Thus the process of decision making itself requires analysis on an ongoing basis. Decision making is often made based on cultural factors whereas the best alternative might be quite different. The subject touches ethical aspects as well as psychological considerations. Schon die vergangenen 3 Jahrzehnte versuchte die Politik mit zahlreichen Reformen den Kostenanstieg einzudammen. Es gibt nur wenige Wirtschaftbereiche, die in solch kurzer Zeit mit so vielen Gesetzesanderungen zu tun hatten, wie das Deutsche Gesundheitssystem. Schlagworte wie demographischer Wandel, Kopfpauschale oder Burgerversicherung fielen bereits in den vergangenen Jahren, doch blieb es lediglich bei Gesprachen und Diskussionen. 2003 wurde die Gesundheitsreform verabschiedet, deren Veranderungen aber nicht ausreichen, die Probleme der gesetzlichen Krankenversicherung zu beheben. Anstatt die Strukturen und Anreize des Systems zu verandern, wurde weiterhin eine Budgetierung und Rationierung verfolgt und der entscheidende Durchbruch blieb bis heute aus. In der grossen Koalition deutet sich jetzt eine Einigung auf das Modell eines Gesundheitsfonds an. SPD und Union konnen somit ein neues Finanzierungsmodell vorweisen, das weder Kopfpauschale noch Burgerversicherung heisst. Ich mochte in meiner Hausarbeit mit dem jetzigen Ist-Zustand auseinander setzen, den demographischen Wandel erlautern, mich mit den verschiedenen Modellen der Burgerversicherung und der Kopfpauschale auseinander setzen und zum Schluss die ersten Vorschlage des noch nicht beschlossenen Gesundheitsfonds untersuche

Diplomarbeit aus dem Jahr 2003 im Fachbereich Gesundheitswissenschaften, Note: 1,0, Berufsakademie Sachsen in Plauen, 40 Quellen im Literaturverzeichnis, Sprache: Deutsch, Abstract: In der heutigen Zeit, in der es im sozialen Sektor zu gravierenden Veranderungen kommt, weil die Solidarsysteme nicht mehr finanzierbar sind, ist es auch fur Wohlfahrtsvereine uberlebensnotwendig, okonomisch zu denken und zu handeln. Die Rahmenbedingungen, in denen Non Profit Organisationen tatig werden, sind aktuell als schwierig zu bezeichnen. Der Gesetzgeber fordert eine Liberalisierung des sozialen Dienstleistungsmarktes, private Anbieter drangen auf den Markt und nicht zuletzt verstarkt sich die Konkurrenz zwischen den Wohlfahrtsverbanden. Aufgrund dieser Tatsachen setzt sich der Autor in der vorliegenden Arbeit mit Charakteristika in Wohlfahrtsvereinen auseinander. Anhand des Praxisbeispiels des DRK-KV Zwickau e.V. werden die Handlungsfelder und Moglichkeiten der Dienstleistungserbringung im Non Profit Bereich beschrieben. Verbunden ist dies mit einem Abwagen von Chancen und Risiken in diesem Tatigkeitsbereich. Die Notwendigkeit der Implementierung betriebswirtschaftlicher Steuerungsinstrumente, verknupft mit Ansatzen zur Einfuhrung dieser, werden anschliessend aufgezeigt. Mogliche Problembereiche bei der Integration betriebswirtschaftlicher Prozesse in Wohlfahrtsverbande werden am Schluss der Abhandlung dargestellt und Losungsvarianten angeboten. Am konkreten Praxisbeispiel der Einfuhrung eines umfassenden Qualitatsmanagements in den DRK-KV Zwickau e.V. wird der Nutzen von Steuerungsinstrumenten im Non Profit Sektor, aber auch die damit verbundenen Schwierigkeiten und Problemfelder aufgezeigt
The plea was advertised in the British Medical Journal in February 1929: seeking "strong energetic Medical Women with post-graduate experience in Midwifery" for "country work" in western Canada. A young Dr. Mary Percy was intrigued. After graduating with degrees in medicine and surgery from the University of Birmingham in 1927, she had been searching for the kind of opportunity which would offer both adventure and practical experience. She answered the advertisement and set off for the Peace River region of Northern Alberta in June of 1929. Little did she know that her "adventure" in the Canadian north was to last more than seventy years. Suitable for the Wilds : Letters from Northern Alberta, 1929-1931, is a collection of Dr. Mary Percy Jackson's letters written to family and friends in the early years of her practice, from 1929-1931. The letters offer a fascinating glimpse at life in northern Alberta at the beginning of the Depression, when the area was being farmed and settled by new European immigrants. These homesteaders, along with the area's Aboriginal and MA (c)tis population, were Dr. Percy's patients, scattered throughout a territory covering nearly 400 square miles. Vigilant about vaccination, nutrition, and preventive medicine, she quickly proved to be a talented physician who was truly ahead of her time, particularly in the area of tuberculosis treatment and prevention. Dr. Percy's dedication, good nature, and unfailing sense of humour shine through in her letters. This delightful and captivating collection is a tribute to her indomitable spirit.
This book of biology and medicine shows how diseases: Sickle cell anaemia, Duchenne muscular dystrophy, are related to the adaptation of our organism to aerial respiration. This adaptation is operated by a genetic switch substituting a set of foetal proteins, for more adequate, regulated, adult isoforms. We discover how foetal or adult metabolic pathways, may control the switch, and propose pharmacological treatments to boost the expression of the foetal gene, acting as a "spare wheel" to replace the adult gene when it is mutated. In fact this switch recapitulates a process reminding of the evolution of amphibians that left their pond to live in air and land. The foetus is also an aquatic creature that discovers at birth aerial respiration and the new weight of his body. His blood and muscle proteins will adapt. The metamorphosis is not as evident as for a tadpole, but still as deeply written in our genes. In fact, the switch is our second metamorphosis, the story started much earlier, when a host cell, already surviving oxygen, incorporated a bacteria, our future mitochondria, that had a more efficient oxidative metabolism. A symbiotic arrangement followed. In the course of development, the most ancient pathways come on stage first, followed by the most recent mitochondrial acquisitions. The developmental maturation of metabolic pathways changes our cells, it is our first metamorphosis. It is involved in apoptosis in diseases such as Azheimer's or Cancer. Since mitochondria took the burden of making ATP, the ancient oxidative mechanism became redundant. Its ATPase evolved forming acidic compartments that control neurotransmission or thermoregulation. This third metamorphosis is implicated in other diseases (adrenoleucodystrophy). Finally primates, who lost uricase, developed diseases related to the role of uric acid which became their new antioxidant: Gout, Autism or Schizophrenia seem to depend on this last, forth metamorphosis.

The debate over the ethical issues raised by stem cell research concerns essentially the practice of taking cells from human embryos and the consequent destruction of the embryo. This work, going to the heart of the controversy over such manipulations, discusses the ethical question of the legal status of the embryo. At the moment when, in France, the bioethics laws have come up for review, questions regarding the statute of the embryo return in the heart of scientific debates. Breakthroughs in the field of embryonic stem cell biology offer a glimpse of the considerable therapeutic possibilities.
