There are many superb books on how to do operations but there are few or none on how to assist at them, and none written either by or for medical students or doctors. Therefore, the skills that make an expert surgical assistant are difficult to acquire. Normally, they can only be learnt in a haphazard way, by spending years in the operating theatre. Silicone prosthesis-based breast augmentation has shown year-on-year increases in popularity in both the USA and UK, but as surgical and implant technologies improve, adverse capsular contracture (ACC) has become a key determinant of outcome. ACC is the term given to the abnormal soft tissue reaction to a foreign body leading to constrictive fibrosis. Constrictive fibrosis refers to the poorly understood phenomenon by which the capsule that normally envelops an implant shrinks in size. The formation of a capsule around any foreign material entering the body is a normal response of the organism, which is not normally pathological in nature. This book discusses this adverse capsule and its role in breast augmentation.

Tissue is frequently damaged or lost in injury and disease. There has been an increasing interest in stem cell applications and tissue engineering approaches in surgical practice to deal with damaged or lost tissue. Tissue engineering is an exciting strategy being explored to deal with damaged or lost tissue. It is the science of generating tissue using molecular and cellular techniques, combined with material engineering principles, to replace tissue. This could be in the form of cells with or without matrices. Although there have been developments in almost all surgical disciplines, the greatest advances are being made in orthopaedics, especially in cartilage repair. This is due to many factors including the familiarity with bone marrow derived mesenchymal stem cells and cartilage being a relatively simpler tissue to engineer. Unfortunately significant hurdles remain to be overcome in many areas before tissue engineering becomes more routinely used in clinical practice.

It is only recently that the use of the endoscope as the sole visualizing tool has been introduced in transsphenoidal pituitary surgery with its favorable related implications and minimal operative trauma. Of course, microscopic and endoscopic anatomy are basically the same, but the optical distorsion of endoscopic images is quite substantial compared to microscopic depictions. An endoscope lens produces images with maximal magnification at its center and severe contraction at its periphery. Nearer images are disproportionally enlarged and remote images are falsely miniaturized. This optical illusion may disorientate a surgeon who is not familiar with this peculiar condition at the skull base. Robotics is one of the hottest topics in medicine today, with an international interest that is exponentially growing. The introduction of robotic technology into modern operating theatres has provoked a revolutionary change in the basic surgical approach, with many advantages over traditional open surgical treatment, including faster recovery and a significantly lower risk of surgical trauma. While the benefits of minimally invasive surgery are apparent, the expansion of laparoscopic surgery throughout the field has been relatively slow due to the steep learning curve and the level of practice and specialism required to perform such procedures.
