Highlighting the experiences of midwives who provide care to women opting outside of guidelines in the pursuit of physiological birth, Claire Feeley looks at the impact on midwives themselves, and explores how teams and organisations can support or discourage the promotion of women's birth choices. This book investigates the processes, experiences, and sociocultural-political influences upon midwives who support women's alternative birthing choice and argues for a shift in perspective from notions of an individual's professional responsibility to deliver woman-centred care, to a broader, collective responsibility. "Innovation Processes in Medical Education" is the first empirical study to investigate the barriers to successful educational change. Using a qualitative case study approach, it examines the factors that have either supported or impeded change in four medical schools: Case Western Reserve, University of Illinois, McMaster University, and Michigan State University. Following an overview of the historical forces that led to the current structure of the medical school curriculum; the authors discuss their research methodology and data, and analyze their attempts at innovation in each of the four cases, examining the relative merits of each effort. A matrix is developed that will help medical school innovators establish implementation strategies by showing them how to arrive at optimal strategies based on the specific context in which they work.

The organization and accomplishment of high quality care transitions relies upon the coordination of multiple professionals, working within and across multiple care processes, settings and organizations, each with their own distinct ways of working, profile of resources, and modes of organizing. In short, care transitions might easily be regarded as complex activities that take place within complex systems, which can make accomplishing high quality care challenging. As a subject of enquiry, care transitions are approached from many research, improvement and policy perspectives: from group psychology and human factors to social and political theory; from applied process re-engineering projects to exploratory ethnographic studies; from large-scale policy innovations to local improvements initiatives. Choice The role of the nursing home has expanded in the late twentieth century due to both the growing percentage of elderly in the U.S. population and to society's tendency to over-institutionalize people. In recent years, the kinds and quality of care given to the elderly in nursing homes have received intense scrutiny. This timely bibliography focuses on nursing assistants--the personnel who are with the elderly around the clock, doing a variety of tasks, ranging from helping them with basic functions to comforting them during periods of distress. Nursing assistants provide as much as 90 percent of the direct care received by the elderly in the nursing home setting. Emphasizing the psychosocial skills that make the nursing assistant's job so important to the well being of nursing home residents, Geriatric Nursing Assistants collects and annotates the heretofore scattered references to nursing assistants and includes literature pertinent to the construction of models that improve nursing-assistant practice. Activities necessary to further develop the nursing-assistant occupation, including political action, are investigated in Chapter Seven, which also considers the moral aspects of a progressive agenda for nursing assistants. This reference seeks to improve services to nursing home residents and represents a valuable, practical contribution to the geriatric field. It will be useful to nursing home administrators and directors of nursing homes who must address ways to improve the working conditions of nursing assistants; to academicians in their research, training, and advocacy efforts; and to the training directors and supervisors in the field who can directly aid nursing assistants in the acquisition of needed knowledge and skills.

International Classification for Nursing Practice (ICNP) functions as a common language for nursing in Europe. It is developed as a common nomenclature and classification system, reflecting the specific clinical realities of nursing practice. Telenurse promotes the use of ICNP in Europe according to a double strategy involving the following approaches; 1) organisation of dissemination conferences on a European level and dissemination meetings on a national level. 2. Building of simple demonstrators illustrating possible real uses of ICNP in shared electronic patient records. Telenurse has focused on two aspects of the use of ICNP in electronic records. Firstly, entry of ICNP data in relation to daily systematic documentation of nursing care and secondly, statistical analysis of ICNP data extracted from the electronic patient records in relation to the development of clinical quality and resource management. In relation to the DataIn phase Telenurse spans both streams within the group of projects concerned with electronic patient records, viz. the stream concerned with the architecture of electronic patient records and the stream concerned with the architecture of terminology's. In relation to the DataOut phase the small scale demonstrators of electronic patient records have been equipped with data extraction mechanisms for clinical databases. These developments have been supplemented by software showing how the results of the analysis of data stored in the databases can be fed back to clinical practice using electronic means.
Reacting against the dominance of obligation-based moral theories in both general and nursing ethics, the author proposes a 'strong' (action-guiding) account of a virtue-based approach to moral decision-making within contemporary nursing practice. Merits and criticisms of obligation and virtue-based approaches to morality are identified and examined.
