Over the years, interventional radiology has developed many effective and less invasive procedures. As the number and types of interven tions increase, radiologists are becoming more and more involved in clinical patient care. This includes pre-interventional patient workup, sophisticated medical therapy and monitoring during interventions, regular inpatient rounds and complete follow-up management. Therefore interventional radiologists are faced with a broad spectrum of clinical and pharmacological questions. Adjunctive medical therapy should increase the patient's comfort, should improve the success rates, and should further reduce the risks of the procedures. In order to fulfill our responsibility to the patients, a profound knowledge of certain drugs is indispensable. This in cludes, for example, sedatives, analgesics, cardiovascular drugs and agents to prevent infections, thromboembolic complications or restenoses. Moreover, a good monitoring system during complex procedures will increase the safety of radiological interventions. These topics and several more have been presented and discussed during an international symposium in Basel, Switzerland, in January 1992. It is the benefit of the authors, that all these results could be published shortly after the event. Connected with our thanks to all the co-workers the editors hope to find this publication assisting an increasing number of safe interventions.

As an addition to the European postgraduate training system for young neurosurgeons we began to publish in 1974 this series of Advances and Technical Standards in Neurosurgery which was later sponsored by the European Association of Neurosurgical Societies. This series was first discussed in 1972 at a combined meeting of the Italian and German Neurosurgical Societies in Taormina, the founding fathers of the series being Jean Brihaye, Bernard Pertuiset, Fritz Loew and Hugo Krayenbiihl. Thus were established the principles of European co operation which have been born from the European spirit, flourished in the European Association, and have throughout been associated with this serIes. The fact that the English language is well on the way to becoming the international medium at European scientific conferences is a great asset in terms of mutual understanding. Therefore we have decided to publish all contributions in English, regardless of the native language of the authors. All contributions are submitted to the entire editorial board before publication of any volume. Our series is not intended to compete with the publications of original scientific papers in other neurosurgical journals. Our intention is, rather, to present fields of neurosurgery and related areas in which important recent advances have been made. The contributions are written by spe cialists in the given fields and constitute the first part of each volume.

New imaging technology and more sophisticated image processing systems will have a profound effect on those areas of medicine which are concerned with imaging for diagnosis and therapy planning. Digitally formated data will form the basis of an increasing number of medical imaging modalities. Before the diagnostic imaging department of the future will largely be digital, many problems have still to be solved as regards image quality, costs, and ease of use. The computer and other information science derived methods will contribute towards solving many of the problems in these areas. It is widely expected that there will be an information science derived evolution in imaging for radiology and related departments. Computer assistance may be applied to image generation, e.g. CT, MRI, DR and DSR, storing and transferring of images, and viewing, analysing and interpreting of images. The application of computers to these activities (which characterise radiological departments), may be defined as Computer Assisted Radiology (CAR) . In the main, CAR will promote the transition from analog imaging systems to digital systems, integration of digital imaging modalities through Picture Archiving and Communication Systems (PACS') and the graduated employment of Medica~ Work Stations (MWS) for diagnosis and therapy planning. It will transfer geographically, organisationally and/or mentally isolate imaging activities towards fully integrated multi-imaging modality diagnostic departments. This development will have a considerable impact on patient management, on the medical profession and on the health care system.

Regular physical exercise is associated with substantial health benefits. Recent evidence not only holds for cardiovascular effects promoting "physical health", but also for the central nervous system believed to promote "brain health". Moderate physical exercise has been found to improve learning, memory, and attentional processing, with recent research indicating that neuroprotective mechanisms and associated plasticity in brain structure and function also benefit. Physical exercise is also known to induce a range of acute or sustained psychophysiological effects, among these mood elevation, stress reduction, anxiolysis, and hypoalgesia. Today, modern functional neuroimaging techniques afford direct measurement of the acute and chronic relation of physical exercise on the human brain, as well as the correlation of the derived physiological in vivo signals with behavioral outcomes recorded during and after exercise. A wide range of imaging techniques have been applied to human exercise research, ranging from electroencephalography (EEG), magnetoencephalography (MEG), near infrared spectroscopy (NIRS), magnetic resonance imaging (MRI) to positron emission tomography (PET). All of these imaging methods provide distinct information, and they differ considerably in terms of spatial and temporal resolution, availability, cost, and associated risks. However, from a "multimodal imaging" perspective, neuroimaging provides an unprecedented potential to unravel the neurobiology of human exercise, covering a wide spectrum ranging from structural plasticity in gray and white matter, network dynamics, global and regional perfusion, evoked neuronal responses to the quantification of neurotransmitter release. The aim of this book is to provide the current state of the human neuroimaging literature in the emerging field of the neurobiological exercise sciences and to outline future applications and directions of research.

- Donation refusal is high in all the regions of Argentina. - The deficient operative structure is a negative reality that allows inadequate donor maintenance and organ procurement. - In more developed regions, there are a high number of organs which are not utilized. This is true for heart, liver and lungs. Small waiting lists for these organs probably reflect an inadequate economic coverage for these organ transplant activities. - There is a long waiting list for cadaveric kidney transplants, which reflect poor procurement and transplant activity. - Lack of awareness by many physicians leads to the denouncing of brain deaths. In spite of these factors, we can say that there has been a significant growth in organ procuration and transplantation in 1993, after the regionalization of the INCUCAI. Conclusions Is there a shortage of organs in Argentina? There may be. But the situation in Argentina differs from that in Europe, as we have a pool of organs which are not utilized (donation refusal, operational deficits, lack of denouncing of brain deaths). Perhaps, in the future, when we are able to make good use of all the organs submitted for transplantation, we will be able to say objectively whether the number of organs is sufficient or not.
