Hernias - Signs / Symptoms, Management & Surgical Complications (Hardcover): Mark Suckow Hernias - Signs / Symptoms, Management & Surgical Complications (Hardcover)
Mark Suckow
R4,166 R3,898 Discovery Miles 38 980 Save R268 (6%) Ships in 12 - 19 working days

Patients affected with a hernia continue to represent a significant clinical population. Resulting from varied causes and associated with an array of pathophysiologies, a hernia can pose marked diagnostic and treatment challenges for the clinician. Sometimes sub-clinical and sometimes life-threatening, a hernia may require little or substantial medical intervention. This book consists of sixteen chapters by noted clinicians and scientists. Many chapters include background pathogenesis, typical symptoms experienced by patients, diagnostic measures, and therapeutic approaches. Current information is summarised and interpretations offered where appropriate. Basic pathogenic mechanisms and pathophysiology are considered in chapters such as those on the inheritance of a groin hernia (Burcharth and Rosenberg), and on alterations in arterial blood gases both before and after surgery (Tsounaki). These chapters make evident both the progress made in understanding the processes underlying hernias and related diseases, as well as the incomplete understanding we have with respect to such processes. Descriptions of clinical presentations, diagnostic approaches, and medical and surgical management are provided for several types of hernias, including inguinal, diaphragmatic, incisional, and Richter's. The utility of new methods for correction, such as laparoscopy and components separation, is described in the chapters by Maxwell and Baird, Marescaux et al, Mazzochi et al, and Sabino et al. These chapters reflect the technological improvements that have been brought to the successful treatment of hernias. It is likely that, with time, further definitions and refinements will additionally advance the understanding and approaches to hernias. The goal of this book is to summarise our understanding of basic concepts of hernia, at present, as a reference for scientists and clinicians.

This book highlights remarkable new endoscopic, laparoscopic, and thoracoscopic approaches to the removal of surgical lesions for different pathologic conditions under general endotracheal anesthesia in infants and children. It details how newer techniques in airway management, monitoring, regional nerve blocks for these innovative minimally invasive approaches have resulted in a decrease in intraoperative and postoperative morbidity and early recovery process after pediatric anesthesia. This unique book contains features that provide the audience with several clinical scenarios where exceptional surgical outcomes are achieved with optimum pre-op preparation via collaborative team efforts. To date there is no other textbook emphasizing the anesthetic and surgical management during the most innovative advances in endoscopic surgery in children. Per oral endoscopic myotomy (POEM) for the definitive treatment of achalasia in children is probably the most advanced endoscopic surgery done successfully in children. POEM is the best example of endoscopic surgery performed via natural orifices known as the Natural orifice transluminal endoscopic surgery (NOTES). Another new laparoscopic surgical intervention -Median Arcuate Ligament surgical release for Median Arcuate Ligament Syndrome (MALS) for patients with Postural Orthostatic Tachycardia Syndrome (POTS) is described with established perioperative protocols emphasizing the need for early admission, intravenous hydration, and premedication. Novel approaches in the anesthetic management in children with short bowel syndrome for bowel lengthening techniques like the serial transverse enteroplasty (STEP) and in teenagers for laparoscopic bariatric surgery with adjustable gastric banding (AGB) and vertical sleeve gastrectomy (VSG) for morbid obesity are new areas that would enlighten the readers. Anesthetic Management in Pediatric General Surgery is an invaluable resource for pediatric anesthesiologists, surgeons, and their trainees specializing in the care of pediatric patients.

For years the administrative of anesthesia was extremely dangerous and risky. Because of this the surgeon and anesthesiologist had to balance the risks and benefits for each patient before proceeding with surgery. In the last two decades the care of the surgical patient has changed dramatically. New equipment, monitors and pharmacologic agents have transformed surgical technique and improved outcomes. Patients once deemed "too sick" for the operating room are found frequently on operating room schedules nationwide. Today, anesthesiology for the healthy patient in most developed countries is extremely safe. However, perioperative complications still occur. These events can be catastrophic for patients and may have serious implications for residents, surgical and anesthesiology staff and nurses. Prompt recognition and management of these incidents may reduce or negate complications. This is based on a fundamental base of knowledge acquired through several avenues and practiced with other team members to maximize outcomes. Engagement of all caregivers impacts outcomes. Many organizations do not have the structural components or education to recognize or manage these catastrophic events. This textbook will provide educational material for the many students, as well as nurses, residents or attending physicians who participate in perioperative medicine. It will focus on the most serious perioperative complications and include a discussion of the pathophysiologic and pharmacologic implications unique to each. Additionally, it will provide medicolegal information pertinent to those providing care to these patients. All chapters will be written with the most current and relevant information by leading experts in each field. The layout and format is designed to be purposeful, logical and visually effective. Other features include review questions and answers, chapter summaries and shaded call-out boxes to facilitate learning. Catastrophic Perioperative Complications and Management will be of great utility for medical and nursing students, anesthesiology residents, student nurse anesthestists, surgical residents, nurses involved in perioperative medicine as well as surgical and anesthesiology attending physicians.

