This volume brings together the world's leading experts on urban and transport planning, environmental exposures, physical activity, health and health impact assessment to discuss challenges and solutions in cities. The book provides a conceptual framework and work program for actions and outlines future research needs. Of the 758 species of hard ticks (family Ixodidae) currently known to science, 137 (18%) are found in the Neotropical Zoogeographic Region, an area that extends from the eastern and western flanks of the Mexican Plateau southward to southern Argentina and Chile and that also includes the Greater and Lesser Antilles and the Galapagos Islands. This vast and biotically rich region has long attracted natural scientists, with the result that the literature on Neotropical ticks, which are second only to mosquitoes as vectors of human disease and are of paramount veterinary importance, is enormous, diffuse, and often inaccessible to non-specialists. In this book, three leading authorities on the Ixodidae have combined their talents to produce a summary of essential information for every Neotropical tick species. Congenital malformations are worldwide occurrences striking in every condition of society. These severe physical abnormalities which are present at birth and affecting every part of the body happen more often than usually realized, once in every 33 births. The most common, after heart defects, are those of the neural tube (the brain and spinal cord) which happen in as many as one in every 350 births. They have been noted as curiousities in man and beast throughout recorded history and received great attention in our time by various fields of study, for example, their faulty prenatal development by embryologists, familial patterns by geneticists, causation by environmentalists and variability by population scientists. Attention turned much in recent years to the relation of these malformations to deficiency of a particular dietary ingredient, folic acid, a subject this book analyzes in depth. The greatest conundrum of all, which this latest matter like so much else hinges on, is the amazing fact of the tremendous, almost universal decrease in the frequency of these anomalies since early in the 20th century. The most feared attribute of the human pathogen "Vibrio cholerae" is its ability to cause outbreaks that spread like wildfire, completely overwhelming public health systems and causing widespread suffering and death. This volume starts with a description of the contrasting patterns of outbreaks caused by the classical and El Tor biotypes of "V. cholerae." Subsequent chapters examine cholera outbreaks in detail, including possible sources of infection and molecular epidemiology on three different continents, the emergence of new clones through the bactericidal selection process of lytic cholera phages, the circulation and transmission of clones of the pathogen during outbreaks and novel approaches to modeling cholera outbreaks. Encephalitis lethargica ('sleeping sickness') was a mysterious disorder that swept the world in the decade following the First World War, before disappearing without its cause having been identified. Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease - attacking both motor and mental functions - appeared just as the separation of neurology and psychiatry had reached a critical point. Encephalitis lethargica sufferers presented an unprecedented combination of neurologic and psychiatric symptoms - including previously puzzling phenomena primarily associated with schizophrenia and hysteria, as well as behavioral changes and attention deficit disorders in children - that not only underscored the unity of mind and movement in the CNS, but also illuminated the critical role played by subcortical structures in consciousness and other higher mental functions that had formerly been associated with the soul and more recently presumed to be localized to the human cerebral cortex. Encephalitis lethargica exerted a greater influence on clinical and theoretic neuroscientific thought between the two World Wars than any other single disorder and had an enduring impact upon neurology and psychiatry. This book will be of interest to an educated audience active or interested in clinical (neurology, psychiatry, psychology) or laboratory neuroscience, particularly those interested in neuropsychiatry, as well as to those interested in the history of the biomedical sciences.

This book offers a radically different perspective on the topic of health inequity. Carey, Tai, and Griffiths use Perceptual Control Theory (PCT) to deconstruct current approaches to understanding, investigating, and addressing problems of health inequity. In the book, the authors propose that health inequity is not a problem per se. Disrupted control, they argue, is the problem that needs to be addressed. This book is the first one in which basic demographic models are rigorously formulated by using modern age-structured population dynamics, extended to study real-world population problems. Age structure is a crucial factor in understanding population phenomena, and the essential ideas in demography and epidemiology cannot be understood without mathematical formulation; therefore, this book gives readers a robust mathematical introduction to human population studies. In the first part of the volume, classical demographic models such as the stable population model and its linear extensions, density-dependent nonlinear models, and pair-formation models are formulated by the McKendrick partial differential equation and are analyzed from a dynamical system point of view. In the second part, mathematical models for infectious diseases spreading at the population level are examined by using nonlinear differential equations and a renewal equation. Since an epidemic can be seen as a nonlinear renewal process of an infected population, this book will provide a natural unification point of view for demography and epidemiology. The well-known epidemic threshold principle is formulated by the basic reproduction number, which is also a most important key index in demography. The author develops a universal theory of the basic reproduction number in heterogeneous environments. By introducing the host age structure, epidemic models are developed into more realistic demographic formulations, which are essentially needed to attack urgent epidemiological control problems in the real world.

