Over the past 30 years, there has been increasing recognition of psoriatic arthritis as a distinct clinical entity. Psoriatic arthritis occurs mostly in patients with psoriasis and may affect up to 1% of the general population. It has many similarities to other forms of spondyloarthritis, and must be differentiated from related conditions. Given that most patients with psoriatic arthritis have skin and musculoskeletal diseases that significantly affect their quality of life and function, patients are ideally managed in a multidisciplinary clinic with rheumatologists, dermatologists and a nurse specialist, physical therapist and occupational therapist. Psoriatic Arthritis covers the epidemiology and diagnostic and classification criteria, describing the clinical features of the disease, including skin and nail involvement, articular, and other extra-articular manifestations. Laboratory features and imaging characteristics are covered in detail, along with co-morbidities and their impact. A comprehensive review of skin disease therapy is also provided, along with the various treatment options for joint disease, including traditional disease modifying therapy and newer biologic agents . This comprehensive yet concise and practical volume is the perfect guide to psoriatic arthritis for the busy practitioner, and will be of interest to trainees and specialists in rheumatology and dermatology.

Osteoarthritis (OA) is among the top 10 of most disabling diseases in the Western world. It is the major cause of pain and disability among the elderly. This book provides a contextual review of recent research on neuromuscular factors and behavioral risk factors for functional decline in OA, with a special emphasis on explanatory mechanisms. In addition, the book discusses innovative approaches to exercise and physical activity in OA, derived from research on behavioral and neuromuscular risk factors for functional decline in OA. Recent research has shown that neuromuscular factors (such as muscle strength, joint laxity) and behavioral factors (such as avoidance of activity, depressed mood) predict pain and disability in OA. Furthermore, exercise and physical activity are among the dominant interventions aiming at reducing pain and disability, and innovative interventions targeting neuromuscular and behavioral interventions have been recently developed. This research has been published as separate papers, with the result that the field is in need of an integrative contextual review that puts the research into theoretical perspective. TARGETED MARKET SEGMENTS Rehabilitation specialists, health psychologists, gerontologists, rheumatologists, pain specialists

This book brings together the congenital, acquired and traumatic conditions which may affect the child's cervical and thoraco-lumbar spine. The immature spine poses considerable diagnostic dilemmas and radiographs are often difficult to interpret. The aetiology of deformity, its assessment, monitoring and treatment are comprehensively described. Back pain in children receives special attention. Care has been taken to identify symptoms and signs which should raise our level of anxiety and those which are likely to resolve. Where surgery is necessary the principles and risks are defined.
Children's upper and lower limb orthopaedic disorders has been written to give orthopaedic practitioners an easily accessible and concise description of regional disease and conditions encountered in the practice of pediatric orthopaedic surgery. The upper limb chapters describe fetal development and how this may go wrong. As always, function dominates our management plans although cosmetic considerations must be borne in mind. Perinatal brachial plexus injury is analyzed and a plea made for early recognition and reconstruction of both plexus lesions and peripheral nerve injuries. The lower limb chapters consider limb deficiency and deformity and how best we should manage them. Those prominent afflictions of the child's hip - dysplasia, Perthes' disease and epiphyseal slipping - are fully discussed. Chapters on the common conditions affecting the knee, foot and ankle complete the book.
In this book we discuss how children grow, how to measure this and how to decide whether development falls within normal parameters. It is important not to over-investigate, but, if necessary, which tests and imaging should we choose? We consider how best to look after our patients in hospital, out-patients departments and at home. This leads us to consider problems with the norm, including the more generalised bone and cartilage disorders, most now genetically determined. The section concludes with chapters on infection, arthritis and that most emotive of topics, the tumours of childhood.

Fibromyalgia is a common medical disorder characterised by widespread pain, often coupled with other symptoms such as fatigue, disturbed sleep and chronic headaches. Because the pain seems to originate in the muscles, traditional doctors assume that's where the problem lies. However, recent research shows that the disorder is directly related to decreased levels of the neurotransmitter dopamine. Patients suffer from an over-aggressive fight-or-flight response, causing an increased adrenaline level. The solution according to rheumatologist Dr. David Dryland, is to regulate levels of dopamine in the body using approved medications which help mute pain signals travelling from the body to the brain. The book will help readers and their doctors develop a protocol to successfully treat fibromyalgia, identify what causes the disease and flare ups, determine which symptoms can be attributed to fibromyalgia and which cannot and non-drug treatments for relieving the painful symptoms of the disease.
In the middle of the twentieth century, few physicians could have predicted that the modern diagnostic category of osteoporosis would emerge to include millions of Americans, predominantly older women. Before World War II, popular attitudes held that the declining physical and mental health of older persons was neither preventable nor reversible and that older people had little to contribute. Moreover, the physiological processes that influenced the health of bones remained mysterious. In Aging Bones, Gerald N. Grob makes a historical inquiry into how this one aspect of aging came to be considered a disease. During the 1950s and 1960s, as more and more people lived to the age of 65, older people emerged as a self-conscious group with distinct interests, and they rejected the pejorative concept of senescence. But they had pressing health needs, and preventing age-related decline became a focus for researchers and clinicians alike. In analyzing how the normal aging of bones was transformed into a medical diagnosis requiring treatment, historian of medicine Grob explores developments in medical science as well as the social, intellectual, economic, demographic, and political changes that transformed American society in the post-World War II decades. Though seemingly straightforward, osteoporosis and its treatment are shaped by illusions about the conquest of disease and aging. These illusions, in turn, are instrumental in shaping our health care system. While bone density tests and osteoporosis treatments are now routinely prescribed, aggressive pharmaceutical intervention has produced results that are inconclusive at best. The fascinating history in Aging Bones will appeal to students and scholars in the history of medicine, health policy, gerontology, endocrinology, and orthopedics, as well as anyone who has been diagnosed with osteoporosis.

Disorders of the Achilles tendon are universal, affecting people in a wide range of age groups. Because the Achilles tendon is one of the most pow- ful musculotendinous structures in the body, the impact of an injury to the Achilles tendon becomes magnified. There is a wide range of disorders or problems that can involve the insertional region, where pathology may rest with bone, tendon, or bursae. A completely different set of pathologic entities resides in the noninsertional region, one of which may include the frustrating degenerative tendinopathy. As our growing population ages but remains phy- cally active longer into life, the incidence of these disorders will continue to increase. I am proud to be given the opportunity to write the foreword to this text, which is intended for foot and ankle surgeons worldwide. Seldom does a book on a single entity become a current concepts review, as this work has. Too often, textbooks are not published for several years after the chapters have been written, making them obsolete upon publication. Not so with this book, which deals with timely topics on the Achilles tendon. Dr. James Nunley has compiled this work in slightly over a year, thus providing the reader with sta- of-the-art material. Dr. Nunley had the foresight to create a much needed techniques-oriented book dealing with the complexities of the Achilles tendon. His approach was to develop a comprehensive guide to managing Achilles tendon problems.
