatitis B virus (HBV). For them, the vaccine, as currently applied, has no value. Given the possible consequences of HBV infection, the number of those chronically infected with HBV presents an enormous public health challenge. For example, the major etiology of hepatocellular carcinoma (HCC) is chronic infection with HBV. Although fifth in cancer incidence, worldwide, HCC/liver cancer is the third leading cause of cancer death. The high mortality as- ciated with HCC arises because the disease is often detected late and is unresponsive to treatment. The number of deaths caused by PHCC is expected to rise over the next 20 years. Those chronically infected with HBV have a life risk of death to HCC of between 10 and 25%. Even the limited efficacy of drugs for the treatment of chronic HBV helps underscore the point that this disease is responsive to therapy. Drugs that target the polymerase (e. g. , hepsera and lamivudine) and interferon alpha represent two distinct strategies and show that both conventional antiviral and immunothe- peutic approaches can be used in management. However, the current inventory of therapeutics is inadequate. Interferon alpha is of limited value, only parenterally ava- able, and fraught with adverse reactions.

Although the means of diagnosing and treating fungal infections have improved greatly over the last decade fungi still represent a serious threat to the health of immunocompromised and immunodeficient patients. The aim of this book is to give an in-depth assessment of our current understanding of the Biology of the main fungal pathogens and how they interact with the host s immune response. The aim of this book is to provide detailed protocols for studying the molecular biology of the pathogen Mycobacterium tuberculosis, and its int- actions with host cells. Another recent milestone for M. tuberculosis has been the development of efficient mutagenesis me- odologies, the lack of which has been a major handicap in functional studies.

The Plasmodium spp. parasite was identified as the causative agent of malaria in 1880, and the mosquito was identified as the vector in 1897. Despite subsequent efforts focused on the epidemiology, cell biology, immunology, molecular biology, and clinical manifestations of malaria and the Plasmodium parasite, there is still no licensed vaccine for the prevention of malaria. Physical barriers (bed nets, window screens) and chemical prevention methods (insecticides and mosquito repellents) intended to interfere with the transmission of the disease are not highly effective, and the profile of resistance of the parasite to chemoprophylactic and chemotherapeutic agents is increasing. The dawn of the new millennium has seen a resurgence of interest in the disease by government and philanthropic organizations, but we are still faced with compl- ities of the parasite, the host, and the vector, and the interactions among them. Surprisingly what separates us from the open environment all around us sometimes is a single layer of epithelial cells. Septic shock remains a serious medical condition with high mortality. Despite many advances in intensive care medicine and antibiotic devel- ment, this has not changed appreciably in the last 20 years. Frustratingly, over the same period of time, enormous advances have been made in understa- ing the underlying pathogenic mechanisms of this condition. This has resulted in the development of several novel therapies for septic shock, which, despite excellent theoretical grounds for their efficacy, have failed in altering mort- ity attributable to sepsis. The reasons for these failures are multiple, but it is clear that further research is required aimed at increasing our understanding of the basic pat- physiological processes that occur following infection. Research into septic shock draws upon a number of different disciplines, ranging from molecular and cellular biology to physiological measurements on whole animals. China's concentrated HIV epidemic is on the brink of becoming a generalized one and syphilis infection has become a major public health threat. Social factors relating to gender and gender inequality exacerbate the spread of HIV and sexually transmitted infections (STI) in China. A better understanding of the proximate social determinants of HIV related to gender will be crucial to effectively curbing HIV and other STIs in China. Aspects of China's governance - including administrative procedures, the developing legal system, social institutions, and the public health infrastructure - are instrumental in shaping strategies and responses to HIV. International studies suggest that women who are more economically and socially vulnerable may also have a greater risk of HIV infection, yet few initiatives have focused on discrete areas where achievable and sustainable gender policy measures could be linked to the public health response. This study presents perspectives ranging from criminology to social psychology to better understand how gender perspectives can inform HIV policy in the context of China.