In 1986, the emergence of a novel brain disease in British cattle presented a unique challenge to scientists. How that challenge was addressed has been the subject of a public inquiry and numerous academic studies conducted to date. However, none of these investigations has sought to examine the reasoning of scientists during this critical period in the public health of the UK. Using concepts and techniques in informal logic, argumentation and fallacy theory, this study reconstructs and evaluates the reasoning of scientists in the ten-year period between 1986 and 1996. Specifically, a form of presumptive reasoning is described in which extensive use is made of arguments traditionally identified as informal fallacies. In the context of the adverse epistemic conditions that confronted scientists during the BSE epidemic, these arguments were anything but fallacious, serving instead to confer a number of epistemic gains upon scientific inquiry. This book argues for a closer integration of philosophy with public health science, an integration that is exemplified by the case of scientific reasoning during the BSE affair. It will therefore be of interest to advanced students, academics, researchers and professionals in the areas of public health science and epidemiology, as well as philosophical disciplines such as informal logic, argumentation and fallacy theory and epistemology.

Almost a decade has passed since the first edition of this book was published and the explosion of new research in stroke is very apparent. Several important themes have coloured medicine during this time: the development of evidence based health care, new optimism for acute drug treatments for stroke, the growth of new information about the human genome, a dramatic increase in the number of stroke clinicians and researchers world-wide, and the establishment of the Cochrane Collaboration. All of these trends are relevant in revising a book of this nature. Clinical epidemiology remains an essential foundation for the practice of evidence-based healthcare. The enthusiasm of the pharmaceutical industry has resulted in a large number of new trials, often too small to detect clinically important differences, and have increased the number of clinicians actively engaged in stroke research. It is even more important that these clinicians should have a good understanding of the design of trials, the rationale for randomisation and blinding, and the importance of selecting the most appropriate outcomes. Surprisingly, little of practical importance has resulted from the decade of exploration of the human genome - the best indication of a person's risk remains their phenotype and not their genotype. The impetus to organise stroke medicine into a defined speciality, with emphasis on acute stroke, is even stronger - but it is essential to bear in mind the importance of interdisciplinary working, the value of primary care, geriatric medicine, psychiatry and rehabilitation in the prevention, treatment rehabilitation and long-term care of stroke patients. The Cochrane Collaboration is currently grappling with a task of enormous size - the compression of tens of thousands of randomised control trials -and other scientific evidence- into systematic reviews of the effects of interventions. Citations to this work are made in the section on management. In this edition, there is an extra opening section entitled Epidemiology which comprises the nuts and bolts of the subject and reviews the purposes of health care for stroke patients. The remaining sections follow the format of the first edition: Diagnosis, Management, and Prognosis. All chapters have been substantially updated. The book retains its focus on epidemiology that is relevant to the clinician and attempts to use examples from stroke to illustrate many aspects of epidemiological thinking. How do we draw the line,' he asks 'between "knowing" (the acquisition of new medical information) and "doing" (the application of that new knowledge). What are the long- term consequences (moral, social, economic, and biological) of responding to a demand that medicine always do everything that can be done?' This book now issued in paperback is a collection of critical and controversial essays discussing intractable ethical issues and evidence-based problems in modern medicine. The essays together with responses were published over a ten-year period in the journal Paediatric and Perinatal Medicine. Most of the examples, but not all are taken from perinatal medicine, the field in which the author has worked for many years. This volume collects essays by the late bioethicist John D. Arras, best known for his many contributions to the methodology of bioethics.
