This book represents a refreshing and broad view of the nature of allografts, as well as the challenges involved in their use as critical therapy for a variety of different health problems. It is the work of a group of very talented and experienced physicians representing different medical disciplines, but all of them intimately involved with the use of allografts, presenting their distilled experience. The notion of allografts is explored in specialty areas ranging from orthopaedics and thoracic surgery to kidney and liver transplantation, in an effort to identify certain common themes having to do with the immunological response to the allograft and the various methods to avoid rejection. Chapters in this book present current challenges in the use of bone allografts in complex orthopaedic procedures, in addition to dealing in depth with the use of hepatic and renal allografts in liver and kidney transplantation. Complex issues, such as the role of the immune system trying to deal with allograft acceptance or rejection on the one hand, and the presence of hepatitis B or C infection on the other hand, with the addition of the possibility of coexisting hepatocellular malignancy, are at the core of this book. The authors present the therapeutic problems seen in clinical practice and offer possible solutions, based on their extensive experience. This book will be a valuable resource to scientists involved in disciplines such as transplantation, immunology, and orthopaedics, as the themes that are analysed here cover a broad range of areas.
The field of surgery has seen significant changes over the last decade. New surgical techniques in thyroid, parathyroid and adrenal surgery characterized by reduced trauma of access and new technology have heralded minimally invasive techniques that have allowed surgeons to perform operations with results similar to traditional approaches whilst offering advantages in recovery time and cosmesis. This book focuses on topics of minimally invasive endocrine surgery; laparoscopic and robotic colorectal resection for cancer; the right anterolateral thoracotomy technique for patients with simple congenital heart disease; long-term clinical status, quality of life and functional outcomes after laparoscopic approach to achalasia; the evolution of component separation techniques for massive incisional hernias; the advances towards beating heart surgery; and finally, the operative techniques and outcomes of needlescopic surgery.

The fourth edition of this highly successful textbook discusses the practical applications of the various methods of regional anaesthesia, including ultrasound, as well as giving an account of their theoretical aspects. The well-established author team use their experience to provide a practical and stimulating book which reflects everyday clinical activity, supplemented by high definition ultrasound images and schematic diagrams. The book will also discuss the place of regional anaesthesia in current medical practice, particularly with regard to pain processes, ultrasound imaging, and patient outcomes. Regional anaesthesia is a rapidly expanding sub-speciality of anaesthesia: routine provision of long-lasting pain relief and the introduction of real-time ultrasound block has fuelled a demand for cadaver and ultrasound-based anatomy training courses. Thus, it is more important than ever to have a thorough grounding in this exciting subspecialty.

The much-anticipated revision of the second edition of The SAGES Manual: Fundamentals of Laparoscopy, Thoracoscopy, and GI Endoscopy, has been completely restructured, reorganized, and revised. The Manual has been split into two volumes for better portability. Volume I, Basic Laparoscopy and Endoscopy covers the fundamentals and procedures performed during surgical residency. Volume I will be the first volume used by students, residents, and allied healthcare professional trainees. Material has been added to these fundamentals and procedures that will also be of interest to experienced surgeons. Volume II, Advanced Laparoscopy and Endoscopy covers more advanced procedures, generally taught during fellowship. All of the sections have been reorganized with a critical eye to the needs of the modern minimal access surgeon. Two new editors have been added. Chapters have been revised by both new authors as well as many stalwart authors from previous editions. These portable handbooks cover all of the major laparoscopic and flexible endoscopic procedures in easy-to-read format. Indications, patient preparation, operative techniques, and strategies for avoiding and managing complications are included for the complete spectrum of both "gold standard" and emerging procedures in diagnostic and therapeutic laparoscopy, thoracoscopy, and endoscopy. The scope, detail, and quality of the contributions confirm and demonstrate the SAGES commitment to surgical education. This manual is sure to find a home in the pocket, locker or briefcase of all gastrointestinal endoscopic surgeons and residents.
Cancer surgery of the mouth and throat can involve major incision wounds, tracheotomy, or necessitate the breaking of bones. Recognizing the benefits and success of robotic surgery in other areas of medicine, the authors present its use in head and neck surgery, resulting in less surgery, less deformity, and faster rehabilitation. In this new book, the authors describe the preoperative aspects of Transoral Robotic Surgery (TORS), including patient evaluation, instrumentation and set-up, moving on to diagnostic endoscopic and surgical techniques for tonsillectomy, laryngectomy, tongue base and glottic resection, and speech and swallowing considerations. They conclude with an examination of the potential for TORS in skull-base surgery, its future application and, importantly, telesurgery via the Internet. A pioneering book for a pioneering surgical technique in head and neck surgical technique and management, this should be on the shelf of every otolaryngology-head and neck surgeon. http://www.youtube.com/watch?feature=player_embedded&v=DTp7Mv9hkbI

The struggle to perform well is universal, but nowhere is this drive to do better more important than in medicine. In his new book, Atul Gawande explores grippingly how doctors strive to close the gap between best intentions and best performance in the face of obstacles that sometimes seem insurmountable. His vivid stories take us to battlefield surgical tents in Iraq, to a polio outbreak in India, and to malpractice courtrooms around the country. He discusses the ethical dilemmas of doctors' participation in lethal injections, examines the influence of money on modern medicine, and recounts the astoundingly contentious history of hand-washing. And he gives a brutally honest insight into life as a practising surgeon. Unflinching but compassionate, Gawande's investigation into medical professionals and their progression from good to great provides a detailed blueprint for success that can be used by people in every area of human endeavour.