What's New in Hip Arthroscopy (Hardcover): Aprato Aprato What's New in Hip Arthroscopy (Hardcover)
Aprato Aprato
R3,466 Discovery Miles 34 660 Ships in 12 - 19 working days

Hip arthroscopy has seen significant developments in the last 25 years. Until a decade ago, it was considered an emerging surgical technique, but today it is an established and effective treatment for various hip pathologies. The development of hip arthroscopy is strictly linked with the evolution of the concept of femoroacetabular impingement. The description of this new pathology and the open surgical treatment described by Professor Ganz were actually the basis for the development of hip arthroscopy. Femoroacetabular impingement is, nowadays, the main indication for hip arthroscopy. Femoroacetabular syndrome should be regarded as a triad of symptoms, clinical signs and imaging findings. The goal of the treatment, in these cases, should not be simple anatomic correction but to obtain a hip without impingement in the normal range of motion. In addition, labrum, cartilage, capsule and soft tissues should be evaluated and lesions should be treated. In particular, the role of the labrum has been investigated during the last several years and several techniques for labral repair and labral reconstruction have been developed. Despite the progress in diagnosis and treatment of several hip pathologies, the exact indications of hip arthroscopy are still debated. Defining the limits for arthroscopic treatment in cases of advanced osteoarthritis, bone deformity and periarticular soft tissues pathologies is one of the goals of current investigations concerning hip arthroscopy. This book is intended as a guide to the innovation and development in hip arthroscopy over the last 10 years ranging from an update of the surgical indications to rehabilitation after hip arthroscopy. In every chapter, an expert in hip arthroscopy discusses a specific subject focusing on latest trends and surgical techniques. Attention is focused especially on surgical indications, patient positioning and approaches, labral and capsular management, ischiofemoral impingement management, extra-articular pathologies, trauma and finally post-operative rehabilitation.

A Comprehensive Guide to Esophagectomy (Paperback): Wade Nolan A Comprehensive Guide to Esophagectomy (Paperback)

R1,817 Discovery Miles 18 170 Ships in 12 - 19 working days

This guide first discusses the benefits of different anastomotic techniques, procedures for improving healing, methods to assess gastric conduit perfusion, and methods for the detection and management of the cervical anastomotic leak after esophagectomy. The authors review previous literature on minimally invasive esophagectomy, particularly reports that have focused on thoracoscopic esophagectomy with mediastinal lymph node dissection, describing recent advances in robot-assisted minimally invasive esophagectomy for esophageal cancer. The concluding study focuses on hybrid laparo-thoracoscopic esophagectomy, with the concept of widening the operation area of the laparoscopic transhiatal approach, reducing the operation area of the right thoracoscopic approach and eventually aiming for radical laparoscopic transhiatal esophagectomy without blunt dissection.

This atlas demonstrates how to perform each available extraperitoneal hernia repair via a set of high-quality annotated images showing step-by-step guidance on how to perform the surgery. Robotic extraperitoneal hernia procedures are considered great teaching procedures especially with a dual teaching console. The book bridges the gap between traditional hernia and laparoscopic hernia texts by combining both approaches to create a book with a unique visual approach. Preoperative, intraoperative, and postoperative figures are integrated to highlight the importance of these step-by-step procedures, enhance skill and efficiency, and avoid surgical pitfalls. Detailed descriptive figures accompany step-by-step instructions and include specific anatomical annotations that describe the anatomy and layers of the abdominal wall during hernia procedures. Robotic Hernia Surgery provides a comprehensive, insightful and state-of-art review of this field, and serves as a valuable resource for surgeons, surgeons in training, and students with an interest in hernia and robotic hernia surgery.
