This report continues a series of annual studies by the USFA of on-duty firefighter fatalities in the United States. The specific objective of this study is to identify all on-duty firefighter fatalities that occurred in the United States and its protectorates in 2009 and to analyze the circumstances surrounding each occurrence. The study is intended to help identify approaches that could reduce the number of firefighter deaths in future years.

USFA contracted with TriData Corporation to conduct an in-depth examination of four investigation units from small to medium size jurisdictions; to survey by phone a wide assortment of investigators; and to help three units uncover aspects of their organization and operations needing improvement.

By the mid-nineteenth century, efforts to modernize and industrialize Mexico City had the unintended consequence of exponentially increasing the risk of fire while also breeding a culture of fear.
