In this report, the Commission provides updated guidance on radiological protection against radon exposure. The report has been developed considering the latest ICRP recommendations for the system of radiological protection, all available scientific knowledge about the risks of radon, and the experience gained by many organisations and countries in the control of radon exposure. The report describes the characteristics of radon exposure, covering sources and transfer mechanisms, the health risks associated with radon, and the challenges of managing radon exposure. The Commission recommends an integrated approach for controlling radon exposure, relying as far as possible on the management of buildings or locations in which radon exposure occurs, whatever the use of the building. This approach is based on the optimisation principle, and is graded reflecting the responsibilities of key stakeholders, notably in workplaces, and the intent of the national authorities to control radon exposure. The report also provides recommendations on managing radon exposure when workers' exposures are considered as occupational, and the appropriate requirements of the Commission should be applied.

This volume contains the proceedings of a symposium held in Marseille on April 6, 1992, on the topic "Heterogeneity of Alzheimer's disease. " This was the eighth of a continuing and very successful series of meetings related to Alzheimer's disease organized by the Fondation Ipsen pour la Recherche Therapeutique. These symposia, known as "Colloques medecine et recherche," started in 1987 and have dealt with widely different aspects of the disease such as immunology, genetics, neuronal grafting, biological markers, imaging, growth factors, and last year's less conventional topic of Neurophilo- sophy and Alzheimer's disease. The next IPSEN symposium dedicatet to Alz- heimer's disease will take place in Lyon on June 21, 1993, and will deal with "Amyloid protein precursors in development, aging, and alzheimer's disease. " It is being organized by Konrad Beyreuther, Colin Masters, Marc Trillet, and Yves Christen. Until a few years ago, several names were used to refer to the most common cause of dementia in the elderly. They included such terms as "senile psycho- sis," "organic brain syndrome," and "senile dementia. " Following Kraepelin, the term "Alzheimer's disease" was often restricted to an uncommon condition starting at a younger age (before 60 or 65 years of age).

Rapid developments of new investigative techniques and diagnostic modalities such as CT and MRI have revolutionized modern diagnostic imaging. Dr. Sperber and her contributors expertly facilitate understanding of the fundamentals of pulmonary disorders and promote a realistic and appropriate use of the exciting new diagnostic modalities. Methodologies of choosing techniques are also discussed. The first section of the book reviews the applications of the various radiographic modalities. The second part is organized by disease - emphasizing the radiographic manifestations and indicating appropriate imaging techniques most useful for diagnosis.

Following the issuance of new radiological protection recommendations in Publication 103 (ICRP, 2007), the Commission released, in Publication 110 (ICRP, 2009), the adult male and female voxel-type reference computational phantoms to be used for the calculation of the reference dose coefficients for both external and internal exposures. While providing more anatomically realistic representations of internal anatomy than the older stylised phantoms, the voxel phantoms have their limitations, mainly due to voxel resolution, especially with respect to small tissue structures (e.g. lens of the eye) and very thin tissue layers (e.g. stem cell layers in the stomach wall mucosa and intestinal epithelium). This report describes the construction of the adult mesh-type reference computational phantoms (MRCPs) that are the modelling counterparts of the Publication 110 voxel-type reference computational phantoms. The MRCPs include all source and target regions needed for estimating effective dose, even the m-thick target regions in the respiratory and alimentary tract, skin, and urinary bladder, assimilating the supplemental stylised models. The MRCPs can be directly implemented into Monte Carlo particle transport codes for dose calculations, i.e. without voxelisation, fully maintaining the advantages of the mesh geometry.

