At the turn of the millennium, the world experienced a dramatic increase in funding for global health programs. Competing demands for these resources meant that policymakers needed access to valid, evidence-based information on the costs and consequences of a broad range of health interventions. By providing systematic and comparable information about purchasing health in LMICs, Disease Control Priorities in Developing Countries, Second Edition greatly informed and enriched these conversations and we hope -- improved resource allocations. Nearly ten years later, increased attention to chronic diseases and the importance of health systems in providing access to quality care is once again reshaping the global health landscape. Low- and middle-income countries are continuing to set priorities for funding and deploying specific interventions, but with a greater appreciation for the contribution of program and economic evaluation to informed decision-making. The need to make decisions across an increasingly complex set of policy and intervention choices, attuned to specific health system capabilities, makes a third iteration of Disease Control Priorities all the more critical. Similar to the first and second editions of Disease Control Priorities (DCP1 and DCP2), the aim of the third edition (DCP3) is to influence program design and resource allocation at global and country levels by providing an up-to-date comprehensive review of the effectiveness of priority health interventions. It presents systematic and comparable economic evaluations of selected interventions, delivery platforms, and policies based on newly developed economic methods. DCP3 further expands on the scope of intervention assessments found in DCP1 and DCP2 by presenting findings in nine individual volumes. It is clear that optimal global health programming requires a comprehensive evidence-base to help determine what works, what does not, how much it will cost. DCP3 will allow users to set global and national priorities for health in an informed manner."

This book presents a comprehensive, state-of the-art guide and review of ultrasound applications for children and infants with surgical problems. Cystic Fibrosis (CF) is a multisystem disease whose symptoms and signs involve the gastrointestinal tract (thus affecting nutritional status), endocrine system, reproductive system and the respiratory tract (nose, sinuses and lungs). Despite new treatments, the median survival for patients with CF is less than optimal, primarily due to complications of obstructive lung disease. Currently there are approximately 60,000-80,000 people worldwide with CF. The clinical manifestations of CF are caused by dysfunction of CFTR (cystic fibrosis transmembrane conductance regulator), a multifunctional cyclic-AMP regulated ion channel protein. Over time, there has been dramatic improvement in CF patient life expectancy, in large part related to earlier diagnosis (newborn screening), better understanding of molecular genetics and underlying pathophysiology, the integrated and highly specialized Cystic Fibrosis Foundation Accredited Care Centers, and development of a wide range of new treatments and therapies, some of which target the basic CFTR defect. This edition of Pediatric Clinics of North America will offer general pediatricians and family physicians, as well as subspecialists, an update of the extraordinary progress made in the understanding and treatment of Cystic Fibrosis.

Each chapter will focus on the known molecular characteristics of specific childhood cancers, focusing on how the molecular 'drivers' can be exploited from a therapeutic standpoint with currently available targeted agents. Childhood acute leukemias are one of the main causes of death in children aged 1 to 14 years in some countries; and unfortunately, we have been unable to prevent it. Certainly, a good parcel of it is due to the poor understanding about its etiology. This book aims to describe the most important theories and hypothesis regarding childhood acute leukemia. Written by the most outstanding researchers in the field, this book intends to contribute to a greater understanding of the etiology of this disease. It goes beyond the simple and common analysis of risk factors, which hardly allows us to draw definite conclusions. By addressing the etiology of the disease, discussing from molecular biology until epidemiology and clinical manifestations, this book will guide present and future approaches, contributing for a better clinical management of leukemia in children. Almost one out of every three US children is overweight or obese, with minority youth at highest risk. There are limited efficacious pediatric obesity interventions available for clinicians, and successful weight loss trials for minority youth are rare. Even fewer interventions have been shown to significantly improve clinical health outcomes such as adiposity, blood pressure, and cholesterol level, and maintenance of behavior change over the long-term remains a challenge Translation I research in which "bench" findings are applied to the "bedside" is uncommon in the behavioral arena. Thus, advances in our understanding of fundamental human processes such as motivation, emotion, cognition, self-regulation, decision-making, stress, and social networks are not being optimally applied to our most pressing behavioral health problems. This issue of Pediatric Clinics will focus on promising behavioral treatments "in the pipeline" that have been translated from basic behavioral science and are the process of refinement and proof of concept testing.

