There are growing questions regarding the safety, quality, risk management, and costs of PCC teams, their training and preparedness, and their implications on the welfare of patients and families. "Surgical Education: Theorising an Emerging Domain" delineates surgical (as opposed to medical) education as a new and emerging field of academic enquiry. This reflects profound changes in healthcare training and practice on an international basis. As such, this book introduces, examines and explores the contribution of selected concepts and theories to surgical learning and practice. The first four chapters consider core facets of surgical education, such as simulation, while subsequent chapters take a key idea, often well known in another field, and examine its relevance to surgical education. Of course, performing invasive procedures is no longer the exclusive preserve of 'traditional' surgeons. Boundaries between surgery and the interventional specialties (radiology, cardiology, intensive care) are becoming increasingly blurred, especially as technology continues to expand. Changing work patterns and explosive technological development mark this out as a major growth area. New educational approaches (e.g. the use of simulation) are emerging. And all clinical practice is a team activity, where clinicians from many specialties (medicine, nursing, allied professions) come together with shared goals. For all the above groups, and their patients, education (teaching, training, learning and assessment) is of crucial importance. Yet the unique characteristics of surgical education have not previously been addressed from an educational perspective, nor have its possibilities as a new research domain been mapped. The domain needs to be theorised and its epistemological foundations established. There is thus both a need and a market for a definitive work in this area, aimed at surgeons, other clinicians, non-clinicians, educators, and others interested in this new domain.

Liver Transplantation: Challenging Controversies and Topics grew out of a need I perceived within the fields of transplant hepatology and liver transplantation. Liver transplantation has rightly gained recognition as an established therapy for end-stage liver disease. Few would argue that liver transplantation is one of the few truly lifesaving and life-altering treatments within medicine and surgery. Not many realize that 20 years passed from the time of the first human liver transplantation in 1963 to its acceptance as therapy by the 1983 NIH Consensus Conference on Liver Transplantation. In 2008, 25 years will have passed since the 1983 NIH conference-a mere 25 years for a field that has provided patients hope, doctors options, and to some the "gift of life. " Many issues in liver transplantation involve indications, patient selection, and outcomes after transplantation-these are standard topics, covered by textbooks of hepatology and transplantation. Nasal continuous positive airway pressure ventilation is the gold standard in the treatment of obstructive sleep apnea. Long-term compliance rates are about 60%. Therefore, several alternative treatment options are of special interest. Beside conservative therapies, various surgical concepts exist. The field of surgery for sleep disordered breathing has rapidly grown with new instrumentation and surgical techniques in the last 10 years. Surgeons in these fields have to attend scientific meetings, participate in workshops, and read the literature to stay up to date. In our sleep laboratories we conduct 30 polysomnographies each night. Each year, we perform almost 1,000 surgical procedures for sleep disordered breathing apart from numerous other conservative and apparative treatment modalities. Referring to our expe- ence and the present literature, we tried to give new information on surgical techniques in this second edition. The chapters are grouped in different anatomical fields of interest. We wanted to give general advice and specific new hints for the surgery of sleep disordered breathing so that the reader learns basic techniques followed by more advanced surgery. In addition to the illustrated surgical descriptions, the chapters contain informations about indications and contraindications of each surgical procedure and the postoperative care. Special interest has been dedicated to evidence-based medicine. So in each chapter, there is a table of references summarizing the effectiveness of the procedure and EBM grade.
With the growing age of the population, the proportion of elderly patients with diffuse coronary artery disease and severe comorbidities is increasing. The characteristics of these patients render them unsuitable for currently recognized revascularization procedures (CABG, PTCA). Therefore, there is an urgent need for new treatment modalities which could alleviate their symptoms and increase blood flow to the ischemic myocardium, while being less invasive for their fragile condition. The starting point for the treatment of hip disease depends on how we can preserve the natural hip joint and on steps leading to regeneration of the diseased, injured, or destroyed joint. Preservation and regeneration treatments following traditional and theoretical methods do not need to use expensive materials such as the artificial joints used in arthroplasty. On the other hand, these treatments are difficult to perform and require a long rehabilitation period. The result has been a move away from traditional methods, denying the surgeon experience and knowledge of the benefits associated with the most biologically appropriate treatment.
