Over the past 10 years, studies have shown that the rates of fertility vary in different ethnic groups. Ethnic differences also play a significant role in the outcome of assisted reproductive technology (ART) cycles. In the United States, minority groups--African Americans, Hispanics (mainly Mexicans and Central Americans), East Asians (Chinese, Japanese, Koreans, Philippinos) and South Asians (Indians, Pakistanis, and Bengalis)--have significantly lower chances of live births compared to Caucasian women. Birth outcome data collected by the Society for Assisted Reproductive Technology shows a worsening trend in conception rates between the years 1999-2000 and 2004-2006, raising more concern that the disparity in fertility rates between minority groups and white women is widening over time. Alcohol and other drugs of abuse cause significant physi ological changes, especially during development. The effects on the infant and child range from severe mental retardation to mild changes in activity and neurological functions. Although the level of intake needed to cause fetal damage is not clear, the magnitude of the problem is significant, with many long-term sequelae. As a result, it becomes critical to better diagnose and manage drug and alcohol use during pregnancy. This must involve special training for health care professionals. In addi tion, recognition of the psychosocial factors affecting alcohol use, especially by youth and young adults, is critical to modi fying behavior, and thus reducing fetal alcohol exposure. Cultural considerations can also come into play in modi fying alcohol and drug use by women so as to reduce fetal damage. The trends in alcohol and drug use by youth forecast rising levels of damage to infants. These children will need extensive medical and educational care for years to decades. Clearly, understanding of the role women must take in modifying their alcohol and drug use during pregnancy will facilitate changes in our cultural and educational practices that will help reduce fetal trauma from alcohol.

The field of androgen excess disorders has advanced substantially since the original publication of this book. The Androgen Excess Society (AES) was founded to bring together investigators in the field. A better understanding of the screening, progression, and molecular genetics of nonclassic adrenal hyperplasia (NCAH) has improved the clinical care and diagnostic accuracy of these patients. New criteria for the diagnosis of the polycystic ovary syndrome (PCOS) were proposed in Rotterdam, criteria that have resulted in controversy and, hopefully, initiation of new studies. A Social Worker's Investigation of Childbirth Injured Women in Northern Nigeria investigates Vesicovaginal fistula (VVF), a childbirth injury commonly found among younger adolescent wives in northern Nigeria. Women with fistulae continuously drip urine. Their offensive odor often leads to life as social outcasts. Millions of women across Africa and the developing world suffer from this condition, but it is preventable and curable. This work examines the problem from the perspective of a social worker. It is not intended as a medical treatise, but instead deals with the condition from an ecological perspective using a systems approach. Its focus is on VVF as it relates to the social environment of the affected women. The author defines and describes VVF as it manifests itself in Africa, along with the history and epidemiology of the condition and its treatment. It describes the life course of Hausa women who are most affected by VVF in northern Nigeria and how their position in society predisposed them to childbirth injury. Testimonials of the victims about their struggles of survival and their road to a cure are narrated. Short and long term preventive measures are given.
