This book is designed to consolidate the relevant literature as well as the thoughts of professionals currently working in the field into a practical and accessible reference for the emergency medical technician, student, nurse, resident, and attending emergency physician. Each chapter is divided into four sections: case presentation, discussion, review of the current literature, and recommendations. Designed to serve simultaneously as a learning and reference tool, each chapter begins with a real case that was encountered in an ED setting. The case presentation is followed by a short discussion of the case, as if at a morbidity and mortality conference, by a panel of experienced attending physicians explaining how they would approach the ethical dilemmas associated with the case, and a review of the existing literature.

Acute appendicitis is one of the most common surgical abdominal emergencies worldwide, with a lifetime risk of 8.6% for males and 6.7% for females. This compilation discusses how while non-operative treatment of appendicitis has been proposed as an alternative to surgery, appendicectomy remains the mainstay of treatment in most cases and techniques have shifted from open to laparoscopic methods. The current knowledge in pathogenesis, diagnosis, and clinical management of acute appendicitis is also discussed. Next, the authors present an updated review chapter of appendicitis regarding its prevalence and risk factors, particularly how the diagnosis of appendicitis is based on a history of diffuse abdominal pain, which usually starts in the central region of the abdomen and migrates to the lower right quadrant of the abdomen. The residual remnant or "stump" of the appendix after an initial appendectomy may become inflamed causing what is commonly referred to as stump appendicitis. Treatment with antibiotics may be attempted as an alternative to surgery; however, the vast majority of cases require definitive surgical excision of the remnant appendix. The authors propose that keeping this disease in the differential is important in reducing complications associated with delayed diagnosis.

In this book, the epidemiology, identification of and management of injuries sustained from blunt abdominal trauma (BAT) in childhood are discussed, with a focus on recent advances and future research. Open abdomen (OA), which refers to leaving abdominal incisions open after surgical procedures, and its management are presented as well. Also in this book, bacterial peritonitis and intra-abdominal abscesses, which are major complicated intra-abdominal infections post abdominal trauma are discussed. The severe abdominal consequences of falling, particularly among older adults, is reviewed as well. Finally, periodontitis is known to be a risk factor for abdominal aortic aneurysm (AAA). The authors focus on the role of specific periodontopathic bacteria in AAA and are able to conclude that periodontal pathogens play a major, critical role in AAA development.
There are various underlying causes which initiate the sequence of events resulting in compartment syndrome including fractures, bleeding, tightly applied casts or extrinsic compressions devices, burns or vigorous exercise. The first chapter of this book serves to review the current understanding of ACS's pathophysiology and highlight implications for the modernization of diagnosis and treatment. The second chapter reviews the evidence relevant to how anesthesiologists can influence the genesis of compartment syndrome. Anesthesia may influence the development of CS through the use of regional anesthesia, optimum blood pressure control and oxygen delivery, and tailored analgesia in the peri-operative period. It provides support for the best anesthesia practices in an effort to avoid this complication which may have serious adverse effects for the patient. The three essential components of conventional pressure monitoring systems are examined in Chapter 3, which can assist in the diagnosis of CS. In Chapter 4, the authors present their research on acute limb compartment syndrome (LCS), which when left untreated, can result in muscle necrosis and limb loss, and in severe cases, death. Both the prevention and treatment of compartment syndrome in severe cases is examined in detail. Chapter 5 focuses on lower limb compartment syndrome, a clinical entity resulting from increased pressure in a non-expansile tissue space, which is associated with significant morbidity and mortality. Chapter 6 focuses on Paraspinal Compartment Syndrome (PSCS), resulting in the "Bourdon tube effect." The last chapter, Chapter 8, reviews orbital compartment syndrome (OCS), a rare but treatable ophthalmic surgical emergency, caused most often by a retrobulbar hematoma, facial trauma being the most frequent etiological factor |
