The Republic of Indonesia, home to over 240 million people, is the world's fourth most populous nation. Ethnically, culturally, and economically diverse, the Indonesian people are broadly dispersed across an archipelago of more than 13,000 islands. Rapid urbanization has given rise to one megacity (Jakarta) and to 10 other major metropolitan areas. And yet about half of Indonesians make their homes in rural areas of the country. Indonesia, a signatory to the United Nations Millennium Declaration, has committed to achieving the Millennium Development Goals (MDGs). However, recent estimates suggest that Indonesia will not achieve by the target date of 2015 MDG 4 - reduction by two-thirds of the 1990 under - 5 infant mortality rate (number of children under age 5 who die per 1,000 live births) - and MDG 5 - reduction by three-quarters of the 1990 maternal mortality ratio (number of maternal deaths within 28 days of childbirth in a given year per 100,000 live births). Although much has been achieved, complex and indeed difficult challenges will have to be overcome before maternal and infant mortality are brought into the MDG-prescribed range. Reducing Maternal and Neonatal Mortality in Indonesia is a joint study by the U.S. National Academy of Sciences and the Indonesian Academy of Sciences that evaluates the quality and consistency of the existing data on maternal and neonatal mortality; devises a strategy to achieve the Millennium Development Goals related to maternal mortality, fetal mortality (stillbirths), and neonatal mortality; and identifies the highest priority interventions and proposes steps toward development of an effective implementation plan. According to the UN Human Development Index (HDI), in 2012 Indonesia ranked 121st out of 185 countries in human development. However, over the last 20 years the rate of improvement in Indonesia\'s HDI ranking has exceeded the world average. This progress may be attributable in part to the fact that Indonesia has put considerable effort into meeting the MDGs. This report is intended to be a contribution toward achieving the Millennium Development Goals. Table of Contents Front Matter Summary Chapter 1 Introduction Chapter 2 The Data Conundrum Chapter 3 Maternal, Fetal, and Neonatal Mortality Chapter 4 The Indonesian Health Care System Chapter 5 The Quality of Care Chapter 6 Governance Issues Chapter 7 Strengthening the Financing of Maternal and Neonatal Health Chapter 8 Recommendations Appendix: Fertility Decline in Indonesia and Its Relationship to Maternal Mortality Glossary
This book has been written to summarise the available knowledge regarding transcutaneous bilirubinometry, the first technique that allows a completely non-invasive measurement of a clinical variable. This technique led to important changes in the management of neonatal jaundice, the most common clinical condition of new-born infants world-wide. Transcutaneous bilirubinometry has its roots in the old observation that yellow skin colour is due to the deposition of bilirubin, a chromophore molecule. More recently, transcutaneous bilirubinometry has involved many disciplines; and accordingly, this book has contributions from physicists, biologists, paediatric dermatologists and neonatologists who have clinical research expertise in the field of neonatal hyperbilirubinemia. We hope that this book may prompt the advancement of research that will answer the as-yet unanswered questions, improve the reliability and usefulness of transcutaneous bilirubinometry, and respond to the clinical needs of our little patients.
Paediatric orthopaedics deals with the diagnosis and management of diseases and injuries in bones, joints, muscles and nerves, in children. The second edition of Neonatal Orthopaedics examines orthopaedics problems in neonates, focussing on antenatal diagnostic measures, diagnostic points and salient features of each disorder. The book begins with an introduction to the general aspects of neonatal orthopaedics, discussing anatomy and physiology, antenatal diagnosis, genetics, skeletal and muscular disorders and birth injuries. The following sections describe various disorders found in different parts of the neonate body - lower limb, vertebral, upper limb and shoulder. Each chapter discusses in detail conditions that may be encountered, describing their etiology, clinical features, differential diagnosis and treatment. Keywords for each chapter have also been included. The final sections discuss neonatal infections and miscellaneous conditions including tetanus, gangrene, conjoined twins and rheumatic disorders. Key points Second edition examining etiology, clinical features, diagnosis and treatment of orthopaedics disorders in neonates Features diseases and injuries in all sections of the body Each chapter includes a keywords' box Previous edition published in 2007
It has been close to six decades since Watson and Crick discovered the structure of DNA and more than ten years since the human genome was decoded. Today, through the collection and analysis of a small blood sample, every baby born in the United States is screened for more than fifty genetic disorders. Though the early detection of these abnormalities can potentially save lives, the test also has a high percentage of false positives - inaccurate results that can take a brutal emotional toll on parents before they are corrected. Now some doctors are questioning whether the benefits of these screenings outweigh the stress and pain they sometimes produce. In "Saving Babies?" Stefan Timmermans and Mara Buchbinder evaluate the consequences and benefits of state-mandated newborn screening - and the larger policy questions they raise about the inherent inequalities in American medical care that limit the effectiveness of this potentially lifesaving technology. Drawing on observations and interviews with families, doctors, and policy actors, Timmermans and Buchbinder have given us the first ethnographic study of how parents and geneticists resolve the many uncertainties in screening newborns. Ideal for scholars of medicine, public health, and public policy, this book is destined to become a classic in its field.
