Fungal infections are an increasing problem in critically ill patients and these infections carry an attributable mortality that is much higher than corresponding bacterial infections. In both Europe and North America, Candida infections predominate and much of this book concentrates on the epidemiology, risk factors, diagnosis, and treatment of these infections. Particular reference is made to the cross-infection problems of Candida infection and the importance of infection control and preventative measures. However, other fungal infections are increasingly being seen in critically ill patients. This book is designed to offer a European and North American perspective on each topic. There are many similarities in experience and clinical practice but also significant differences that we hope will stimulate further thought and study. We hope the book will be of interest to intensivists, infectious disease specialists, medical microbiologists, and all those with an involvement in critical care. Progress in intensive care medicine is resulting in a growing population of critically ill patients at risk of fungal infection. Further improvements in survival will require a multidisciplinary approach.

Disaster medicine has occupied an increasingly important niche within the specialty of emergency medicine over the latter half of the 20th century. Regardless of whether an event was natural, anthropogenic, or a combination of both, emergency medicine was and is the ideal discipline to develop the human resources, the strategies, the tactics, and the evidence- based research to elevate the field of disaster medicine. It began, organizationally speaking, with Hurricane Hugo in 1989 and it is continuing through the 2019 COVID- 19 pandemic and beyond. Now that we are well into the 21st century, we must steady our footing as a specialty on the 20th century's foundation so that we can wrestle with the more intricate challenges of the future. In the past two decades our global populations have experienced sectarian violence, wars, genocide, migration, terrorism, emerging infectious diseases, and pandemics. These natural and anthropogenic events will only worsen exponentially as we become intrinsically trapped by the effects of climate change. The United Nation's Intergovernmental Panel on Climate Change has recently concluded that regardless of how we try to reverse the effects of global warming, what we are experiencing now in terms of hurricanes, floods, and droughts will continue to worsen over the next 30 years. Should we, as a species, continue to equivocate and as global temperatures climb, these events will become more frequent and more catastrophic. "Catastrophic" may even be too mild a term. It is possible that over the next few years, the academic arm of emergency medicine may need to consider developing a curriculum devoted to "Cataclysmic Medicine." To confront these possibilities, there first needs to be a knowledge of disaster medicine and disaster management at its most basic level. It begins with crisis leadership, command and control, communications, and coordination. When all these pieces of the jigsaw puzzle are fitted together properly and enclosed by a proper ethical frame, then the best and most ethical and moral disaster medical can will be provided to the patient and to the community. The intent of this book is to introduce these concepts using diverse viewpoints and scenarios. Readers are challenged to cogitate, create, and layer their own set of building blocks upon the preexisting foundation, thereby reinforcing and sustaining their own capacity to prepare for and respond to any adverse eventuality, mass casualty or otherwise. At the Sixth Wiggers Bernard Conference, a group of scientists from various disciplines discussed new findings relating to nitric oxide synthase inhibitor in shock, sepsis, and organ failure. Over the past two decades it has been increasingly recognized that whole-body ultrasound is an invaluable tool in the critically ill. In addition to offering rapid whole-body assessment, it has the advantage of being a bedside approach that is available at all times and can be repeated at will. Accordingly, it permits the immediate institution of appropriate therapeutic management. The International Brain Hypothermia Symposium 2004was the second time I have had the honor of opening such a gathering on brain hypothermia treatment. It was a great pleasure to greet the participants in the hope that their valuablecontributions would make the Tokyo meeting memorable. Brainhypothermia has long been seen as a promising method that may overcome current limitations on brain resuscitation in patients with severe brain damage. However, although excellentresults have been obtained in experimental animal models, for some reason brain hypothermia has not alwaysbeen successful clinically, and resolving this problem has been a major challenge facing physicians specializing in brain therapies. The ICUmanagement of recent research has uncovered newmechanisms ofbrain damage not seen in animal models, including brain thermo-pooling at temperatures above 40 C in severe brain damage, masking neuronal hypoxia even with normal cerebral blood flow. Stress-related hyper glycemia with brain hypothermia was expected to generate useful results in patients with external injuries, cerebral occlusive stroke, and cardiac arrest. In recent clinical studies of brain hypothermia treatment, many excellent results began being reported on the manage ment of severe brain injury, ischemic stroke, and post-resuscitation after cardiac arrest. However, in clinical brain hypothermia treatment many questions remained about appro priate treatment targets, leu management technique, prevention of complications, control of brain tissue temperature, management of hypothermia insult, and mechanisms underly ing the onset of vegetative states."

Much research over the past 30 to 40 years has shown that the inflammatory response, while critical for host defense during microbial infection, may itself play a central role in the pathogenesis of sepsis. Although key mediators responsible for this injury have been identified, efforts clinically to augment our conventional antimicrobial and supportive therapies during sepsis with agents modulating the inflammatory response have been unsuccessful. As a result, the mortality associated with this lethal syndrome, especially when complicated by shock, has remained persistently high.
