In the Federal Republic of Germany today there are 6 to 8 million hypertensive patients. Of them, 3 to 4 million show organic cardiac manifestations of hypertension. Of all deaths occurring before the age of 65, 40% are attributable to hypertension, and the total mortality from hypertension is about 25%. The high cardiac morbidity and mortality potential of the risk factor 'hyper tension' lies in the development of cardiac hypertrophy, heart failure and coronary artery disease. In addition, hypertensive patients are predisposed to develop secondary cardiac diseases. The aim of the present study was to analyse the myocardial function and coronary haemodynamics in essential hypertension, i. e. the most common type of pressure load imposed on the human heart. Consequently, the function and mode of operation of the hypertensive heart are described - a type of hypertrophy of the human heart which to date has almost been ignored in pathophysiology and diagnostics. On the basis of the findings, conclusions are drawn for the differential diagnosis and therapy of the hypertensive heart accompanied by hypertrophy, heart failure and coronary artery disease. In the last decade, ACE inhibitors have become a cornerstone of cardiovascular drug therapy. Beneficial effects in patients with hypertension, heart failure, and post myocardial infarction have been established in large clinical trials. In parallel, ex perimental research has shifted the interest from the immediate homeostatic function of the circulating renin-angiotensin-system to the multiple paracrine functions of local tissue renin-angiotensin-systems, including hypertrophy, hyperplasia, athe- oenesis, and vasomotion. . It was the aim or a symposium held in Hamburg, October 22-24, 1992, to bring together an international panel of experimental and clinical scientists and to discuss the most recent findings on the role of angiotensin and associated mediators in car diovascular adaptation and disease, with a particular emphasis on the treatment of such disease by ACE inhibitors. Hypertensive heart disease is one of the most frequent diseases in man. It is esti mated that there are six to eight million hypertensive patients in the Federal Re public of Germany today. This corresponds with an incidence of 10%-15% in the whole population. Approximately between three and four million show organic eardiac manifestations of hypertension. Of all deaths occurring before the age of 65,30%-40% are attributable to hypertension, and the total morta lity from hypertension is about 20%-25%. Recent epidemiological and clini cal studies have revealed similar figures for other countries, such as the United States and Japan. Hypertensive hypertrophy and cardiac failure are found to be many times more frequent than eardiae hypertrophy and failure of any other origin. Despite the apparent clinical and social importance of the biologieal system "blood pressu re and the heart," data on eardiac performance, on systemic and coronary haemodynamics and on the morphological, bioehemical and contractile proper ti es of ventrieular heart muscle in both experimental and clinical hypertension are rare. This is the reason and stimulus for having organized this symposium, which is the first one coneeming this topie."

Der arterielle Bluthochdruck ist die haufigste Form der Druckbelastung des linken Ventrikels. Der Hochdruck selbst gilt als einer der wichtigsten Risikofaktoren der koronaren Herzkrankheit. In dieser 3. Neuauflage werden erstmals systematische Untersuchungen zur Hypertrophieregression des Ventrikelmyokards sowie zur Beeinflussung der hypertensiven Mikroangiopathie dargestellt. Daruber hinaus analysiert der Autor die Probleme der Inzidenz von Herzrhythmusstorungen bei hypertensiver Herzhypertrophie, der diastolischen Eigenschaften des linken Ventrikels, der stummen Myokardischamie sowie der Rheologie des Koronarkreislaufs. Die Neuauflage berucksichtigt die Umsetzung der neuesten wissenschaftlichen Erkenntnisse in der Therapie des Hochdruckherzens."

In den letzten Jahren haben Hemmstoffe des Konversionsenzyms bei der Indikation hoher Blutdruck, Herzinsuffizienz und bei Zustanden nach Myokardinfarkt eine Dif- ferentialtherapie erlaubt, die in ihren Ergebnissen bemerkenswert ist und uber eine Organ protektion zur Senkung der Mortalitat dieser letalen kardiovaskularen Erkrankungen beigetragen haben. Experimentelle Untersuchungen haben die Bedeutung der organstandigen Renin-Angiotensin-Systeme hervorgehoben und neue Aussichten zur Behandlung der Hypertrophie des Herzens und der Arteriosklerose der Gefasse eroeffnet. Im hier vorliegenden Band, in englisch erschienen als Supplement der Zeitschrift Basic Research in Cardiology, werden die Vortrage eines Internationalen Sym- posiums in Hamburg vom 22. bis 24. Oktober 1992 in deutscher UEbersetzung zusammengefasst. Der interdisziplinare Charakter des Symposiums kommt in ex- perimentellen und klinischen Untersuchungen zum Ausdruck. Die Herausgeber hof- fen, dass das Symposium "Angiotensin and the Heart" sich stimulierend auf die Grundlagenforschung und die Behandlung von kardiovaskularen Erkrankungen mit Hemmstoffen des Konversionsenzyms auswirkt.
