Although lymphoscintigraphy was originally introduced into clinical routine for identification of the cause of peripheral edema, more recently it has been widely applied for radioguided biopsy of the sentinel lymph node in patients with solid cancers. The procedure is now considered crucial for adequate planning of oncologic surgery in a growing number of cancers. This atlas presents a collection of richly illustrated teaching cases that demonstrate the clinical relevance and impact of lymphoscintigraphy in different pathologic conditions. The first laparoscopic cholecystectomy, performed by Prof. Philippe Mouret in 1987 and described by himself in the first chapter of this book, was an event that revolutionized surgery in the past few decades. Although the majority of surgeons today are unfamiliar with the his- ry of early minimally invasive surgery developments, it is important to realize that the advent of laparoscopy led not only to new surgical te- niques, but also to a change in the doctrine of medical care, by streng- ening the concept of minimal invasiveness. This is particularly the case for biliary lithiasis, for which laparoscopy has provided major benefits in terms of both diagnosis and surgical tre- ment. However, our understanding of the etiology of this disease, as well as the availability of effective medical therapies, have remained subst- tially unchanged over time. The application of the innovative approach has not been, in routine practice, without problems: between 1990 and 2000, the rate of serious complications from laparoscopic cholecystectomy were four times higher than those observed with open surgery. Even initial attempts at lapa- scopic treatment of biliary common bile duct stones resulted in excessive complications and associated medico-legal problems. Nonetheless, today, most indications have been stated, techniques have been standardized, and a new aspect to the treatment of biliary lithiasis has appeared.
Minimally invasive joint replacement has become one of the hottest topics in the orthopedic world. In the case of the knee, however, this concept has been widely misunderstood. All too frequently it is taken to mean keyhole surgery in which traditional components are implanted with a shorter surgical time. Such an approach in fact has few benefits and various possible dangers. This book provides an in depth review of the current practice in treating elderly patients with coronary artery disease (CAD) as well as an update of the existing literature. Since the introduction of laser devices to the medical sciences this technology has created great interest. Specifically, the laser's unique physical properties and precise bio-tissue interactions render this versatile source of biologic energy an attractive tool for multiple therapeutic purposes in cardiovascular medicine. Over the course of the last 2 decades the utilization of laser technology has become an important component for the management of patients with complex cardiovascular diseases. During this time period, cutting edge laser technology including a variety of wave length generators, newly designed catheters, and a selection of advanced optic fibers have been introduced and applied in the cardiovascular circulation.
