Extreme Prematurity - Practices, Bioethics and the Law (Hardcover, New): Geoffrey Miller Extreme Prematurity - Practices, Bioethics and the Law (Hardcover, New)
Geoffrey Miller
R2,470 Discovery Miles 24 700 Ships in 10 - 15 working days

Extreme Prematurity examines the controversial issues surrounding the clinical management of this group of neonates by the intervention of modern neonatal intensive care. The foregoing of life-sustaining treatment is of particular importance. The subject matter is very relevant because of the alarming increase in multiple and preterm births, due to the increase in women who are undergoing assisted reproductive procedures, and the large increase in premature labor. No recent book covers the subject in such comparable breadth. The first section of this very timely monograph covers the epidemiology and practices in different parts of the world; the second section covers bioethics considerations, including ethical theories, moral principles and quality of life issues; the third section covers national and international guidelines; the last section covers medical law aspects in the US and around the world.

Home Birth - The Politics of Difficult Choices (Paperback, New): Mary L. Nolan Home Birth - The Politics of Difficult Choices (Paperback, New)
Mary L. Nolan
R1,087 Discovery Miles 10 870 Ships in 10 - 15 working days

The rhetoric of choice is much used in UK health policy and home birth is one of the three options that women are entitled to choose between when deciding where to have their baby. However, many women making this choice run into considerable opposition from the maternity service. Home Birth: the politics of difficult choices focuses on the experiences of women whose choices were opposed by health professionals during their pregnancy journey. It confronts why and how women are being denied home birth and raises some challenging issues for current midwifery practice. Using ten women's narratives, this important volume explores why women might want to give birth at home and considers ideas of risk and informed choice in pregnancy and birth. The book includes chapters on communication and language; fear and stress; advocacy and autonomy; fathers' experience of contested place of birth and free birthing. Pointers to best practice are presented whilst the text incorporates women's narratives throughout, making this a practical and relevant read for midwifery students as well as practising midwives and childbirth educators, all of whom have a duty to make home birth a real option for women.

Perinatology - Evidence-Based Best Practices in Perinatal Medicine (Hardcover, 1st ed. 2022): Renato Augusto Moreira De Sa,... Perinatology - Evidence-Based Best Practices in Perinatal Medicine (Hardcover, 1st ed. 2022)
Renato Augusto Moreira De Sa, Eduardo Borges da Fonseca
R5,505 Discovery Miles 55 050 Ships in 10 - 15 working days

This book presents the latest evidence-based guidelines for perinatal management and is designed to help obstetricians and neonatologists minimize complications and offer patients the best possible care. Since 1960, there has been a significant increase in basic and clinical investigations on normal and pathological pregnancy in the developed world. This has provided insights into the physiopathology of pregnant women, fetuses and newborns and led to the development of new technologies, bringing about a new medical subspecialty: perinatal medicine. The book is divided into eight main sections: The first examines basic periconceptional care and discusses the ethical aspects of perinatology. The next section focuses on prenatal considerations, such as the nutritional aspects of gestation and puerperium, physical exercise during pregnancy, routine laboratory tests, prenatal care of multiple gestations and the role of the neonatologist in prenatal care. The third and fourth sections then explore fetal evaluation, and clinical intercurrences in pregnancy, respectively. The next section addresses pregnancy complications: prevention, diagnosis and management. The sixth section covers the basic aspects of congenital infections and the seventh examines labor and delivery aspects. Lastly, the final section includes chapters on neonatal assistance. Written by leading experts in obstetrics, neonatology, and perinatology, this thoroughly updated, comprehensive resource reflects the latest information in all areas, including genetics and imaging.

Caesarean - Just Another Way of Birth? (Paperback, New Ed): Rosemary Mander Caesarean - Just Another Way of Birth? (Paperback, New Ed)
Rosemary Mander
R1,460 Discovery Miles 14 600 Ships in 10 - 15 working days

This book critically analyzes the place of caesarean in childbearing at the beginning of the twenty first century. It questions the changes that are taking place in childbirth and, in particular, the effects and implications of an increase in caesarean births.

