Ethnic Differences in Fertility and Assisted Reproduction (Hardcover, 2013 ed.): Fady I. Sharara Ethnic Differences in Fertility and Assisted Reproduction (Hardcover, 2013 ed.)
Fady I. Sharara
R4,439 R3,581 Discovery Miles 35 810 Save R858 (19%) Ships in 12 - 19 working days

Over the past 10 years, studies have shown that the rates of fertility vary in different ethnic groups. Ethnic differences also play a significant role in the outcome of assisted reproductive technology (ART) cycles. In the United States, minority groups--African Americans, Hispanics (mainly Mexicans and Central Americans), East Asians (Chinese, Japanese, Koreans, Philippinos) and South Asians (Indians, Pakistanis, and Bengalis)--have significantly lower chances of live births compared to Caucasian women. Birth outcome data collected by the Society for Assisted Reproductive Technology shows a worsening trend in conception rates between the years 1999-2000 and 2004-2006, raising more concern that the disparity in fertility rates between minority groups and white women is widening over time. This comprehensive book serves to answer the questions that arise when managing infertility in a multi-ethnic population. An expert assembly of key leaders in the field of reproductive medicine imparts insight and clinical experience in order to identify and analyze the possible causes of racial disparities in fertility outcome. Some of the reviewed causes include higher Body Mass Index (BMI), tubal diseases, metabolic syndrome, and fibroids in African Americans; tubal disease and higher early pregnancy loss in Hispanics; higher incidence of diminished ovarian reserve and lower BMI in East Asians; and higher incidence of polycystic ovarian disease (PCOS) in South Asians. The book also provides a review of data on access to care and ART services in developing countries. A thoughtful combination of evidence-based medicine and advanced treatment options, this book is sure to distinguish itself as the definitive reference on ethnic differences in assisted reproduction.

Down's Syndrome Screening and Reproductive Politics - Care, Choice, and Disability in the Prenatal Clinic (Paperback):... Down's Syndrome Screening and Reproductive Politics - Care, Choice, and Disability in the Prenatal Clinic (Paperback)
Gareth M. Thomas
R1,367 Discovery Miles 13 670 Ships in 12 - 19 working days

Nominated for the Foundation of Sociology of Health and Illness Book Prize 2018 In the UK and beyond, Down's syndrome screening has become a universal programme in prenatal care. But why does screening persist, particularly in light of research that highlights pregnant women's ambivalent and problematic experiences with it? Drawing on an ethnography of Down's syndrome screening in two UK clinics, Thomas explores how and why we are so invested in this practice and what effects this has on those involved. Informed by theoretical approaches that privilege the mundane and micro practices, discourses, materials, and rituals of everyday life, Down's Syndrome Screening and Reproductive Politics describes the banal world of the clinic and, in particular, the professionals contained within it who are responsible for delivering this programme. In so doing, it illustrates how Down's syndrome screening is 'downgraded' and subsequently stabilised as a 'routine' part of a pregnancy. Further, the book captures how this routinisation is deepened by a systematic, but subtle, framing of Down's syndrome as a negative pregnancy outcome. By unpacking the complex relationships between professionals, parents, technology, policy, and clinical practice, Thomas identifies how and why screening is successfully routinised and how it is embroiled in both new and familiar debates surrounding pregnancy, ethics, choice, diagnosis, care, disability, and parenthood. The book will appeal to academics, students, and professionals interested in medical sociology, medical anthropology, science and technology studies (STS), bioethics, genetics, and/or disability studies.

Substance Abuse During Pregnancy and Childhood (Hardcover): Roland R. Watson Substance Abuse During Pregnancy and Childhood (Hardcover)
Roland R. Watson
R3,660 Discovery Miles 36 600 Ships in 10 - 15 working days

Alcohol and other drugs of abuse cause significant physi ological changes, especially during development. The effects on the infant and child range from severe mental retardation to mild changes in activity and neurological functions. Although the level of intake needed to cause fetal damage is not clear, the magnitude of the problem is significant, with many long-term sequelae. As a result, it becomes critical to better diagnose and manage drug and alcohol use during pregnancy. This must involve special training for health care professionals. In addi tion, recognition of the psychosocial factors affecting alcohol use, especially by youth and young adults, is critical to modi fying behavior, and thus reducing fetal alcohol exposure. Cultural considerations can also come into play in modi fying alcohol and drug use by women so as to reduce fetal damage. The trends in alcohol and drug use by youth forecast rising levels of damage to infants. These children will need extensive medical and educational care for years to decades. Clearly, understanding of the role women must take in modifying their alcohol and drug use during pregnancy will facilitate changes in our cultural and educational practices that will help reduce fetal trauma from alcohol.

