This book explores the hospital via organisational ethnography (OE), an approach that involves a mix of fieldwork methods designed to analyse the hospital which also includes participatory observation, qualitative interviews and shadowing. One way to define a hospital is by its high level of formal organisation, resulting in written or digital communication as the main source of communication in patient journals, minutes and medical and quality guidelines. In contrast, in this book, the aspects of the informal organisation will be the focus. In spite of the many formal regulations of healthcare, hospitals are also chaotic organising places where many different groups of people interact in order to negotiate, to practice and to make sense of daily work tasks. The underlying argument is that, in the mundane everyday life of hospitals, frontline workers and their interactions with patients and local managers remain at the core of organising hospitals. The overall purpose of this book is to report stories back from the field of healthcare, demonstrating how people, spaces and work (as examples of events) become important elements of organising hospitals. The book will be of interest to students and scholars in and across healthcare management, organisation studies, ethnography, sociology, qualitative methods, anthropology, service management and cultural studies.

Over the last two decades, attempts to control the problem of tuberculosis have become increasingly more complex, as countries adopt and adapt to evolving global TB strategies. Significant funding has also increased apace, diagnostic possibilities have evolved, and greater attention is being paid to developing broader health systems. Biomedical revolutions seem to have radically altered the environment for HIV transmission: anti-retrovirals (ARVs) and drugs to reduce mother-to-child transmission promise to cut HIV transmission rates, as does male medical circumcision. However, the hopeful messages of UNAIDS are tempered with warning about expenditure shortfalls and calls for funding. Contributions to this book remind us that, along with the external financial constraints, there have been new fractures in state power and in the organisation of health systems. More than this, the book fundamentally calls into question whether biomedical interventions can change the social roots of this disease. Given their span of affectedness, escalating rates of greenhouse gas emissions shape the ecological circumstances of generations to come and implicate them in harm relations they had no hand in creating. The realization is that human-inspired climate-destructive practices reverberate across plural time frames, thereby raising serious questions about the value of conventional interpretations of the copresence of sources of climate harm and their effects on the health and environmental living standards of all peoples. If injuries provoked by environmental degradation emerge across multiple time frames and affect generations differentially, where do we draw the boundaries of the just society, and how do we identify its most relevant subjects? This book explores how such questions have ignited one of the most important debates on democratic justice in recent years - that between generations. For mobilized youth and future justice coalitions campaigning internationally, expanding resource inequalities (regionally and intergenerationally) are fundamentally issues of unfair exclusions and asymmetries in relations of power between generations. The book offers a comprehensive overview of new insights being generated through such debate on the limitations of democratic presentism, as well as current institutional applications of civil and human rights norms.
