In 1973, the American Psychiatric Association decided to publish a revised edition of their Diagnostic and Statistical Manual (DSM). There was great hope that a new manual would display psychiatry as a scientific field and aid in combating the attacks of an aggressive anti-psychiatry movement that had persisted for more than a decade. The Making of DSM-III is a book about the manual that resulted in 1980-DSM-III-a far-reaching revisionist work that created a revolution in American psychiatry. Its development precipitated a historic clash between the DSM-III Task Force - a group of descriptive, empirically oriented psychiatrists and psychologists - and the psychoanalysts the Task Force was determined to dethrone from their dominance in American psychiatry. Doktorarbeit / Dissertation aus dem Jahr 2011 im Fachbereich Medizin - Diagnostik, Note: 1,5, Georg-August-Universitat Gottingen, Sprache: Deutsch, Abstract: Hintergrund: Bei Patienten mit fortgeschrittenen Erkrankungen in der Palliativversorgung mussen beim Einsatz diagnostischer Massnahmen physische, psychosoziale (und finanzielle) Belastungen besonders sorgfaltig mit dem erhofften therapeutischen Nutzen abgewogen werden. Die Haufigkeit nicht-invasiver Diagnostik in der Palliativversorgung, die bevorzugten Methoden, Anwendungsprobleme und deren therapeutische Relevanz sind jedoch nicht bekannt. Methodik: Die Hospiz- und Palliativerhebung HOPE 2007 erfasste die Anwendung mikrobiologischer und bildgebender Diagnostik bei Palliativpatienten in Deutschland. Ferner wurden 70 Ultraschalluntersuchungen in der eigenen Palliativeinrichtung analysiert und abgeglichen. Bei 691 von insgesamt 3184 Patienten wurde das vorgegebene Modul zur Erfassung diagnostischer Massnahmen (DIA) genutzt. Ergebnisse: Die stationar behandelten Patienten mit DIA (86,1%) wiesen eine im Vergleich zum HOPE-Gesamtkollektiv vergleichbare Altersverteilung und Krankenhausaufenthaltsdauer auf. Bei diesen Patienten nahmen mikrobiologische Untersuchungen 26,1% aller dokumentierten Untersuchungen ein, bildgebende Verfahren 69,1%. Davon nahmen Sonografien mit 67,1% den grossten Anteil an bildgebenden Verfahren auf Palliativstationen ein, wahrend auf onkologischen Stationen eher Rontgenuntersuchungen, CT oder MRT zum Einsatz kamen. Nuklearmedizinische Verfahren wurden selten eingesetzt. In 80,8% der stationar erfassten Falle wurde der Einsatz bildgebender Verfahren als therapeutisch entscheidungsrelevant eingeschatzt. Transportprobleme und Schmerzen unter der Untersuchung waren die am haufigsten benannten Probleme. Die Diagnose der Lungenembolie ist sehr schwierig. Untersuchungen zeigen sehr haufiges Nicht-Erkennen der todlichen Gefahr. Vielleicht liegt das an einer falschen Sichtweise: Die Lungenembolie versteckt sich unter vielen Masken und totet ohne Vorwarnung. In der Lungenfachklinik Lenglern uberraschte uns der Pathologe immer mit der Schreckensnachricht: "Todliche Lungenembolie." In der Herzchirurgie der Universitat Gottingen und der Bauchchirurgie des Krankenhauses Northeim zeigten sich immer wieder thrombo-embolische Komplikationen. Und auch in der Allgemeinpraxis wurde die Thrombo-Embolie zum Schrecken. Es stellte sich die Frage: Was kann man besser machen? Die Antwort kam aus Gottingen: Krankheit ist besser zu begreifen wenn man sie als mathematischen Korper versteht.
