My practice life has spanned 36 years and during that time I have been involved in untangling countless mysterious maladies-or at least trying to do so. All of these efforts were without the benefit of any formal training about family systems. I am greatly encouraged by this book because it first draws attention to the intricate web that mankind has woven for itself. The family physician has often been caught up in this web, and therefore rendered impotent. Efforts to understand all of this are to be applauded. It has been my good fortune to know the editors, Leonard Roberts and Michael Crouch and, as a family physician, I feel that their "hearts are in the right place." They have grown up, medically speaking, in an era when society has become more complex, where life is not easy. Birth and its medical participants are suspect; childhood is complicated by divorce and loneliness; adolescence is a time of aimless searching; young adults are hard pressed to earn a living; the quality of life is being threatened somewhat by the overgrowth of high technology; dying with dignity is at a premium. The editors are to be commended for helping us clarify the role of the family physician in all of this.
Rick lelovsek is one of the "old timers" in computer medicine, developing and using computer systems for over 15 years. At Duke University, he developed one of the first computerized medical records for obstetrics and gynecology and collaborated with Drs. Stead and Hammond in the development of billing and accounting systems. A founding member and current officer of the American Association for Medical Systems and Informatics (AAMSI), he heads up and writes a quarterly newsletter for a special interest group in computers for the American College of Obstetricians and Gynecologists. He is a genuine (medical) "computernik." . I was asked as a favor to review his manuscript and suggest any needed changes before it was sent to a publisher. (He was not at that time planning to send it to Springer-Verlag.) Reading it was one of those pleasures I usually associate with a review article that finally explains those things about a subject that I was aware of but never really understood. The author's principal aim is to win over the patient through the development of the "positive" aspects of his psychopathology-a concern the significance of which I have also discovered, especially in schizophrenic therapy. It is in this specific sense that Peseschkian speaks of "positive" psychotherapy. His model is a notable synthesis of psychodynamic and behavior-therapeutic elements, making an essential contribution to unified relationships within psychotherapy. In this way Peseschkian is attempting not to directly confront the patient's resistances. The consultation takes place in a loving way through allusions to poetry, proverbs and oriental fairy tales and myths, to which Peseschkian, as a Persian, has direct access. His ability to offer his patient a great treasure of handed-down wisdom knows no bounds. Anyone who has personally experi enced the author's therapeutic enthusiasm and optimism will understand why this method of short psychotherapeutic procedure is highly successful in its effects. "The Elderly" is the third monograph in a series on management and treatment in major patient groups and subspecialties. Each book is complete in its own right. The whole series, however, has fill a gap between standard text-books of medicine been prepared to and therapeutics and research reviews, symposia and original arti cles in specialist fields. The aim of the series is to present up-to-date authoritative advice on patient management with particular refer ence to drug treatment. The first two volumes in the series, on gastrointestinal diseases and rheumatic diseases, were addressed to major therapeutic areas or subspecialties. The present volume, "The Elderly," is intended to provide overall guidance on the manage ment of a range of medical problems in elderly patients. The elderly make up an increasing proportion of the population and require a substantial proportion of health care resources. The management of elderly patients falls not only upon specialist geriatricians but also upon a range of others, including general practitioners, general physicians and almost all other medical specialists with the excep tion of paediatricians. It has become apparent in recent years that the management of disease in the elderly in general and drug treatment in particular presents new problems and challenges. Some of these relate to the wide spectrum of disease in elderly patients, in whom there is often multiple pathology, while others concern age-induced changes in drug handling or drug effect."
This work presents the knowledge and skills necessary for successful written communication in family medicine. It is intended for use by teachers of family medicine who, as part of their academic responsibilities, are called upon to produce written documents in a wide variety of areas. The book has also been written to serve as a resource for leaders presenting faculty development activities in various aspects of written communication, including writing for publication, administrative and educational communication, and other topics of interest to academicians. The Task Force on Professional Communication Skills was formed in 1981 as an initiative of the Board of Directors and the Communications Committee of the Society of Teachers of Family Medicine (STFM). In early meetings, the Task Force defined its goal as improvement of the communication skills-both written and oral-of STFM members. A survey of Task Force members revealed that the greatest challenges lay in the area of written communication skills, although the needs are not confined to medical article and book writing, but extend to the full range of academic communication. The Task Force set as its first task the creation of a monograph on written communication in family medicine.
The contributors to this volume, like many others concerned with medical education have for a long whilebeen conscious of the fact that in the training of medical students and in the practice of medicine too little attention is being paid to the psychological aspects of illness and to the use of psychotherapy in patient care. In an attempt to fill this gap medical students at University College Hospital, London, have since 1958 been given the opportunity on a voluntary basis to treat a pa tient with weekly analytically-orientated psychotherapy under supervision for a year or longer. In 1977 a similar scheme was started at the Psychosomatic Clinic of Heidel berg University. We were fortunate in obtaining generous financial support from the European Community which has enabled teachers and students from our two uni versities in Great Britain and the Federal Republic of Germany to collaborate in this student-psychotherapy teaching project and to compare our experiences. In this book we present an account of this joint educational endeavour, including our successes and our failures, as well as our attempts to solve some of the problems we have en countered. We decided to let individual supervisors and teachers who inevitably differed in their approach write their own contributions rather than try to present a unified pic ture which would not have given a fair impression of our work and experience. Ifthis has led to some repetition in the text we hope readers will appreciate the reason for it."
The book provides a summary of results from the 13th year of the BEACH program, a continuing national study of general practice activity in Australia. From April 2010 to March 2011, 958 general practitioners recorded details about 95,800 GP-patient encounters, at which patients presented 149,005 reasons for encounter and 146,141 problems were managed. For an 'average' 100 problems managed, GPs recorded: 69 medications (including 56 prescribed, seven supplied to the patient and six advised for over-the-counter purchase); 11 procedures; 23 clinical treatments (advice and counselling); six referrals to specialists and three to allied health services; orders for 30 pathology tests and six imaging tests. A subsample study of more than 31,000 patients suggests prevalence of measured risk factors in the attending adult (18 years and over) patient population were: obese - 27 per cent; overweight - 35 per cent; daily smoking - 15 per cent; at-risk alcohol consumption - 25 per cent. One in five people in the attending population had at least two of these risk factors. A companion publication, A Decade of Australian General Practice Activity 2001-02 to 2010-11 is also available.
