Many patients with brain damage are left with a range of neuropsychological deficits that impair normal cognitive process. It is generally recognised that these less obvious cognitive deficits (including memory, language, perception, attention and executive disorders) militate against full recovery often to a greater extent than more traditional medical deficits (e.g. paralysis, sensory loss, etc). Recognition of this has helped fuel the exponential growth in cognitive neuropsychology and neuroscience over the past 30 years. In turn, this theoretical approach has been used to guide and inform the development of cognitive therapies designed to remediate cognitive impairments and their functional consequences. Cognitive rehabilitation has over the last decade grown to become an established and influential therapeutic approach. In dem gleichen Bericht werden 13 wei- tere zuverHissige epidemiologische Bevolkerungsstudien zitiert. Die geschatzte totale Inzidenz einschlieBlich der Ruckfalle betrug 2,2 pro 1000 im Jahr. Aufgrund von zahlreichen anderen Studien in GroBbritannien, Danemark, Finnland, der Schweiz und den USA darf angenommen werden, daB die Haufigkeit des Schlag- anfalls in den Industriestaaten der westlichen Zivilisationen von Land zu Land nur geringen Schwankungen unterworfen ist. Extrapoliert auf die Schweiz wurde dies bedeuten, daB wir in unserem Land pro Jahr etwa mit 12000 Patienten, die den ersten Schlaganfall oder einen Ruckfall erleiden, zu rechnen haben. Ange- sichts der hohen Mortalitatsrate wahrend der ersten 3 Monate nach dem Schlag- anfall einerseits und der Tatsache, daB viele Patienten nur leicht erkranken und eine sehr gute spontane Regenerationstendenz zeigen andererseits, ist es nicht ver- wunderlich, daB nur ein Teil der Erkrankten besonderer rehabilitativer MaBnah- men bedarf. Trotzdem kann man mit Harris (1971) annehmen, daB den Schlagan- fall iiberlebende Hemiplegiker etwa 25% aller schwer behinderten Menschen in einer beliebigen Bevolkerungsgruppe in Westeuropa ausmachen. Eine detaillierte australische Studie ergab, daB man auf eine Bevolkerung von 100000 Personen mit jahrlich etwa 110 den Schiaganfalliangere Zeit uberlebenden neuen Schwer- behinderten rechnen muB, die dringend einer Krankenhausbehandlung oder einer Rehabilitation in einem spezialisierten Rehabilitationszentrum bedurfen. In den vergangenen 10-15 Jahren konnten grosse Fortschritte in der ope rativen Behandlung angeborener Fehlbildungen der Hand erzielt werden. Mehr und mehr wurde die bis dahin noch weit verbreitete Zuruckhaltung gegenuber chirurgischen Eingriffen aufgegeben. Wesentlichen Anstoss zu die ser Entwicklung gab, zumindest im deutschsprachigen Raum, die Thalido mid-Katastrophe (1959-1962), in deren Verlauf eine uberaus grosse Anzahl von Kindern mit Schaden an den oberen Gliedmassen zur Welt kam. Die durchweg schweren, zudem sehr seltenen Deformitaten brachten eine Fulle von Problemen mit sich und stellten damit eine grosse Herausforderung an alle dar, die sich mit der Behandlung solcher Missbildungen befassten. Von hier gingen viele Impulse aus, die auf eine in Ausweitung begriffene Handchirurgie trafen und auf das Gesamtgebiet der Handfehlbildungen fort wirkten, mit dem sich gerade die Orthopadie seit jeher besonders eng verbun den fuhlte. Idee und Vorarbeit zum vorliegenden Atlas reichen bis zum Jahre 1964 zuruck. Dopo aver presentato il processo di maturazione e sviluppo della scienza riabilitativa che ha permesso di passare dal concetto della "cura del difetto" alla nuova mentalita del "prendersi cura", si analizzano le basi metodologiche su cui si costruisce il programma riabilitativo e l'esercizio fisioterapico: dalla diagnosi di difetto e compenso alla definizione del problema principale, dall'individuazione degli obiettivi di trattamento alla costruzione di una seduta fisioterapica. Nella seconda parte viene presentata un'analisi della postura seduta nel bambino sano riguardante lo sviluppo, i meccanismi neurofisiologici e gli aspetti biomeccanici ad esse sottese.
