The Medicolegal Library is the first and only series of its kind. Its importance is self-evident. During the last decade, science, especial ly medical practice, has become an increasingly complex undertak ing. Recent dramatic developments in medicine have given rise to both theoretical controversies and practical dilemmas. Society is struggling with new scientific, economic, cultural, and legal issues. The establishment of a series such as this appears vital for lawyers and physicians, for sociologists and psychologists, and for the pub lic at large. The editorial board of the Medicolegal Library consists of dis tinguished scientists from many different countries and disciplines. Judge Amnon Carmi acts as principal series editor. Judge Carmi is the author of many books and articles on medical law, and is Editor in-Chief of Medicine and Law: An International Journal. The Medicolegal Library will issue two books each year. The first four books, soon to be published, will deal with child abuse, euthanasia, disability, and nursing, laws and ethics."

This volume provides, for the first time, multidisciplinary perspectives on the problem of awareness of deficits following brain injury. Such deficits may involve perception, attention, memory, language, or motor functions, and they can seriously disrupt an individual's ability to function. However, some brain-damaged patients are entirely unaware of the existence or severity of their deficits, even when they are easily noticed by others. In addressing these topics, contributors cover the entire range of neuropsychological syndromes in which problems with awareness of deficit are observed: hemiplegia and hemianopia, amnesia, aphasia, traumatic head injury, dementia, and others. On the clinical side, leading researchers delineate the implications of awareness of deficits for rehabilitation and patient management, and the role of defense mechanisms such as denial. Theoretical discussions focus on the importance of awareness disturbances for better understanding such cognitive processes as attention, consciousness, and monitoring.
Rheumatoid arthritis is a common disease, an estimated 1. 5% of the population being under medical treatment for it in European countries and elsewhere. However, to date the origin of this chronic inflammatory disorder has not been elucidated. Evidence for the involvement of microbial agents is still tenuous, the rea- son for the defective control of inflammation is still unknown, and the disease all too of- ten progresses to crippling stages despite long-term treatment. As T. McKeown recently stated with regard to rheumatoid arthritis, "not enough is even known to indicate which is the more promising approach, and the pru- dent course is to investigate both origins and mechanisms". While working on animal model diseases as well as on aspects of rheumatoid arthritis and related disorders in man, the members of the Special Research Area on patho- physiology of rheumatoid inflammation in man and. animals (Sonderforschungsbereich 54 "Pa- thomechanismen der rheumatoiden Entztindung bei Mensch und Tier"; maintained by the Deut- sche Forschungsgemeinschaft) at the Veterinary and Medical Schools, Hannover, have been guided by the concept that by studying clinically and morphologically similar diseases in several species, mutually stimulating results might lead to improved knowledge of the pathophysio- logy of rheumatoid arthritis. In particular, from working with model diseases in animals new approaches to the etiology of rheumatoid arthritis might be derived, while detailed knowledge of pathogenetic processes in rheu- matoid arthritis could further research in chronic stages of pertinent animal diseases.

The increasing frequency of coronary diseases, the still insufficient results of prevention have brought physicians to try to reduce as much as possible the con sequences of ischaemic heart disease. Coronary units and new treatments have some what reduced the dramatic complications of the acute phase; but after the latter, evil effects of the disease remain at the physiological, psychological and social level. It is the aim of rehabi itation to analyze these problems, to reduce them or to solve them whenever possible. When this conception of "complete coronary care" took the lead, the necon ditioning effects of heart disease and of immobilization on physical conditi ons were emphasized: physical training programmes were prepared and their main lines and physiological effects are now fairly well known. But it was soon obvious that the psychological problems raised by the patient and his environment 'are equally important. Their approach is however much more difficult and their treatment particularly complex. So, the Council on Rehabilitation of the International Society of Cardiology is really grateful to Professor Dr. HALHUBER and Dr. Dr. STOCKSMEIER for giving the opportunity to specialists to discuss together this particular aspect of coronary disease. The discussions have been devoted to the methods of approaching the patient as well as to the therapeutic aspects and the role of psychologists in the rehabilitation programmes: some papers have been devoted also to the possible role of the personality as a risk factor in the development of the disease."

This study on "Exercise testing and training in coronary heart disease" is a remarkable compilation of numerous research studies, primarily from labora tories in Europe and the United States over the last decade or more. The topic reflects a growing awareness of and concern about the rapidly expanding understanding of the pathophysiology of coronary atherosclerotic heart disease. Since muscular exercise increases aerobic metabolism of myocardial and working skeletal muscles, greater flow of oxygenated blood is required by each; yet underlying vascular disease restricts these responses. This implicit paradox is here considered forthrightly. Examination of these relationships in symptomatic patients requires care and caution, because of the potential and occasionally real risks entailed. Accordingly, indirect assessment, particularly by noninvasive techniques, becomes increasingly important to detecting and monitoring - for the safety of the patients studied - the evidence of myocardial ischemia and impairment of left ventricular function under stress. Adequate assessment requires well-designed experimental studies to quantify true relationships and to measure the limits of functional capacity and the mechanisms of its impairment. Further more, alterations can be induced by therapy, whether this be pharmacological, medical or surgical, or achieved by physical reconditioning through exercise training. Not only is the cardiovascular system impaired by discrete and diffuse vascular lesions at central sites, but the degree of impairment is dynamic rather than static."
