It has by now been established that carpal ligamentary lesions may lead to instability and ultimately to carpal arthritis. However, the variety of anatomopathological classifications and the multiplicity of surgical repair techniques reflect the difficulty of grasping this ligamentary pathology. Clinical analysis and traditional investigation techniques must successfully address the complexity of the wrist, which is largely due to the number of synchronized bones involved in every movement of the hand. The clinical analysis of the wrist is insufficient to precisely measure the mobility of each carpal bone, making paraclinical examinations also significant for clinical diagnosis. The tools available today range from X-rays to arthroscopy, including arthroCTscans and soon MRI. As for repair techniques, their diversity must not hide the fact that a biomechanical and physiological principle is involved and must be identified. This book presents the findings of an expert group in the field of wrist pathology. It seeks to analyze, understand, explain and make comprehensible, if not obvious, the reflections that each practitioner should employ during the diagnosis and treatment of carpal ligamentary lesions. The focus is consciously not on degenerative carpal pathology, which usually enjoys more visibility. In recent years wrist problems have increasingly attracted the attention of orthopaedic and hand surgeons. An international symposium was held at the Nagoya Castle Hotel, Nagoya, Japan from March 6th through March 8th, 1991 to further understanding and promote discussion of wrist problems among a representive international group. Approximately 300 participants from 16 different countries assembled for these discussions. This monograph consists of 40 selected papers based on presentations given at the international symposium. ince 1985, it has been my privilege to be the Director of The - ciety of Byron Smith Fellows. During the years in which Byron SSmith was with us, his former fellows would meet at the Ame- can Academy of Ophthalmology's annual meeting. They would present to him, and to one another, interesting cases of their own compli- tions that had occurred or cases that had been sent to them that were complications to be repaired. This forum provided the fellows with an excellent format for evaluating their own problems and also for d- cussing how to treat extremely difficult cases. Shortly after Dr. Brian Brazzo completed his fellowship with me in 1998, he asked if I thought it would be appropriate for him to edit a new book on complications of ophthalmic plastic surgery. Certainly, during his fellowship, it became apparent that Dr. Brazzo was pr- ably the most gifted academician and writer that I had ever had the pleasure of training and meeting. I could think of no one more - propriate to continue the work initiated by Dr. Byron Smith and c- ried on by his students.

Obstetric fistula is as old as mankind. While the incidence has diminished progressively with better health care in Western societies, the situation has changed little in many developing countries. Fistulae of pelvic organs, often monstrous defects, still are a major complication of child-birth causing misery to uncounted young women, and if they cannot find help in one of the very few hospitals with trained specialists, they became urological cripples losing everything: family, home and job. The magnitude of the problem is illustrated by some figures given by Reginald and Catherine Hamlin-about 700 fistula patients treated each year-a total of over 10,000 cases operated upon in their fistula hospital in Addis Ababa, Ethio pia. Most of these injuries could be prevented by better health care at the village level as some studies have shown conclusively. The incidence of fistula is an indicator of the standard of health and obstetrical care. The author of this book-Obstetric Fistula-is an internationally known Australian gynaecologist who for many years has been interested in all aspects of gynaecological urology, especially urinary stress inconti nence, other forms of involuntary loss of urine, and associated gynaeco logical conditions. He has devised a number of new operations to treat pelvic defects. Robert Zacharin's interest in obstetric fistula was a con sequence of his surgical activity in developing countries."

This monograph presents a comprehensive review of the clinical experience in surgical repair of cranial defects which the author has gained during a period of over ten years. Particular problems of patients undergoing cranioplasty, such as neurological impairments, EEG changes, variations of intracranial pressure, and sinking skin flap syndrome are described and discussed. The author presents convincing evidence of the importance of cranioplasty in improving the quality of life of patients with large and disfiguring cranial defects. Unfortunately, cranioplasty still ameliorates only to a minimal extent the general condition in patients suffering of major cerebral lesions. I am convinced that this volume will serve the purpose it was designed for : that it will be a most helpful introduction into the problems related to reconstructive surgery. Fang Zhi-yang 1. 1 History of Traditional Treatment of Burns in China China is a country of ancient culture. The treatment of burn injury was documented more than 1500 years ago. As early as the Eastern Jin Dynasty, the Handbook of Prescriptions for Emergencies written by Ge Hong (281-341 A. D. ) and in the Southern Qi Dynasty (479-502 A. D. ) Liu J uan-Zi's Remedies Left Over by Ghosts, a book dealing mainly with diagnosis and treatment of traumatic wounds, are the earliest known monographs on "external diseases" in China. They record the treatment of burns with topical agents such as the preparation of a lard ointment of the liquid extract of Angelica anomalea Lallem, Gardenia jasminoides Ellis, Glycyrrhiza uralensis Fisch, Radia salviae miltiorrhizae, and the white cortex of willow. These herbs are found to be astringents and antibiotics. During the Sui Dynasty, a doctor of the Imperial Medical Academy (550- 630 A. D. ), Chao Yuan-fang, was authorized by imperial decree to compile the book General Treatise on the Etiology and Symptomatology of Diseases (610 A. D. ), the first book on etiology and symptomatology in China. This book with its valuable descriptions has been an authoritative and important reference for medical students for more than 1000 years.

The introduction of the operating microscope as a surgical tool revolutionized the treatment of peripheral nerve lesions. A new era thus began in the early 1960s, which led to a substantial improvement in the management of nerve lesions. The results of nerve grafting techniques have demonstrated that, independent of the length of the defect, lesions can be successfully bridged. The free tissue transplants with microvascular anastomosis have also opened new, rewarding possibilities for peripheral nerve reconstruction procedures, facilitating the achievement not only of satisfactory anatomical but also of satisfactory fUhctional results. In order to evaluate the state of the art and reflect retrospectively on 25 years of microneurosurgical treatment of peripheral nerves, numerous outstanding scientists and clinicosurgical physicians were invited to Hanover in order to exchange their viewpoints and experiences.
