Mit neu entwickelten diagnostischen Moeglichkeiten und verbesserten Therapieverfahren ist eine zunehmend differenzierte Behandlung von Lungenmetastasen boesartiger Tumoren moeglich geworden. Je nach Ursprungstumor und klinischer Situation wird ein unterschiedliches Vorgehen empfohlen. Die Entwicklung neuer Materialien und Techniken ftir die Gefass- chirurgie hat den Horizont der gesamten Chirurgie in ungeahnter Weise erweitert und befruchtet. Stand fruher im wesentlichen die sachgemasse Versorgung von Gefassen im Vordergrund, so bieten sich heute zahllose Moeglichkeiten einer Rekonstruktion der Blut- strombahn und daruber hinaus der Schaffung neuer Blutwege bis hin zum Anschluss transplantierter oder kunstlicher Organe. Kein Zweig unseres Faches ist hiervon ausgeschlossen. Wenn sich auch besondere Schwerpunkte der Gefasschirurgie entwickelt haben, so muss doch jeder Operateur zumindest die sein eigenes Interessen- gebiet betreffenden Gefasseingriffe beherrschen. Insofern mutet der hier und da geausserte Alleinvertretungsanspruch ftir die gesamte Gefasschirurgie kurios an. Viele Chirurgen sind von dieser sturmischen Entwicklung uber- holt worden, wahrend andere mangels Weiterbildungsmoeglichkei- ten oder eines geeigneten Krankengutes Muhe haben, diesen Fort- schritt mitzuvollziehen. Diese Hemmnisse zu uberwinden, hat sich der Autor ausdrucklich zum Ziel gesetzt. Die Gefasschirurgie -wie jede andere operative Tatigkeit -steht auf drei Saulen. Einmal muss der gegenwartige Stand des Wissens uber die Moeglichkeiten und Grenzen der Gefasschirurgie vorhan- den sein. Dann gilt es, die im Grunde einfachen, wenn auch durch besondere Sorgfaltspflicht gezeichneten Methoden der atraumati- schen Handhabung von Gefassen und deren Naht zu erlernen und schliesslich muss der Zugriff zum Gefasssystem "sitzen", das heisst, eindeutig, zweckmassig und schonend sein. Clinical cardiac electrophysiology is one of the most rapidly expanding fields in cardiology. There are currently no comprehensive case based books in this field. A Case Review of Cardiac Electrophysiology is a case based review of cardiac electrophysiology. The aim of this book is to provide a comprehensive case based review of cardiac electrophysiology. It will include implantable device cases as well as ablation cases and difficult clinical cases and may be used as a useful review in cardiac electrophysiology for those taking board examinations. There will also be cases that will be useful for associate professionals working in the field of cardiac electrophysiology including those individuals working for industry.
Approximately 120 different surgical procedures are used to correct congenital heart diseases, and the burden that this places on the surgeon is compounded by the exceptional complexity of the techniques and the rarity of many of the lesions. Training is problematic, not least because of legal issues, and currently available texts, drawings, and images are no substitute for real live surgery. This book is the ultimate reference on patent foramen ovale (PFO), a defect in the septum of the heart estimated to be present in more than twenty percent of the adult population and a proven cause of systemic embolism. All chapters have been written by internationally recognized experts in the field and cover PFO genetics and anatomy in addition to the effect PFO can have on various different organs and its supposed involvement in several conditions and clinical syndromes such as migraine, diving incidence, platynea orthodeoxia, economy class syndrome, sleep apnea and persistent desaturations in mild right ventricular dysfunction. Left ventricular assist devices (LVADs) have been shown to be superior to medications (beta blockers, digoxin, ace inhibitors, diuretics) for management of advanced heart failure. Other surgical alternatives, like mitral valve repair, aortic valve repair, and the Dor procedures (remodeling the shape of the left ventricle) are also important in the management of patients with CHF. These are topics that will all be covered in the book.