Synchrotron radiation has important applications in medical imaging, especially in such areas as intravenous coronary angiography, mammography, bronchography, and monochromatic computed tomography. In medicine, phase contrast imaging is a particularly exciting development, with the potential to become a new X-ray eye in the 21st century. To discuss these and related topics, medical doctors met with physicists and other researchers in Haga, Japan, in August 1997. This volume contains the proceedings of the International Workshop on Medical Applications of Synchrotron Radiation (HAGA 97), where practitioners from the world of medicine could communicate directly with researchers from the world of physics to discuss practical and theoretical aspects of medical imaging with synchrotron radiation. In the past few years there have been impressive advances in the implementation of modern MRI techniques for the assessment of MS patients with the ultimate goal to define MRI markers of MS evolution. Over 10 years have passed since the first edition of The Mediastinum was published in 1977. I have been very gratified by the response to the first edition and determined to do a second edition as soon as possible. However, good intentions are sometimes difficult to achieve and a decade has passed. This period has been one of enormous growth in the discipline of diagnostic imaging. In the study of the mediastinum, computed tomog raphy, and more recently magnetic resonance, have revolutionized our diagnostic capabilities. This second edition of the mediastinum is in tended to emphasize the importance of these modalities to the evalua tion of mediastinal disease. In addition, an attempt will be made to integrate into the text the many new and important observations relat ing to all aspects of mediastinal imaging which have appeared in the literature since 1977. The overall emphasis, however, will remain the same: that accurate radiologic diagnosis is based upon a thorough understanding of corre lated radiographic anatomy and pathology. The investigation of the brain by means of ultrasound has acquired increasing importance in the last years because it permits insight into the spatial relationships within the intact human skull in a short time without endangering the patient. The road from the first ultra sonic investigations on the exposed brain to the detection of intracranial midline shifts on the intact skull, the registration of echo pulsations and recently, to ultrasonotomography has been a long one already. However, this development is by no means at an end. Following the suggestion of numerous colleagues concerned with echo-encephalography in this country and abroad, the Neurosurgical Clinic of the University of Erlangen-Nuremberg organized an "International Symposium on Echo-Encephalography" on April 14th and 15th, 1967. Here there was an open exchange of experience on the results obtained up to the present. The limitations of the method and sources of error as well as the directions of future development of the ultrasonic echo procedure were discussed."

The contemporary management of patients with cancers of the head and neck is under careful scrutiny and major changes are being introduced in order to improve the potenti al not only for long-term control but also for less in the way of disfiguring and distres sing complications associated with the treatment programs. In 1988, the American Cancer Society estimates that there will be 42400 new cases of malignant tumors of the head and neck diagnosed with 12 850 deaths. In general, the prognosis for patients with malignant tumors of the head and neck re gion depends upon the site of origin, the local and regional extent of the tumor, the Kar nofsky status of the patient as well as the patient's general medical condition. The poten tial for cure for early stage tumors is extremely high particularly for those lesions involving the vocal cord, oral cavity, and the anterior two-thirds of the tongue. Major advances have been made in the management of head and neck cancer by the innovative utilization of surgery with radiation therapy. Small tumors can be cured by ei ther surgery or radiation therapy with equally good results. However, far advanced tu mors are more complicated and more difficult to cure requiring combined, integrated, multimodal programs of management. Therefore, the previously general poor prognosis for advanced tumors is becoming better with more aggressive treatment regimens."

At first glance it appears that little has happened in our understanding of bronchogenic carcinoma, since five year survival rates have not changed appreciably over the past ten years. This is partially true, however the depth of our understanding has increased and will continue to do so at a rapid pace over the next five to ten years. Information on the basic tumor biology, identification of important groups at high risk and im proved delivery of cytotoxic agents in the treatment of lung cancer, will all add to improve the outcome. The purpose of this text is to provide useful background information and to serve as a reference for approaching the patient with lung cancer. Therefore it will serve as a review for some and as a beginning for others. An important starting point in any discussion of lung cancer is an epidemiological survey of the topic (Chapter I). For those who do not avoid the hazards and present with symptoms, what is the most logical approach in determining the diagnosis (Chapter II). This chapter is intended to provide a general overview of the subjects covered in detail in the remainder of the text."

The aim of this work was not an exhaustive review of all pediatric echo graphic problems but a presentation of our daily experience with ultrasonography in pediatric practice. Several teams of pediatric radiologists from France and Belgium have combined their experiences to produce a practical book. The different authors have been chosen for their specific experience and their special area of work. The different chapters express therefore personal opinions. Primary reconstructive surgery is assuming increasing importance in the management of fractures of the spine. Analysis of the injury and thus surgical decision-making are greatly facilitated by the diagnostic power of CT and MRI. This volume provides a systemic introduction to the interpretation of CT and MRI images of injuries to the spinal column and the spinal cord, with special emphasis on the assessment or residual stability. Since survey X-rays remain the indispensable first step in radiodiagnosis, the typical appearances of spinal injuries on conventional films are also shown. This will help the reader interpret the CT and MRI images and also reflects the procedure in radiologic practice. The books classification of spinal fractures, together with the attempt to conclude how the injury happened by analyzing the damage caused, paves the way for individually oriented therapy.

