In 1976, the outbreak of a new strain of swine flu at the Fort Dix, New Jersey, army base prompted an unprecedented inoculation campaign. Some forty-two million Americans were vaccinated as the National Influenza Immunization Program hastened to prevent a pandemic, while the World Health Organization (WHO) took a wait-and-see approach. Fortunately, the virus did not spread, and only one death occurred. But instead of being lauded, American actions were subsequently denounced as a “fiasco” and instigator of mass panic. In Influenza, George Dehner examines the wide disparity in national and international responses to influenza pandemics, from the Russian flu of 1889 to the swine flu outbreak in 2009. He chronicles the technological and institutional progress made along the way and shows how these developments can shape an effective future policy. Early pandemic response relied on methods of quarantine and individual scientific research. In the aftermath of World War II, a consensus for cooperation and shared resources led to the creation of the WHO, under the auspices of the United Nations. Today, the WHO maintains a large and proactive role in responding to influenza outbreaks. International pandemic response, however, is only as strong as its weakest national link—most recently evidenced in the failed early detection of the 2009 swine flu in Mexico and the delayed reporting of the 2002 SARS outbreak in China. As DehnerÆs study contends, the hard lessons of the past highlight the need for a coordinated early warning system with full disclosure, shared technologies, and robust manufacturing capabilities. Until the “national” aspect can be removed from the international equation, responses will be hampered, and a threat to an individual remains a threat to all.
Respiratory syncytial virus (RSV) is an important respiratory pathogen of infants and young children and is the most common cause of lower respiratory tract infection-related hospitalization of children under the age of one. In this book, the authors present current research in the study of the prevention, diagnosis and treatment of RSV. Topics discussed include the genetic variables that play a role in the presentation and the outcome of severe RSV disease; cellular immunity and immunoproteomics of HLA class I ligands in HSRV infection and recurrent infections, chronic sequelae, and risk factors for RSV. This book looks at pulmonary diseases, their aetiology, diagnosis and treatment with a focus on bronchitis, emphysema and pneumonia. Bronchitis, an inflammation of the bronchi that carry air to the lungs, is examined in this book, as well as bronchiectasis, a severe form of bronchitis characterised by bronchial dilatation. Emphysema, on the other hand, is due to the destruction of lung tissue. This book describes a case of mediastinal and subcutaneous emphysema observed in the II stage of labour, otherwise known as healthy primigravida. Finally, Meconium aspiration syndrome (MAS), a major cause of respiratory morbidity and mortality in the term and post-term new-borns is explored, as well as the strategies for the management and prevention of chronic bronchitis (CB) and chronic obstructive pulmonary disease (COPD), a disease state associated with an abnormal inflammatory response of the lungs. Other chapters in this book discuss parapneumonic effusions (PPE) associated with bacterial pneumonia, the impact of ventilator-associated pneumonia in the Intensive Care Units and the effects of pranayama breathing patterns on the automatic nervous system.
This book provides an overview of sleep and sleep disorders for practicing clinicians. Sleep disorders represent a major portion of the chief complaints seen by pulmonologists and other clinicians. Patients with sleep-related conditions often present with non-specific complaints that require a broad and detailed knowledge of the wide range of sleep disorders and their consequences. This concise, evidence-based review of sleep medicine offers a guide to pulmonologists, primary care physicians, and all clinicians involved in caring for patients with sleep disorders. Providing a focused, scientific basis for the effects of sleep on human physiology, especially cardiac and respiratory physiology, chapters also outline a differential diagnosis for common sleep complaints and an evidence-based approach to diagnosis and management. This includes a review of the current standards of practice and of emerging technology and unresolved issues awaiting further research. In all, this book provides a clear diagnostic and management program for all the different sleep disorders and includes key points and summaries. This new edition expands the scope of the previous to include additional sleep disorders and the most affected populations. Six new chapters are added on health disparities in sleep medicine, models of care for patients with sleep disorders/care coordination, sleep disordered breathing in pediatric populations, sleep in hospitalized patients, sleep in pregnancy, and sleep in older patients. Essentials of Sleep Medicine is an invaluable resource for physicians, clinical psychologists, respiratory care practitioners, polysomnographic technologists, graduate students, clinical researchers, and other health professionals seeking an in-depth review of sleep medicine.