Preterm neonates remain at increased risk for adverse bilirubin-related outcomes, including acute bilirubin encephalopathy relative to term infants. Yet, most vulnerable neonates are likely benefit form the potent anti-oxidant properties of bilirubin. Evidence-based guidelines for the management of hyperbilirubinemia in preterm infants, however, are lacking. High concentrations of unconjugated bilirubin can cause permanent neurologic damage in infants, evident through magnetic resonance imaging of chronic bilirubin encephalopathy or kernicterus. There is a growing concern that exposures to even moderate concentrations of bilirubin may lead to subtle but permanent neurodevelopmental impairment referred to as bilirubin-induced neurologic dysfunction. Our current use of phototherapy to decrease bilirubin loads and its potential photo-oxidant properties is a biological conundrum that has been questioned in the use of phototherapy for very low birth weight neonates. In this issue of Clinics in Perinatology, we provide updates on the current understanding of the biology, mechanisms of increasing bilirubin load due to hemolysis, decreased bilirubin binding capacity and glucose-6-phosphate dehydrogenase deficiency, as well as clinical strategies to operationalize the thresholds for hyperbilirubinemia interventions in preterm infants.

The impact of cardiovascular disease on an infant extends from the fetal period to well beyond childhood. Perinatalogists and neonatologists can impact maternal and fetal health through wide range of diagnostic modalities and interventional techniques. For our edition focused on cardiovascular health, we sought to encompass the breadth of knowledge that would be the most relevant for the bed side clinician. Our goal was to assemble contents that would allow a clinician to quickly peruse the journal, and then be prepared to make a medical decision. The interaction between cardiology and perinatology/neonatology includes genetics, diagnostics, interventions, counseling, routine stabilization and day to day care. Ultimately, the goal is to establish the foundation for a healthy adult. For this reason, we have even included chapters on topics that are significant on a day to day basis (such as the proper environment for a newborn) and a long term basis (like the overall neurodevelopmental impact of our interventions). Hopefully, whether in the middle of the night as an emergency reference or during the day as a reliable guide, this edition of Clinics in perinatology will be an important bedside tool for anyone that participates in the care of a patient with perinatal heart disease.

This book is concerned with one of the major areas of misdiagnosis in medicine today. It has been estimated that as many as thirty percent of children typed as 'epileptic' when presenting with loss of consciousness, falls or other paroxysmal episodes have, in fact, non-epileptic attacks. Yet becauseof the stigma attached to this label, and the frequent side effects of anti-epileptic drugs, it is important that the alternative etiological mechanisms be recognized. The most common of these conditions, the vagalmediated anoxic seizure or fainting fit, is so frequent that it will be encountered by anyone involved in the practice of clinical medicine in its widest sense. These seizures are discussed in depth using original data and extensively reviewing the literature. Many other less common, but nonetheless important, varieties of fits, faints and 'funny turns' are self-induced by a Valsalva maneuver, and motor events ranging from day-dreaming to the effects of suffocation and brain swelling. Depression is the most common complication of childbirth and results in adverse health outcomes for both mother and child. It is vital, therefore, that health professionals be ready to help women who have depression, anxiety, or posttraumatic stress disorder in the perinatal period. Over a short few decades, the field of pediatric endoscopy has matured from the exploratory to the routine. Performance of endoscopic procedures in children is now a fundamental aspect of the practice of more than 2000 pediatric gastroenterologists in North America, and endoscopic instruments are increasingly being developed with an eye to their pediatric applications. Ensuring safe and effective endoscopy in children requires specific medical knowledge and technical competency, in addition to appropriately designed equipment and settings. Obtaining consent from parents, as well as assent from patients, for the purposes of performing diagnostic and therapeutic gastrointestinal procedures begins with a deep understanding of risks and benefits that endoscopy affords and is typically gained through formal training in the field. Diagnostic endoscopy may help to confirm common pediatric conditions including eosinophilic esophagitis and inflammatory bowel disease, while therapeutic procedures to treat strictures in the GI tract may help children avoid more invasive surgeries. Using endoscopy in children to achieve hemostasis or to remove commonly swallowed foreign bodies, such as lithium batteries or high-powered magnets, can be lifesaving, and the insertion of feeding tubes can help medically complex patients to thrive. In short, pediatric endoscopy is an integral component of healthcare for children, and gaining and understanding of its best practices may help all clinicians to better recognize its role in pediatric disease outcomes.

