Thus, the authors provide a review of classification of transsphenoidal skull base approaches and show the main transnasal corridors to the sphenoid sinus based on the anatomical sphenoidal target and extension of pathology.Lastly, a synopsis of the pathophysiology of post-craniotomy temporomandibular dysfunction and a practical approach to its evaluation and treatment is provided. Several surgical procedures can be used to perform craniotomy, such as the classical pterional, pre-temporal and other fronto-temporal approaches. Obesity is a multifactorial global epidemic with associated risk factors for an array of vascular, metabolic, psychological and economical consequences. Bariatric surgery has revealed a significant link between gastrointestinal metabolism and obesity that extends to resolution of many metabolic diseases including type 2 diabetes and cardiovascular disease risk. Bariatric surgery offers the most effective results and Roux-en Y gastric bypass (RYGB) is the most commonly performed bariatric surgery worldwide. The effective weight loss achieved by RYGB is thought to be caused by the re-arrangement of the gastrointestinal tract leading malabsorption and restriction of the ingested nutrients, and consequently resulting in alterations in the gut hormone levels. The first chapter of this book briefly summarises the current literature on the changes in the gastrointestinal hormones following RYGB. Chapter Two studies the role of bariatric surgery in the resolution of obesity and type 2 diabetes. Chapter Three discusses how to treat a type 2 diabetes patient post-bariatric surgery. The last chapter discusses the recent findings of the relationship between gastric bypass and four different skin disorders.
The Department of Veterans Affairs (VA) Veterans Health Administration (VHA) is one of the largest purchasers of surgical implants, which include biological implants, such as skin and bone grafts, and non-biological implants, such as cardiac pacemakers and artificial joints. Surgical implants fall under VHA's definition of prosthetics, which includes all items that support or replace a body part or function. Federal law grants the Secretary of VA the authority to procure prosthetics and related necessary services in such manner as the Secretary may determine to be proper, without regard to any other provision of law. This book examines factors that influence clinicians' decisions to use particular implants when multiple, similar items are available; selected VAMCs' compliance with pertinent VHA requirements for documenting open- market purchases; and VA's and VHA's oversight of VAMC compliance with implant purchasing requirements.

The thyroid gland is located in the neck deep to the platysma. This is a broad sheet of muscle that crosses over the clavicle to cover the antero-lateral neck. Its anterior fibres interlace with the opposite side, finding an attachment to the mandible and the dermis of the lower part of the face. The platymsa covers the sternocleidomastoid and the strap muscles of the neck: sternothyroid, sternohyoid and omohyoid.