Written by medical specialists and faculty of the Department of Obstetricians and Gynaecology, National University Health Services, Integrated Approach to Obstetrics and Gynaecology is a textbook for medical undergraduates and residents. This textbook is unique in that it takes an integrated approach in discussing the six phases in the life of a woman, namely, the early years of growing up, the reproductive years, pre-conception, early pregnancy, pregnancy care, and functional ageing in a mature woman.Emphasis has been focused on information that constitutes essential 'core knowledge' in the blueprint of the medical undergraduate curriculum. The format is easy to follow: background introduction to history; examinations and investigations to establish a diagnosis and then treatment and management. However, this may have to be modified as deemed appropriate for a particular topic. Where appropriate, knowledge that is 'good to know' has been incorporated beyond the essential 'core knowledge'.

Androgen Excess Disorders in Women (Hardcover, 2nd Revised edition): Ricardo Azziz Androgen Excess Disorders in Women (Hardcover, 2nd Revised edition)
Ricardo Azziz
R5,704 Discovery Miles 57 040 Ships in 10 - 15 working days

The field of androgen excess disorders has advanced substantially since the original publication of this book. The Androgen Excess Society (AES) was founded to bring together investigators in the field. A better understanding of the screening, progression, and molecular genetics of nonclassic adrenal hyperplasia (NCAH) has improved the clinical care and diagnostic accuracy of these patients. New criteria for the diagnosis of the polycystic ovary syndrome (PCOS) were proposed in Rotterdam, criteria that have resulted in controversy and, hopefully, initiation of new studies. The association of insulin resistance with PCOS has been strengthened, and the role of metformin in tre- ing the infertility of the PCOS has been validated. Risks for diabetes and, more cont- versially, cardiovascular disease in women with PCOS have received substantial investigation. Our understanding of the epidemiology and economic impact of these disorders has expanded, emphasizing their critical importance. These are but a few highlights of how the terrain has changed in a relatively brief period of time. In keeping with these advances, the title of this book has been revised to reflect the growing importance of PCOS as the most prevalent androgen excess disorder in women, and arguably, as the one that might have the most serious adverse consequences for general health. There are fewer chapters to provide a more focused elucidation of the area. Several chapters were penned by new (and young) authors who are conducting cutting-edge research in the field.

Midwives and Medical Men - A History of the Struggle for the Control of Childbirth (Hardcover): Jean Donnison Midwives and Medical Men - A History of the Struggle for the Control of Childbirth (Hardcover)
Jean Donnison
R2,899 Discovery Miles 28 990 Ships in 12 - 19 working days

Originally published in 1977 and as a second edition in 1988, this book introduces the reader to the women at the top of the midwifery profession up until the 17th Century who attended the aristocracy and Royalty. The author shows how their successors were gradually driven out of the better paid work until in the middle of the 19th Century it appeared that attendance on childbearing women would inevitably become the male monopoly it has virtually become in North America. This downward trend was reversed, thanks to efforts to preserve for women the choice of female attendance in childbirth and also to the labour of philanthropists to improve maternity services to the poor. However, the drive for the institutionalization and mechanization of childbirth during the 20th Century as well as a chronic shortage of midwives, has once again shone a spotlight on the profession. This unique history of developments in midwifery will be of interest to students of medical politics, 19th Century social history, the sociology of the professions and gender studies.

Durrenda Nash Onolemhemhen
R1,114 Discovery Miles 11 140 Ships in 12 - 19 working days

A Social Worker's Investigation of Childbirth Injured Women in Northern Nigeria investigates Vesicovaginal fistula (VVF), a childbirth injury commonly found among younger adolescent wives in northern Nigeria. Women with fistulae continuously drip urine. Their offensive odor often leads to life as social outcasts. Millions of women across Africa and the developing world suffer from this condition, but it is preventable and curable. This work examines the problem from the perspective of a social worker. It is not intended as a medical treatise, but instead deals with the condition from an ecological perspective using a systems approach. Its focus is on VVF as it relates to the social environment of the affected women. The author defines and describes VVF as it manifests itself in Africa, along with the history and epidemiology of the condition and its treatment. It describes the life course of Hausa women who are most affected by VVF in northern Nigeria and how their position in society predisposed them to childbirth injury. Testimonials of the victims about their struggles of survival and their road to a cure are narrated. Short and long term preventive measures are given. The empowerment of northern Nigerian women for the eradication of this condition is a fundamental and underlying theme of this work.
