Since 2005 a dozen states and more than 15 specialties have reported a physician shortage or anticipate one in the next few years. This anticipated shortage and a worsening of physician distribution are compounded by a projected increased demand for women's healthcare services.

Women's healthcare is particularly vulnerable, because the obstetrician-gynecologist workforce is aging and is among the least satisfied medical specialists. Furthermore, fellowship training in women's healthcare in internal medicine and in maternal child health in family and community medicine involves only a small portion of general internists and family physicians.

In response to this challenge, the Association of American Medical Colleges called for an expansion of medical schools and graduate medical education enrollments. As we cope with significant and rapid changes in organizations and reimbursement, academic departments of obstetrics and gynecology, family and community medicine, and internal medicine have opportunities to create a unified women's health curriculum for undergraduate students, share preventive health and well-woman expertise in training programs, provide improved continuity of care, instill concepts of lifelong learning to our graduates, and better develop our research programs.

The chapters in this book are those presented at the Second International Symposium on Fundamental Problems in Breast Cancer held at Banff, Alberta, April 26-29, 1986. This is, therefore, the second volume in the series of symposia held in the Canadian Rockies. The aim of these symposia is to provide a supportive atmosphere for the development of new concepts as well as for the presentation of high quality scientific data. This is reflected in the chapters presented here. Many of the chapters put forward new hypotheses which will be tested in the clinic or laboratory in the next few years. In choosing the subjects for discourse, we preferred to tackle areas where controversy existed, and in this book we have grouped the chapters around discussion themes. The relative merits of mammography and physical examination in the detection of small lesions within the breast and the natural history of these lesions were important topics at the symposium, and the results of early detection studies will have a major effect on health care costs. We have devoted a large section of this book to this subject. The mechanism of growth and response of endocrine sensitive tumors and current concepts of the statistics and cost-benefit assessment of adjuvant chemotherapy trials comprise two other sections of the book where original and stimulating ideas were presented.

Black Middle-Class Women and Pregnancy Loss: A Qualitative Inquiry is the first qualitative research case study of its kind on Black Infant Mortality (BIM) to focus on a target group of black American-born middle-class professional married women who have all lived through the experience of infant loss. This target group allows Lisa Paisley-Cleveland to examine the BIM phenomenon outside the poverty paradigm and issues attached to teenage pregnancy, as well as to explore contributing factors attached to the persistent black and white disparity in infant mortality rates, which according to CDC s January 2013 report are 12.40 and 5.35 respectively. This book raised the following question: given the disparity in the infant mortality rates among middle-class black and white women, are there factors attached to the pregnancy experience of middle-class black women that could help us understand the adverse birth outcomes for this target group? "Dangerous Motherhood" is the first study of the close and complex relationship between mental disorder and childbirth. Exploring the relationship between women, their families and their doctors reveals how explanations for the onset of puerperal insanity were drawn from a broad set of moral, social and environmental frameworks, rather than being bound to ideas that women as a whole were likely to be vulnerable to mental illness. The horror of this devastating disorder which upturned the household and turned gentle mothers into disruptive and dangerous, mad women, was magnified by its occurrence at a time when it was anticipated that women would be most happy in the fulfillment of their role as mothers.

At the Mie International Symposium held in Japan in April 1994, leading scientists reviewed recent advances in the understanding of the contractile mechanism in smooth muscle. The Project on Reproductive Laws for the 1990s began in 1985 with the realization that reports of scientific developments and new technologies were stimulating debates and discussions among bioethicists and policymakers, and that women had little part in those discussions either as participants or as a group with interests to be considered. With the help of a planning grant from the Rutgers University Institute for Research on Women, the Women's Rights Litigation Clinic at Rutgers University Law School-Newark held a planning meeting that June attended by approximately 20 theorists and activists in the area of reproductive rights. Project purposes, methods, and general shape took form at the meeting. Two goals have characterized the Project's work since then: first, to generate discussion, debate, and, where possible, consensus among those committed to reproductive autonomy and gender equality as to how best to respond to the questions raised by re ported advances in reproductive and neonatal technology and new modes of reproduction; and second, to ensure that those shaping reproductive law and policy appreciate the ramifications of these developments for gender equality. In meeting this twofold agenda, the Project focused on six areas: time limits on abortion; prenatal screening; fetus as patient; reproductive hazards in the workplace; interference with reproductive choice; and alternative modes of reproduction. The Project identified individuals to take respon sibility for drafting model legislation and position papers in the six areas (for the drafters, see the Appendix)."

