After the Nancy Cruzan case was decided by the Supreme Court in
1990, and ultimately resolved by the Courts of the State of
Missouri, the decision to withhold or withdraw life-prolonging
nutrition and hydration appeared to many to be as noncontroversial
as decisions to refuse respirators or dialysis. Even the Catholic
Church held that, although there should be a presumption in favor
of providing nutrition and hydration, the patient or the patient's
surrogate could overrule this presumption, if either believed the
treatment was disproportionate or burdensome. The Schiavo case
changed all that. Although the decision to remove Terri Schiavo's
nutrition and hydration was made by her husband - her legal
surrogate - based on his wife's belief that such treatment was
disproportionate, Schiavo's immediate family protested so much that
the case took years to resolve. It eventually involved all branches
of government at both the state and federal levels. The ethical
dilemmas that such cases pose continue to stir great controversy.
This in-depth examination of these dilemmas provides information
and documentation from many perspectives. The editors have included
a foreword by Dr. Jay Wolfson, Terri Schiavo's court-appointed
guardian ad litem, as well as Dr. Wolfson's report to Gov. Jeb Bush
on the case and Gov. Bush's reply; public statements by President
George Bush and Senators David Weldon, Rick Santorum, Tom DeLay,
Bill Frist, and Barney Frank; statements by the pope and other
representatives of the Catholic Church on this issue; plus much
medical and legal background material on both precedents to the
Schiavo case and its aftermath, including the results of the
autopsy report. For anyone wishing an in-depth understanding of
these complex ethical issues, issues many of us will have to
confront in our own families, this volume is indispensable.
This book aims to fill a gap with an in-depth exploration of
nursing ethics content from the western philosophical tradition and
some of the methods used in teaching this content. It will address
cross-cultural issues in using specific ethics content. It will
also reveal the poverty of the present dualism model in nursing
ethics and replace this with a more complex and more useful model
that invites debate. Its scope is both wide and deep but that is
needed to enrich the basis for teaching nursing ethics. Outlines
and critiques all current ethical theories and considers their
application to nursing practice Explores ethical issues in numerous
cultures Includes case studies drawn from a range of countries
Written by leading nurse educators and philosophers in the field
This book lays a clear foundation of knowledge for any nurse
involved with palliative care. It focuses on the needs and
perspectives of the patient, and seeks to give the reader a strong,
broad understanding of how best to deliver full, rounded care.
Covering the wide range of care provision in hospices, hospitals
and patients' homes, the book draws widely from practice examples
to explain and expand theoretical issues. Research evidence
underpins each of the chapters while guided activities enable
readers to reflect in a focused way on their experience and current
practice. While recognising the importance of team work in
providing good quality palliative care, the book unashamedly
concentrates on the unique nursing skills and knowledge required
for this demanding work. Covering key issues in palliative care
nursing such as spirituality, sexuality, ethics, loss and grief,
this book defines and develops palliative care as a key nursing
specialty. A clear, broad-based approach offers a thorough
introduction for the non-specialist nurse. Written and edited by an
experienced team of nurses working in this field, grounding it in
current practice. Learning outcomes listed at the start of each
chapter aid learning and comprehension. Reflective practice
activities and an outline of CPD is especially useful for students
working independently. Case histories, recommended reading lists,
and references provide a solid evidence base for clinically based
practice and facilitate further study. Thoroughly revised and
updated to reflect changes in policy direction. A new chapter on
pain and symptom management. Revised content reflects the recent
shift in the evidence base concerning spirituality. Includes
psychosocial issues of loss for the patient, their family, and
careers.
While high dependency care may be delivered in designated high
dependency units, it also occurs in many other clinical
environments. The book provides invaluable information for nurses
working in all acute hospital wards and departments including
recovery units, high dependency units, acute surgical or medical
wards, and accident and emergency units. It is also a useful
resource for nurses undertaking specialist post-registration
programmes (particularly in critical care), in-house courses, and
also those who are new to high dependency nursing. Some chapters
are devoted to specific clinical areas such as cardiac care and
respiratory care, with others covering broader clinical issues such
as pain management and post-operative care. The underpinning
anatomy, physiology and pharmacology are included in relevant
chapters. In all these areas there is a strong emphasis on clinical
application. Chapters on non-clinical, yet vital, components of
high dependency nursing are also included to address aspects such
as the development, management, interpersonal and social issues
associated with high dependency care. The second edition of this
highly successful textbook has been thoroughly updated throughout
in line with the latest research, clinical and policy developments.
Principles and Practice of High Dependency Nursing is an invaluable
resource for nurses and managers working in acute care settings and
sets the standard for modern high dependency nursing practice.

The book looks at a broad perspective of decision making and each
chapter focuses on a specific aspect related to making crucial
decisions. Following an initial introduction the book explores the
concept of autonomy and the many factors that influence autonomous
practise. The role of knowledge in decision making, using evidence
to inform decisions, as well as different approaches to decision
making are also examined - including the traditional or rational
approach, decision analysis and the development of professional
judgement. Dilemmas arise when decisions are made and therefore
ethical decision making is an important component of this book.
Management decisions may be different from those related to giving
specific care to women, hence one chapter focuses on making
management decisions. Emphasis is also placed on the role of the
midwife in helping women make their own decisions, the role of
reflection in enhancing the decisions midwives make and the support
midwives can receive from their Supervisor of Midwives. Flowcharts
explain and facilitate the decision-making process. A very
practical approach to decision-making in midwifery, with
contributions from midwives who have considerable experience in
this area Provides guidelines on how to achieve successful
autonomous midwifery practice, enabling theory to be effectively
applied to practice Includes coverage of management roles and
decision-making as well as clinical scenarios, offering frameworks
and flowcharts to guide the inexperienced Suggests different
approaches to making difficult decisions

This book is the first to focus on the work of nearly 600 sisters
from 12 different Catholic orders who nursed wounded and sick Union
and Confederate soldiers between 1860 and 1865. Drawing on archival
sources and the personal papers of the women who participated,
Maher gives a detailed account of their experiences: how they were
called into service, where they served, what duties they performed,
how they looked on their mission, and how they were viewed by those
who worked with them. Through service on the battlefield, in
hospitals, and on transport boats, the sisters became known for
their dedication and practical skills. Maher begins with a
dicussion of Catholic sisters in mid-nineteenth century America and
the development of Catholic nursing during that period. While other
women were prohibited by custom from nursing outside the home,
Catholic sisters had established the practice of caring for the
sick in the community and providing nursing care during epidemics
and other public crises. During the Civil War, their assistance was
sought by Union and Confederate governmental, military, and medical
authorities. Through service on the battlefield, in hospitals, and
on transport boats, the sisters became known for their dedication
and practical skills. Maher examines the impact of their work in
both modifying negative pre-Civil War attitudes towards Catholics
and sisters and in paving the way for the development of a nursing
profession outside the Catholic orders. Basing her study on
letters, journals, and memoirs containing the sisters' personal
accounts of their experiences, Civil War histories, and official
medical and surgical records, Maher offers a richly detailed
picture of a little-known aspect of U.S. history. Of particular
interest for schools of nursing, Catholic educational institutions,
and history courses concerning women's studies, the Civil War
period, religion, and Catholicism.
