Although attempts at radical removal of cancers of the rectum had been performed earlier, it was not until W. E. Miles in 1908 re ported his experience in the management of cancer in this ana tomic part, that the combined abdominoperineal resection be came the recognized and accepted approach for the treatment of this cancer. Miles reasoned that proctectomy removed the cancer, bearing a segment oflarge bowel but also the regional lymphatics into which the cancer spread, not only proximally, but laterally and distally as well. In his monograph in 1926 he stated: . . . Cripps war dabei angenehm uberrascht, trotz fehlender 2 Rekonstruktion bei uber j der Patienten anlasslich von Spat- 3 kontrollen gute Kontinenz zu finden. Die Moeglichkeit geriet aber in der Zwischenzeit weitgehend in Vergessenheit, und im "chirurgischen Armamentarium" hat sich eigentlich nur die parasakrale Rektotornie nach Kraske gehalten, welche den Sphinkterapparat moeglichst unbehelligt lasst. Es ist das Verdienst von Y ork Mason, die mannigfachen Moeglichkeiten des sphinkterspaltenden Zugangs erneut und uberzeugend dargelegt und auf die ausgezeichnete Kontinenz- funktion bei adaquater Rekonstruktion hingewiesen zu ha- ben. Nachdem wir uns von der Nutzlichkeit des sehr pragma- tisch durchgefuhrten Eingriffs uberzeugt hatten, schien es uns unbedingt erforderlich, die anatomischen Voraussetzungen eines moeglichst blutsparenden Vorgehens zu prazisieren und dieses Vorgehen in Beziehung zu setzen zu den heutigen Kenntnissen der normalen Kontinenz respektive Defaka- tionsfunktion. Herr Huber hat sich - beraten durch den Vorsteher des Instituts fur klinische Anatomie im hiesigen chirurgischen De- partement, Herrn Prof. A. von Rochstetter- anhand frischer Dissektionspraparate uber ein Jahr mit den Fragen dieses Zugangs befasst. Der V. Band der allgemeinen und speziellen Operationslehre, die Oto-Rhino- Laryngologischen Operationen, erscheint in diesen Jahren in der dritten Auflage. Er beginnt mit dem vorliegenden Band V/1, der die Operationen an der Nase und im Nasapharynx mit Berucksichtigung der transsphenoidalen Operationen an der Hypophyse und der Eingriffe am vegetativen Nervensystem des Kopfes abhandelt. Band V /2, der anschliessend erscheint, wird die Operationen an den Nasennebenhoehlen und der angrenzenden Schadelbasis beinhalten. Band V/3 mit der Darstellung der oto-rhino-larnygologischen Operationen im Mund-und Halsbereich (DENECKE) sowie Band V /4, die allgemeinchirurgischen Operationen am Hals (SCHWEMMLE), sind 1980 erschienen. Band V /5 wird die Operationen am Ohr und der zugehoerigen Schadelbasis enthalten. Diese Ausweitung des V. Bandes gegenuber seiner zweiten Auflage von 1953 wurde durch das Hinzukommen einer grossen Anzahl neu entwickelter Operatio- nen an den einzelnen Organen im Bereich von Hals, Nase und Ohr notwendig. Neue Erkenntnisse uber die Pathophysiologie und die Funktion der Organe sind in die entwickelten Oprationstechniken eingeflossen. Dadurch wurde eine voellige Neubearbeitung des Stoffes erforderlich. Als Co-Autor fur den vorliegenden Band V/1 konnte Professor WERNER EY, Darmstadt, gewonnen werden, der sich seit Jahrzehnten mit der Chirurgie der ausseren und inneren Nase besonders intensiv befasst hat. Bei der korrigierenden Rhinoplastik und bei den rekonstruktiven Verfahren haben die Autoren versucht, die neuen Erkenntnisse uber eine funktionelle Chir- urgie unter Berucksichtigung der Pathophysiologie der Nase herauszuarbeiten.

Seit der 1. Auflage gab es so wesentliche Neuerungen, vor allem in der Therapie, dass es notwendig war, das Werk fast voellig neu zu konzipieren und zu schreiben. Eine Wissenschaft hutet als wertvollstes Gut, was sich nach Bewahrung durchge- setzt und als Bestand erwiesen hat.