This book grew out ofmy interest in what is often called "the immunological paradox ofpregnancy." How is it possible that the fetus-halfofwhosegenetic apparatuscomesfrom thefather and is foreign to the mother-can survive to term? This is a question that intrigues all immunologists. For me, it has been of interest ever since I heard a lecture on the subject in medical school, long before I thought ofbecoming a "professional immunologist." Indeed, the question ofthe immunological aspect of fetal survival (or demise) should be of interest to any biologist or physician. The question becomes broader ifone considers the immunologic relations between motherand fetus, because they represent a unique symbiotic union. Whatimmunologic problemsinthemothermayaffecttheoffspring, and isitpossiblethatfetal immunology willaffectthe mother? Finally, there is the question ofwhether immunology is important in recur- rent spontaneous abortion. Every authorowes the reader a general oversightofthe book in hand, indicating the terrain to be covered, and, by inference, the territory that will not be explored. 1. This is primarily a book for clinicians. I will only men- tion animal experiments and data in passing, and as they may illuminate a clinical problem or observation. 2. The interest here is the immunology ofmaterno--fetal re- lations, once a pregnancy has begun. Therefore, I will notcover immunological aspects ofsterility, nor touch on the immunological approaches to controlling fer- tility, i.e., "contraceptive vaccines." 3. This is a book mainly concerned with pathogenesis.

Spontaneous abortion, a pattern of defects labeled fetal alcohol syndrome, and more subtle behavioral disturbances that occur in the absence of observable physical anomalies are some of the damaging effects of alcohol, now recognized as one of the leading known causes of birth defects in the western world. So grave are the apparent consequences of drinking alcohol during pregnancy that a new U.S. law requires warning labels to appear on all alcoholic beverages. This label represents the culmination of numerous clinical and experimental studies conducted over the last 15 years. Prompted by an abundance of new information in the area of fetal alcohol exposure, this second bibliography on the subject to be published by Greenwood Press supplements Abel's first volume that contained 3,000 citations and included literature on the subject published between 1973 (the year in which fetal alcohol syndrome was first reported) and 1984. To what extent should parents be able to choose the kind of child they have? The unfortunate phrase 'designer baby' has become familiar in debates surrounding reproduction. As a reference to current possibilities the term is misleading, but the phrase may indicate a societal concern of some kind about control and choice in the course of reproduction. Typically, people can choose whether to have a child. They may also have an interest in choosing, to some extent, the conditions under which they do so, such as whether they have a child with a serious disability or disease. The purpose of this book is to explore the difficult and controversial question of the appropriate ethical and legal extent of reproductive autonomy in this context. The book examines ethical, legal and public policy issues in prenatal screening, prenatal diagnosis (PND), selective abortion and preimplantation genetic diagnosis (PGD). It explores the ethics of these selection practices and the ability of current ethical guidelines and legal mechanisms, including the law on selective abortion and wrongful birth, to deal with advances in genetic and other knowledge in these areas. Unlike in the United States, in England the relevant law is not inherently rights-based, but the impact of the Human Rights Act 1998 inevitably raises questions about the proper scope of reproductive autonomy in this context. The implications of the analysis are considered for the development of relevant law, public policy and ethical guidelines and will be of interest to academics in medical law and ethics, health professionals, lawyers, those working on public policy and students with an interest in these issues.

'Breast is best' is today's prevailing mantra. However, women - particularly first-time mothers - frequently feel unsupported when they come to feed their baby. This new experience often takes place in the impersonal and medicalized surroundings of a hospital maternity ward where women are 'seen to' by overworked midwives. Using a UK-based ethnographic study and interview material, this book provides a new, radical and critical perspective on the ways in which women experience breastfeeding in hospitals. It highlights that, in spite of heavy promotion of breastfeeding, there is often a lack of support for women who begin to breastfeed in hospitals, thus challenging the current system of postnatal care within a culture in which neither service-user nor provider feel satisfied.