The Year Book of Neonatal and Perinatal Medicine brings you abstracts of the articles that reported the year's breakthrough developments in neontatology and perinatology, carefully selected from more than 500 journals worldwide. Dr. Avroy Fanaroff, Emeritus Professor as Case Western Reserve University, is a key opinion leader who has been a driving force in the field for many years. He authors many commentaries himself and has assembled top experts to select the most important journal articles and write commentaries that evaluate the clinical importance of each article and discuss its application to patient care. There's no faster or easier way to stay informed! This annual covers all aspects of care from the fetus, postnatal growth, labor and delivery, infectious diseases, respiratory disorders, cardiovascular health, and gastrointestinal health to name a few. The Year Book of Neonatal and Perinatal Medicine publishes annually in November.
This book looks at genetic screening of new-borns and the ethical principles that guide this practice. The majority of babies born in the U.S. each year undergo screening soon after birth to identify genetic defects that could cause serious illness if left undetected and untreated. The goal is to detect diseases as early as possible so that timely, effective treatment can be initiated even before the onset of symptoms. In most states, new-born screening is now mandated by law. Of the approximately four million babies screened each year, about 5,000 are identified as having serious heritable disorders, most of which are, in varying degrees, amenable to treatment. For more than 40 years, the moral focus of new-born screening has been what is good for the infant. However, as more and more disorders have been added to state new-born screening programs, the traditional ethical principles of screening have been called into question. This aim of this study is to foster public awareness of the practice of new-born screening, the ethical principles that have guided it until now, and the ethical problems posed by its current and future expansion. This book consists of public documents which have been located, gathered, combined, reformatted, and enhanced with a subject index, selectively edited and bound to provide easy access.
Anthropometric measurements of growth parameters form an integral and vital part of paediatric practice and research. It is important to know the norms of vital parameters in order to recognize abnormalities when they arise. This book is intended to be a handy reference for doctors and nurses looking after neonates. It provides gestation-specific reference standards of physical measurements for Hong Kong Chinese newborns. The data has been obtained from observation of over ten thousands babies in twelve hospitals in Hong Kong. Health care professionals will find this book indispensable for their day-to-day care of both sick and healthy neonates.
This book is a collection of some major topics in neonatal neurology, delivering up-to-date data. The book covers several sections in a wide variety of aspects and interests. The topics include: VLBW infants and developmental follow up, kernicterus -- why is it persistent and can we make it go away, drugs and cerebral blood flow in the neonatal brain, neonatal EEG, neurological sequelae following mechanical ventilation, amplitude integrated EEG (aEEG), surgical treatment of post haemorrhagic hydrocephalus in the neonate, perinatal head trauma and neuro-trauma, and ethical approach to neonatal neurological issues.
Birth defects are defined as abnormalities of structure, function, or body metabolism that are present at birth. These abnormalities lead to mental or physical disabilities or are fatal. There are over 4,000 different known birth defects ranging from minor to serious, and although many of them can be treated or cured, they are the leading cause of death in the first year of life. This book presents leading research in this field from around the world.
Extreme Prematurity examines the controversial issues surrounding the clinical management of this group of neonates by the intervention of modern neonatal intensive care. The foregoing of life-sustaining treatment is of particular importance. The subject matter is very relevant because of the alarming increase in multiple and preterm births, due to the increase in women who are undergoing assisted reproductive procedures, and the large increase in premature labor. No recent book covers the subject in such comparable breadth. The first section of this very timely monograph covers the epidemiology and practices in different parts of the world; the second section covers bioethics considerations, including ethical theories, moral principles and quality of life issues; the third section covers national and international guidelines; the last section covers medical law aspects in the US and around the world.
In the present report, ICRP provides information on radiation doses to the infant due to intakes of radionuclides in maternal milk. As in Publication 88 (ICRP, 2001) on doses to the embryo and fetus following intakes of radionuclides by the mother, intakes by female members of the public and female workers are addressed. Acute and chronic intakes are considered at various times before and during pregnancy as well as during the period of breastfeeding. Dose coefficients per unit intake by the mother (Sv/Bq) are given for the selected radionuclides of the same 31 elements for which age-specific biokinetic models were given in Publications 56, 67, 69, and 71. For these elements, doses were calculated for the most radiologically significant natural or artificial radionuclides that might be released into the environment due to various human activities. Dose coefficients are also given in this report for radionuclides of an additional four elements: sodium, magnesium, phosphorus, and potassium.
This scale is the most comprehensive examination of newborn behaviour available and is used in both clinical and research settings around the world. It has been used to study the effects of intrauterine deprivation, maternal substance use, casesarian-section, undernutrition, preterm birth and other pre- and perinatal variables and has also been used in cross cultural studies. Its value as a reliable research instrument has been established for 20 years. The scale is being used increasingly by clinicians in their clinical practice as a way of sensitising parents to the capacities of their newborn infants and as a way of identifying concerns about the baby. With this in mind, this new edition of the scale manual is particularly designed to make it easier for clinicians to use the scale in this way. Guidelines for adaptations for use in clinical practice are clearly outlined. As with previous editions, the manual also provides background information on the scale and it presents information on the administration of each item and detailed criteria for scoring. Information on training is also included.
Over the last three decades there have been important advances in the implementation of neonatal screening programs and analytical techniques for congenital diseases in different parts of the world, including the Asian Pacific area. In the Second Asian Pacific Regional Meeting of the International Society for Neonatal Screening held in Hong Kong in November 1995, leading medical scientists in the field presented exceedingly useful and interesting results of their work. This book is a selection of edited and revised papers presented in that meeting. |