Pediatricians in the Unites States and around the World continue to face a myriad of global health threats affecting child and adolescent health including: 1) infectious diseases of poverty [e.g. human immunodeficiency virus (HIV), tuberculosis, malaria, and neglected tropical diseases] in low-and middle-income countries (LMIC), especially in sub Saharan Africa; 2) emerging and reemerging infectious diseases (such as Ebola); 3) rise of non-communicable diseases (e.g. common mental disorders); 4) unintentional injuries; and 5) environmental health hazards (e.g. climate change). Despite the promising news about rapid declines in maternal and child mortality in the era of Millennium Development Goals (MDGs), which originated from the United Nation (UN) Millennium Declaration in 2000, only 20% and 7% of LMIC are currently on track to attain the maternal and child mortality targets. For example, 44% of deaths in children younger than 5 years occur in neonates. Besides discovery of life-saving interventions (e.g. development of new and improved vaccines) for maternal and child health, we also need to do a better job at bridging the knowledge-implementation gap and increase the effectiveness of proven interventions. For example, despite the availability of effective vaccines to prevent pneumococcal pneumonia, rotavirus gastroenteritis, and human papilloma virus-related diseases (e.g. cervical cancer), use of these vaccines remain suboptimal in LMIC. We need to recognize that global health is also local public health. For example, improving access, equity and quality of care for orphans and vulnerable children, immigrant and newly arrived refugee children in the U.S. remains a challenge. Timely access to psychiatric care for children and adolescents with mental illness is a major concern. The explosion of new age technology (such as the internet) also poses a considerable risk to children and adolescents. Pediatricians also need to be aware of diverse socio-cultural determinants of health and ethical issues in global health service and delivery. This issue of Pediatric Clinics of North America aims to address the above crucial global health challenges affecting children and adolescents. As practicing pediatricians, we have the unique opportunity to influence local and global public health. In the post-MDG era (beyond 2015), collaborative partnerships between various disciplines and across research, education and service is vital to reduce health inequities in children worldwide.

Bronchopulmonary dysplasia (BPD) as a chronic lung disease affecting preterm infants has been recognized for more than 45 years. However, little progress has been made in the prevention and treatment of the disease. BPD continues to be a major morbidity affecting preterm infants. Studies in pre-adolescent children demonstrate that the abnormal lung function from BPD persists well into childhood. Infants with BPD also have an increased risk for adverse neurodevelopmental outcome. Thus BPD imposes a significant burden of adverse health outcomes in preterm infants. The major risk factors for BPD are prematurity, mechanical ventilation, exposure to noxious insults such as oxygen, infections. New research data both in basic science and clinical studies have shed light on the pathogenesis, and possible new treatment or management strategies for the future. In the proposed issue of the Clinics in Perinatology, we would like to comprehensively cover BPD both from a basic science and clinical perspective. Our attempt is to balance new information along with classic topics. The proposed authors for the issue are recognized experts in the topic area/s. We hope that the issue will be of interest to a broad array of readership.