There are two important reasons for publishing this new series entitled "Advances and Technical Standards in Neurosurgery" 1. the lack of any or ganized common European postgraduate training system for young neurosurgeons and 2. the language barriers, which impede the exchange of neurosurgical findings in Europe more than in other parts of the world. The fact that the English language is well on the way to becoming the international medium at European scientific conferences is a great asset in terms of mutual understanding. Therefore the Editors have decided to publish all contributions in English, regardless of the native language of the authors. All contributions are submitted to the entire editorial board before publi cation of any volume. Our series is not intended to compete with the publications of original scientific papers in other neurosurgical journals. Our intention is, rather, to present fields of neurosurgery and related areas in which important recent advances have been made. The contributions are written by specialists in the given fields and constitute the first part of each volume. In the second part of each volume, we publish detailed descriptions of standard operative procedures, furnished by experienced clinicians; in these articles the authors describe the techniques they employ and explain the advantages, difficulties and risks involved in the various procedures. Safety and Efficacy of Radiopharmaceuticals was established as a very important and comprehensive subject at the First Europe an Symposium on Radiopharmacy and Radiopharmaceuticals in Denmark in 1983. The interest in this subject has grown considerably since then due to the growing interest among national authorities to deal with radiopharmaceuticals. The introduction in recent years of nuclear medicine techniques based on radioactive labelled cells and on monoclonal antibodies has stressed the importance of a well functioning approval system for the clinical trial and use of new radiopharmaceuticals. The process of transferring the experience from the non radioactive drug field into the area of radiopharmaceuticals is still ongoing. International organisations such as the World Health Organisa tion is also including this into their quality assurance programme from both the radiopharmaceutical and the radiation hygiene point of view. In order to give an up-to date survey of these areas, experts were invited to prepare review papers under the following headings: Safety and Efficacy of Radiophar maceuticals with Emphasis on Biological Products, Radiophar macy/Radiation Hygiene, Legal Aspects of the Introduction of New Radiopharmaceuticals and some selected aspects of Good Radiopharmacy Practice."

Progress in traumatology of the vertebral spine has been restrained for a long time by two hindering factors. The first obstacle is presented by the differences in approach and a conflict of competences. The neurosurgeons, considering only the spinal chord, have confined themselves to indications for laminectomy, an insufficient and usually ineffective intervention. The orthopedic surgeons, on the other hand, obsessed by the fear of medullary lesions, have long hesitated to apply the fundamental rules for the treatment of fractures, namely precise reduction followed by strict immobilization, thus depriving themselves of the efficacy of radiculomedul lar decompression and of the protection this procedure affords to these structures when they are themselves involved in the trauma. Taking these facts into account, together with the rather poor results of laminectomy, the specialists have wisely and successfully recommended that one should abstain from treating the initial lesion, but rather attenuate the damage by appropiate nursing care and adequate reeducation. The second hindering factor was the insufficient knowledge of the extreme complexity of the anatomic lesions. This explains the orthopedists' relative caution; one only treats well what one knows well. More precise analysis of the lesions, not only of the bones but also of the joints, i. e., the osteofibrous involvement, is mainly based on strict radiologic semiology, which is rendered difficult because these structures are simultaneously affected. We are greatly indebted to Dr. DOSCH, radiologist at our Center, for having untertaken this difficult task, using all available conventional radiologic techniques."

The history of the use of ultrasound in medicine has been one of evolution of technology and innovative methods of applying this technology to imaging body structures. Many scientists and clinicians have contributed to this evolution. Ophthalmic ultrasound has become an indispensible tool in ophthalmic practice, with its own instrumentation and techniques. Ultrasound frequencies used in ophthalmology have generally been higher than those used in general medicine because of a requirement for higher resolution and a lesser need for deep penetration. Most ophthalmic diagnostic equipment uses frequencies in the 10 MHz range. The use of ultrasound frequencies in the 50-100 MHz range is a relatively new development in ultrasound imaging of the eye. This technique has been developed in our laboratories over the past several years. We have progressed from the theoretical description of the basic science required, past the first in-vitro experiments in eye bank eyes, to the construction of an instrument capable of clinical application. We have gained broad clinical experience with this instrument in normal patients and patients with ocular disease. A commercial instrument based on our original clinical scanner has recently become available, allowing other clinicians to apply this tool to ophthalmic research and clinical practice. We have applied the term ultrasound biomicroscopy to this new imaging technique because of similarities to optical biomicroscopy, i. e. , the observation of living tissue at microscopic resolution.

Die moderne Strahlentherapie ist eine auBerst komplexe MaBnahme. Es werden therapeutische Eingriffe vorgenommen, die sich sowohl in kurativer als auch palliativer Beziehung sehr niitzlich auswirken konnen, die aber auch, wie jede andere effektive Therapie, nachteilige Folgen haben konnen. Es handelt sich urn die Ausiibung einer arztlichen Kunst, die auf einer ganzen Reihe von Kenntnissen aufbauen muB, ohne welche die Behandlung nur Stiickwerk bleiben und unweigerlich zum Anstieg von MiB- erfolgen und sogar zu Schaden fiihren miiBte. Die vornehmlich kurative Behandlung bei den einzelnen Geschwiilsten wird in Band XIX und XX behandelt. Wie man beispielsweise die allgemeine von der speziellen pathologischen Anatomie trennt, so ist es auch sinnvoll, von einer allgemeinen und speziellen Strahlentherapie zu sprechen. Wir glauben, daB es sowohl niitzlich als auch notwendig ist, die allgemeine Strahlentherapie in einem eigenen Band getrennt zu be- arbeiten, weil eine Zusammenfassung nicht nur nicht existiert, sondern bei dem raschen Fortschritt un serer Kenntnisse einem realen Bediirfnis entspricht.