This controversial work by a practising midwife and researcher, includes discussion of:

  • the context of the operation and description of it
  • health systems around the world and their caesarean incidence rates
  • decision-making and cultural/medical constraints
  • the short and long term implications of caesarean for baby and mother.

Using up-to-date research, Rosemary Mander bases her argument on a firm evidence-base and argues that the rapidly rising caesarean section rate may not be for the benefit of either the woman giving birth or her baby. Rather, the beneficiaries may actually be those professionals whose investment is in extending the range of theirinfluence and thus increasing the medicalization of normal life.

The First Breath - How Modern Medicine Saves the Most Fragile Lives (Hardcover): Olivia Gordon The First Breath - How Modern Medicine Saves the Most Fragile Lives (Hardcover)
Olivia Gordon 1
R496 R453 Discovery Miles 4 530 Save R43 (9%) Ships in 9 - 17 working days

'Fascinating and moving.' - Adam Kay, author of This is Going to Hurt A BBC Radio 4 A Good Read choice This is a story about the cutting-edge medicine that has saved a generation of babies. It's about the love and fear a parent feels for a child they haven't yet met. It's about doctors, mothers, fathers and babies as together they fight for the first breath. The First Breath is a book about motherhood and medicine. Olivia Gordon decided to find out how, exactly, modern science saved her son's life. Crossing medical memoir with popular science, The First Breath is an investigation into the pioneering fetal and neonatal care bringing a new generation into the world, who would not have lived if they had been born only a few decades ago. The First Breath explores the female experience of medicine and details the relationship mothers develop with doctors who hold not only life and death in their hands, but also the very possibility of birth. From the dawn of fetal medicine to neonatal surgery and the exploding field of perinatal genetics, The First Breath tells of fear, bravery and love. Olivia Gordon takes the reader behind the closed doors of the fetal and neonatal intensive care units, resuscitation rooms and operating theatres at some of the world's leading children's hospitals, unveiling the untold story of how doctors save the sickest babies.

For the Sake of the Children (Paperback, New): Carol A. Heimer For the Sake of the Children (Paperback, New)
Carol A. Heimer
R1,035 Discovery Miles 10 350 Ships in 10 - 15 working days

This study examines the organization of social responsibility in the USA, in particular of critically ill newborn children. Drawing on medical records and interviews with parents and medical staff, the book investigates two neonatal intensive care units, showing the traumas of extreme medical measures, and the sufferings of infants. The accounts are by turns disturbing and heroic, as parents and staff attempt to take charge of the infants' care, redefining their roles as adults and parents, and coping with sometimes awful contingencies. Rather than treating responsibility as an ethical issue, the book focuses on how responsibility is socially produced and sustained. It questions how staff members encourage parents to take responsibility, but keep them from interfering in medical matters, and how parents encourage staff vigilance when they are novices attempting to supervise the experts. The authors conclude that it is not sufficient simply to be responsible individuals. Instead, people must learn to be responsible in an organizational world, and organizations must learn how to support responsible individuals.

Psychodynamic Interventions in Pregnancy and Infancy - Clinical and Theoretical Perspectives (Paperback): Bjoern Salomonsson Psychodynamic Interventions in Pregnancy and Infancy - Clinical and Theoretical Perspectives (Paperback)
Bjoern Salomonsson
R1,154 Discovery Miles 11 540 Ships in 10 - 15 working days

