In keeping with the aims of other books in this Series the Editors have concentrated on the practical aspects of management -in this case of the urinary tract in patients with spinal cord injury. It is well accepted that the management of such patients is best done by those with special experience in this field. Nevertheless, it is essential that urologists should be well informed on matters relating to the neuropathic urinary tract since not all patients will be managed in special centres and, whether their problems are acute or chronic, the wrong clinical decision can lead them into a lifetime of problems. The plan of this book is directed towards examining particular problems and providing definitive answers. Even in this enlightened age of medical progress there are many clinical situations where there is often a choice of treatment. Readers of this Series will be aware that, despite an apparent abundance of information on a topic, it is sometimes not possible to reach a firm conclusion on a management problem. Keith Parsons and John Fitzpatrick are to be congratulated on ensuring that for this subject there are good clear guidelines. The data in the book are up to date and the excellent approach to the subject makes it a valuable addition to Clinical Practice in Urology.

Studies on physical education and sports for persons with disabilities are an interdisciplinary domain in which theory and practice are closely intertwined. The First International Symposium on "Pain and Kampo (Japanese Herbal Medicine)" took place in Tokyo in October, 1992. More than 700 registrants participated, with 12 registrants from seven overseas countries. This book contains summaries of the presentations at this meeting and is published in order to provide up-to-date information to a much wider audience. In China, traditional herbal medicine has been used for more than 3000 years while in Japan, its use has been documented for more than 1000 years, albeit with some changes to the original Chinese formulations. Recently there has been a revival of interest in herbal medicines for several reasons. In summarizing current insights and controversies over concussions in athletics, this book makes the vital point that symptom resolution does not necessarily mean injury resolution. Research shows that dysfunctional pathways continue for extended periods even after a minor concussion. Until the consequences of short-term perturbations and long-term residual brain dysfunctions are better understood, concussions must be treated with respect and given a higher priority for continued research activity.

"Re-education" consists in training people injured either by illness or the vagaries of life to achieve the best functionality now possible for them. Strangely, the subject is not taught in the normal educational curricula of the relevant professions. It thus tends to be developed anew with each patient, without recourse to knowledge of what such training, or assistance in such training, might be.
New paradigms of re-education are in fact possible today, thanks to advances in cognitive science, and new technologies such as virtual reality and robotics. They lead to the re-thinking of the procedures of physical medicine, as well as of re-education.
The first part looks anew at re-education in the context of both international classifications of functionality, handicap and health, and the concept of normality. La crescita esponenziale dell'interesse per la ventilazione non invasiva (NIV) verificatasi negli ultimi 10-15 anni, non solo dal punto di vista clinico e applicativo, ma anche speculativo, ha pochi eguali nella recente storia della medicina.
In Italia e in Europa in generale tale metodica e applicata su larga scala, prevalentemente nei reparti di Pneumologia e nelle Unita di Cure Intermedie Respiratorie, mentre per quanto riguarda la sua applicazione nei reparti di Terapia Intensiva Generale (UTI) i dati emersi da uno studio multicentrico condotto nei paesi francofoni vedono la NIV impiegata in una quantita di casi che rappresenta fino al 50% dei pazienti che richiedono assistenza ventilatoria.
Il recente studio EUROVENT ha inoltre dimostrato come la NIV non si limiti alla sua applicazione "acuta," dal momento che circa 25.000 pazienti sono attualmente ventilati "in cronico" a domicilio. Inoltre, si calcola che milioni di cittadini europei soffrano attualmente di disturbi respiratori durante il sonno, e per molti di essi il trattamento medico di prima scelta e rappresentato dalla NIV. The rehabilitation professions are under pressure to innovate in order to deliver services to a growing market of increasingly savvy consumers. The stroke I am referring to in this book is a CVA (Cerebrovascular Accident). In my case, it was a blockage in the central lobe with a banging headache, dizziness and then the whole left side losing feeling. Not nice! I realised this was life threatening when my consultant turned to my husband and said, “We need to act quickly; time is not on her side.” My husband’s response was, “Don’t stand there, get on with it!” If he hadn’t, my life now would be very different. My concentration levels were not good at first. I knew I needed to keep my brain active: I did not want to lose my mind, memory etc. However, with the help, love and support of others (and two years on), I can now share my thoughts on this experience. Who knows? It may help others to cope. Through this book, I am going to share my deepest darkest days, and explain to you why I feel lucky.