Mit den von anerkannten Sachkennern der Radiologie und der Frauenheilkunde erarbei- teten Beitragen fUr den Band XIII/2 des Handbuches der medizinischen Radiologie konnten erstmals in der Weltliteratur umfassende Zusammenstellungen des gesamten Wissensstof- fes auf dem Gebiet der gynakologischen Radiologie und der radiologisch-geburtshilflichen Diagnostik vorgelegt werden. Die Methodik und den Informationswert der Hysterosalpingographie hat FOCHEM abgehandeIt und gestiitzt auf die fundierten Ergebnisse eigener Arbeit, in klar gegliederten Kapiteln die morphologischen Rontgenbefunde des gesunden und kranken Uterus und der Tuben erlautert. Mit dem Beitrag tiber die gynakologische Rontgendiagnostik in der Padiatrie haben BENZ und WILLICH eine vollstandige Ubersicht der Ergebnisse ihres noch wenig bekannten Forschungsgebietes gegeben, das fUr viele Fachgebiete der Medizin groBe praktische Bedeutung erlangt hat. Die methodischen Grundlagen und den Informationswert der radiologischen Spezial- diagnostik in der Gynakologie haben GROTEMEYER und BREIT zusammengestellt. Einen grundlegenden Beitrag zur Anatomie und Topographie der Organe des weiblichen Bek- kens im Hinblick auf die Rontgen-Ganzkorper-Computer-Tomographie verdanken wir PLATZER, der das Fundament fUr die Bildanalyse dieser neuartigen diagnostischen Me- thode erarbeitet hat. Die spezielle Rontgendiagnostik der gynakologischen Tumoren mit Hilfe der Angio- graphie und Rontgen-Ganzkorper-Computer-Tomographie haben BREIT und ROHDE ab- gehandelt. In besonderen Kapiteln ist die oft vernachlassigte Rezidivdiagnostik bei Ge- schwiilsten von BREIT sowie LISSNER und SCHERER bearbeitet worden. Gestiitzt auf eigene Forschungs- und Entwicklungsarbeit geben die Autoren eine umfassende Ubersicht und berticksichtigen die wertvollen diagnostischen Informationen, die heute mit Hilfe der Rontgen-Ganzkorper-Computer-Tomographie fUr die klinische Arbeit zur VerfUgung stehen.

Der Umfang der Erkenntnisse uber das Mammakarzinom in Theorie, Klinik und Praxis ist in den vergangenen drei lahrzehnten so groB geworden, daB ein eigener Band zur Abhandlung dieses Themas erforderlich wurde. 1m Rahmen dieses Werkes ist es naheliegend, daB die radiologischen Disziplinen sowohl in der Diagnostik als auch in der Therapie besonders zu Wort kommen. Die Atiologie, die entsprechende Ethnologie, die Stochastik der Zellentartung, die Experimen- talmedizin, die Immunologie und die ausfUhrliche Chirurgie werden nicht abgehandelt, weil dadurch der Umfang des Buches wahrscheinlich mehr als verdoppelt werden wurde. Die Diagnose des Mammakarzinoms wurde zum einen durch die Mammographie und Thermographie erweitert und zum anderen durch die gezielte pditherapeutische Gewebsentnahme mit einer entscheidenden Histopathologie betrachtlich verbessert. Die fruhe Entdeckung eines Carcinoma in situ und die Differenzierung in ein lobulares, intraduktales oder infiltrierendes Karzinom bestimmen entscheidend das Schicksal der Frau. Die Frage nach der radikalen Mastektomie mit postoperativer Bestrahlung und der einfachen Mastektomie mit Bestrahlung konnen nur nach sorgfaltigen statistischen Unter- suchungen auf breiter Basis unter Berucksichtigung alterer und neuerer klinischer Erfah- rungen entschieden werden. Zur Vermeidung der oft sehr verstiimmelnden Operation sind die Kombinationsthera- pien, wie einfache Ablation des fortgeschrittenen Karzinoms oder Tumorektomie bei fruhen Karzinomen zusammen mit der Bestrahlung, in den Vordergrund getreten. Die Bedeutung der immer wieder diskutierten pdioperativen Bestrahlung und die Indikation dazu werden sorgfaltig besprochen.