Psychodynamic Interventions in Pregnancy and Infancy builds on Bjoern Salomonsson's experiences as a psychoanalytic consultant working with parents and their babies. Emotional problems during the perinatal stages can arise and be observed and addressed by a skilled midwife, nurse or health visitor. Salomonsson has developed a method combining nurse supervision and therapeutic consultations which has lowered the thresholds for parents to come and talk with him. The brief consultations concern pregnant women, mother and baby, husband and wife, toddler and parent. The theoretical framework is psychoanalytic, but the mode of work is eclectic and adapted to the family's situation and its members' motivation. This book details such work, which can be applied globally; perinatal psychotherapy integrated with ordinary medical health care. It also explains how psychotherapy can be made more accessible to a larger population. Via detailed case presentations, the author takes the reader through pregnancy, childbirth and the first few years of life. He also brings in research studies emphasizing the importance of early interventions, with the aim of providing therapists with arguments for such work in everyday family health care. To further substantiate such arguments, the book ends with theoretical chapters and, finally, the author's vision of the future of a perinatal health care that integrates medical and psychological perspectives. Psychodynamic Interventions in Pregnancy and Infancy will appeal to all psychoanalysts and psychoanalytic psychotherapists working in this area, as well as clinical psychologists, clinical social workers and medical personnel working with parents and infants.

A Guide to Supporting Breastfeeding for the Medical Profession (Hardcover): Amy Brown, Wendy Jones A Guide to Supporting Breastfeeding for the Medical Profession (Hardcover)
Amy Brown, Wendy Jones
R4,491 Discovery Miles 44 910 Ships in 10 - 15 working days

This book is a practical guide for medical practitioners as they navigate through breastfeeding problems that occur in day-to-day practice. If mothers have a breastfeeding complication they are often directed to their GP. In complex situations, medical staff will be making decisions around what treatment plan to follow and whether a mother can keep breastfeeding. In recent years there has been growing evidence that medical professionals often advise mothers to stop breastfeeding while undergoing treatment, when in reality this was not a necessary step. In a time when breastfeeding rates are decreasing, it is important that medical professionals give accurate advice and support a mother's choice to breastfeed if the situation allows it. A Guide to Supporting Breastfeeding for the Medical Profession includes contributions from a wide range of medical professionals and each chapter is written with the practitioner in mind. Contributors include GPs, paediatricians, neonatologists, lactation specialists and midwives. Doctors have a vital role to play in supporting and facilitating breastfeeding, and without the appropriate knowledge they can often inadvertently sabotage it. This book will be of interest to GPs and paediatricians as well as nurse prescribers, midwives and health visitors.

Nurturing the Premature Infant - Developmental Interventions in the Neonatal Intensive Care Nursery (Hardcover): Edward Goldson Nurturing the Premature Infant - Developmental Interventions in the Neonatal Intensive Care Nursery (Hardcover)
Edward Goldson
R1,894 Discovery Miles 18 940 Ships in 10 - 15 working days

As more premature infants survive the immediate neonatal period and require prolonged periods of hospitalization, researchers in fetal development and infancy have begun to reassess the strategies for their care. In the past, the focus of neonatal intensive care was to sustain life, with little attention to the quality and implications of survival. Today, however, researchers and clinicians are seeking to enhance the development of these small infants by ameliorating the effects of extreme prematurity and the associated medical and surgical complications. This book reports the work of leading researchers who have begun to use a variety of developmental interventions in the management of small infants in neonatal intensive care units.

This book provides a highly practical atlas on the use of echocardiography by the neonatologist in the management of the neonate. Whilst recognising the limitations of the non-specialist in echocardiography, the lavish use of illustrations and practical advice will provide an excellent companion to those being trained in and applying this technique in the special care baby unit.Demonstrates how echocardiography can be used in the intensive care situation using clinical examples Superb illustrations with full explanatory text and practical adviceCongenital heart defects, the commonest serious congenital defect seen in the neonatal unit, can be difficult to distinguish clinically from pulmonary abnormalities; echocardiography allows the differential diagnosis to be made

Helping Low Birth Weight, Premature Babies - The Infant Health and Development Program (Hardcover, Twenty-Third): Ruth T.... Helping Low Birth Weight, Premature Babies - The Infant Health and Development Program (Hardcover, Twenty-Third)
Ruth T. Gross, Donna Spiker, Christine W. Haynes
R3,273 Discovery Miles 32 730 Ships in 18 - 22 working days