Esophageal cancer causes an estimated 386,000 deaths worldwide and is the sixth most common cause of death for men. The background characteristics of esophageal cancer treatment are markedly different between Asian and Western countries, however. In tumor histology, squamous cell carcinoma associated with smoking and alcohol consumption is overwhelmingly prevalent in Asia, whereas adenocarcinoma associated with Barrett's metaplasia is markedly prevalent in the West. In Asia, especially in Japan, the key persons who play important roles in the management of esophageal cancer patients are surgeons; in the West those roles are filled by medical and radiation oncologists as well as surgeons. The philosophy of surgeons regarding cancer surgery varies from locoregional to local tumor control, particularly in focusing on lymph node dissection. Physicians' approach to surgical adjuvant therapy differs, therefore, between Asia and the West. Considering these East-West differences in esophageal cancer treatment, the currently available results of Western evidence should not be considered directly applicable to esophageal cancer in Asia. In this book, the authors discuss the knowledge base in Japan in terms of treatment of esophageal squamous cell carcinoma. Since this volume contains a wide spectrum of current information and addresses topics surrounding the treatment of patients with esophageal squamous cell carcinoma, it is highly relevant to Asian physicians and researchers as well as to their counterparts in the West.
The Manual of Pulmonary Surgery has been designed primarily as a guide to the techniques of surgery on the lungs of adults and older children. The information included and the technical points stressed are those in which, in more than 20 years of teaching thoracic surgery, I have found residents to most commonly need instruction. An effort has been made to depict the usual anatomy of the lung and its more frequent variations, as it is encountered by the surgeon. This was necessary because a detailed description of pulmonary anatomy is not readily available. The excellent monograph on Segmental Anat omy of the Lung by Professor Edward Boyden, one of my teachers, is no longer in print. The techniques illustrated are those preferred by the author; no attempt has been made to describe all of the possible variations. Deanne McKeown has illustrated the entire book. Her drawings of the procedures are based on sketches or photographs made in the operating room. The use of full color illustrations to emphasize important technical or anatomical detail adds to the ease of understanding the text. A section on the indications for pulmonary resection that contains the information commonly used for surgical decision-making has been included.
Chest Atlas: Correlated Thin-Section Anatomy in Five Planes is a highly accurate, definitive anatomic reference of the chest. Anatomic specimens and specimen radiographs were obtained from fresh-frozen cadavers whose lungs were re-inflated to render the most accurate, life-like representation possible. The authors then meticulously matched computed tomography, trispiral tomogram, and magnetic resonance images of normal subjects to the corresponding anatomic section. A plain film radiograph of each section completes the correlation between cadaver and clinical imaging techniques. Each section from each plane is displayed on a two-page spread containing the labeled anatomic specimen in full color, the anatomic key, and the radiographic images, thus facilitating the correlation of the anatomic specimen and radiographs. The reader's understanding is also enhanced by: - fresh-frozen cadavers, meticulously prepared and sectioned - sectioning of all planes (axial, coronal, sagittal, left oblique and right oblique) - retention of normal tissue color - section keys to keep the reader oriented - uniform labeling key throughout the book - state-of-the-art clinical images, precisely correlated to the cadaver sections This atlas will be especially useful to radiologists, surgeons, and chest physicians who seek a definitive reference source on normal chest anatomy. This book will be an invaluable reference source for correlating diagnostic images with clinical findings.
This work is a concise review of the principles, procedures and clinical applications which every radiology resident and practicing general radiologist should know about imaging of chest diseases. Organized by anatomy and within each anatomical section by major disease catagory, this book covers the embryologic, clinical and pathological basis for each entity and discusses the radiologic diagnosis and post-treatment follow-up of these diseases. In addition to this coverage, there are chapters on chest trauma, ICU radiography, the post-operative chest, the immunocompromised host, bronchogenic carcinoma, physical agents, and the paediatric chest.
The third triennial congress of the International Society for Diseases of the Esophagus (lSDE) was held in Munich in September 1986 under the presidency of J. Rudiger Siewert. The very well organized and epoch-making meeting was part of the International Esophageal Week and was held in conjunction with the International Conference of Esophageal Diseases, the European Study Group of Diseases of the Esophagus (GEEMO), and International Organization for Statistical Studies of Esophageal Diseases (OESO). I am very pleased that it has been possible to publish the proceedings of the congress so rapidly. The problems associated with diseases of the esophagus are multifaceted, varying considerably according to locality; their study thus neces sitates a large international forum, which is one of the founding aims of the ISDE. We hope that this volume, by virtue of the in-depth and up-dated reports it contains, can playa role in advancing the work being done in this field.