Each year in the United States, 250,000 infants are born too soon, weighing too little. For these low birth weight, premature infants, the future is uncertain, since they are at risk for a variety of serious medical and developmental problems--including behavioral and learning disorders that may have damaging effects for the rest of their lives. The extent to which a comprehensive early intervention program could improve or prevent these adverse outcomes was examined in the Infant Health and Development Program, a randomized controlled trial involving almost 1,000 infants in eight cities in the United States. This book describes in detail the program, its research methodology, the progress of the program, and the results of the clinical trial.
The program was administered by an interdisciplinary team composed of physicians, biostatisticians, child development specialists, and researchers from several disciplines. It was instituted upon the discharge of the infants from the neonatal nursery and was maintained for three years. One-third of the infants were randomly assigned to an intervention group, the remainder to a follow-up group. Infants in both groups received pediatric care and community referral services, but only those in the intervention group participated in a program that included extensive home visits, attendance at a child development center, and group meetings for parents.
The results of the program proved to be clinically important; at age three, the children in the intervention group had significantly higher IQ scores, greater cognitive development, and fewer behavioral problems. The implications of the findings for public policy are equally important, for there is increasing interest in the prevention, early detection, and management of developmental disabilities in children, as evidenced by such legislation as the Education for All Children Act. Strategies to minimize the problems of low birth weight children, with their potential for long-term savings through the prevention of disabilities and their attendant costs, could have significant repercussions in such governmental areas as medical care, education, and social welfare.

The Fetal and Neonatal Pulmonary Circulations (Hardcover, REV): E Weir The Fetal and Neonatal Pulmonary Circulations (Hardcover, REV)
E Weir
R4,132 Discovery Miles 41 320 Ships in 10 - 15 working days

Birth is a sudden, traumatic transition of environments. Once the placental oxygen supply ceases, the foetus has only minutes to establish pulmonary oxygen transport, which requires not only inflation of the lungs, but also sudden and sustained changes in the lung circulation. Not long ago, research in this field was largely restricted to morphology and physiology. Now the powerful new tools of cellular and molecular biology have begun to
shed light on the physiological processes in the developing lung and its supporting vasculature.

In 22 chapters, three main sections explore lung growth and development, vascular cell growth and differentiation, and the mechanisms of hemodynamic control in the neonate; extensive illustrations give a comprehensive picture of pulmonary circulatory development. Factors controlling vasculogenesis and angiogenesis are described by the scientists who pioneered the field. Similarly, the intracellular signaling cascades that determine proliferation or growth inhibition of fibroblasts, smooth muscle cells, and endothelial cells are also presented in an understandable manner. Finally, the role of
substances, such as nitric oxide and endothelin, that control the hemodynamics of the fetal and neonatal circulations are discussed in detail, particularly in relation to the changes occurring at birth.

This book will inform basic scientists as well as the clinician and student, and should be of particular interest to pediatric cardiologists, pulmonary medicine physicians, and vascular biologists.

It aims to be a readily available source of information for resident obstetric staff and family practitioners who undertake obstetrics. It is also intended to provide guidelines for specialist obstetricians in training their juniors in the safest and most effective ways of using drugs during pregnancy, and for midwives, nurses and pharmacists, who need to understand the drug treatment of pregnant women.;The prescribing sections of this handbook are restricted to agents of known safety and proven efficacy and as well as covering the use of drugs in pregnancy the book includes sections on the transfer of drugs into breast milk. Advice on diet, nursing care and physiotherapy are mentioned where relevant and an account of the procedures involved in providing an epidural service are included. Doses of drugs used for newborn babies are given in the last chapter.;A section on preconception counselling has been added to the second edition and the drugs included in the volume have been revised, some preparations which have become of value to obstetric practice have been added and some which have been found to have limited clinical value have been withdrawn. This edition has been adjusted so that the prescribing procedures are in line with those used in North America.
