Rene Theophile Hyacinthe Laennec (1781-1826) is best known for his invention of the stethoscope, one of medicine's most powerful symbols. Histories, novels, and films have cloaked his life in hagiography and legend. Jacalyn Duffin's fascinating new biography relies on a vastly expanded foundation of primary source material, including thousands of pages of handwritten patient records, lecture notes, unpublished essays, and letters. She situates Laennec, the scientist and teacher, within the broader social and intellectual currents of post-Revolutionary France. Her work uncovers a complex character who participated actively in the dramatic changes of his time. Laennec's famous Treatise on Mediate Auscultation was his only published book, but two lesser known works were left in manuscript: an early treatise on pathological anatomy and a later set of lectures on disease. The three parts of Duffin's biography correspond to these books. First, she examines Laennec's student research on the emerging science of pathological anatomy, the background for his major achievement. Second, she uses his clinical records to trace the discovery and development of "mediate auscultation" (listening through an instrument, or mediator, to sounds within the human body). The stethoscope allowed clinicians to "see" the organic alterations inside their living patients' bodies. Finally, she explores the impact of auscultation on diagnostic practice and on concepts of disease. Analyzed here for the first time in their entirety, Laennec's College de France lectures reveal his criticism of over-enthusiastic extrapolations of his own method at the expense of the patient's story. Practising fundamental patient care skills and techniques is essential to the development of trainees' wider competencies in all medical specialties. After the success of simulation learning techniques used in other industries, such as aviation, this approach has been adopted into medical education. This book assists novice and experienced teachers in each of these fields to develop a teaching framework that incorporates simulation. The Manual of Simulation in Healthcare, Second Edition is fully revised and updated. New material includes a greater emphasis on patient safety, interprofessional education, and a more descriptive illustration of simulation in the areas of education, acute care medicine, and aviation. Divided into three sections, it ranges from the logistics of establishing a simulation and skills centre and the inherent problems with funding, equipment, staffing, and course development to the considerations for healthcare-centred simulation within medical education and the steps required to develop courses that comply with 'best practice' in medical education. Providing an in-depth understanding of how medical educators can best incorporate simulation teaching methodologies into their curricula, this book is an invaluable resource to teachers across all medical specialties.
Many people in developing countries lack access to health technologies, even basic ones. Why do these problems in access persist? What can be done to improve access to good health technologies, especially for poor people in poor countries? This book answers those questions by developing a comprehensive analytical framework for access and examining six case studies to explain why some health technologies achieved more access than others. The technologies include praziquantel (for the treatment of schistosomiasis), hepatitis B vaccine, malaria rapid diagnostic tests, vaccine vial monitors for temperature exposure, the Norplant implant contraceptive, and female condoms. Based on research studies commissioned by the Bill & Melinda Gates Foundation to better understand the development, adoption, and uptake of health technologies in poor countries, the book concludes with specific lessons on strategies to improve access. These lessons will be of keen interest to students of health and development, public health professionals, and health technology developers all who seek to improve access to health technologies in poor countries.
Bioengineering is the application of physical sciences and mathematics to the study of living organisms and structures. This book introduces the student to the physical processes and engineering aspects of a systems performance both under normal and abnormal conditions, and helps them to design, develop and use diagnostic or artificial devices to measure, improve, safeguard or replace life functions.
This open access book comprehensively covers the fundamentals of clinical data science, focusing on data collection, modelling and clinical applications. Topics covered in the first section on data collection include: data sources, data at scale (big data), data stewardship (FAIR data) and related privacy concerns. Aspects of predictive modelling using techniques such as classification, regression or clustering, and prediction model validation will be covered in the second section. Regional anaesthesia is currently undergoing a renaissance in paediatric anaesthetic practice, particularly in day case surgery, for post-operative pain relief and in ex-premature babies. However, most practical textbooks of local anaesthetic technique are not written with small children, and particularly newborn infants, in mind. This practical manual has been designed to aid both those aiming to refresh their knowledge of the techniques of regional anaesthesia in babies and children, and those learning these methods for the first time. Whilst it is not intended as a substitute for direct trainig by experts, its clear and concise text and simple diagrams make it an ideal companion to 'hands on' training in the operating theatre. Both authors have extensive experience of teaching anaestheitc trainees. The two introductory chapters of this book describe the relevant pharmacological and biological differences between infants, children and adults, and the general practical aspects of regional anaesthesia. The book is then divided into sections describing the nerve blocks of the major anatomical regions of the body. The descriptions of the different blocks can be read independently and include the relevant anatomy, indications, specific contraindictions, equipment, drugs and dosages, Each description concludes with a step-by-step guide to the block illustrated with photographs and simple diagrams.
In recent years, important technological innovations have made it possible to evaluate the small bowel endoscopically. Thus, traditional diagnostic methods such as imaging techniques are flanked by enteroscopy. The aim of this book is to present the results of enteroscopy as a method to diagnose small bowel disease, correlating it to the other investigative methods and above all to clinical findings. The book is a unique selected collection of images deriving from the results of documented experience and also including a brief clinical presentation, tables and diagrams of diagnostic algorithms. Renowned and accredited experts in gastroenterology, enteroscopy and researchers in the sector of small bowel disease have contributed to this volume.
This new book will enable family practitioners, obstetricians and pediatricians to answer the concerns of the families in their care. It is the first book to make information on this important topic so easily accessible to clinicians. The book is firmly rooted in clinical practice and based on many years of experience, much of it involving the development and implementation of new techniques for examining the fetus in utero. The techniques now available are assessed and better ways to identify pregnancies which are eligible to their application are proposed. The authors describe both the sampling and laboratory techniques available. Other sections cover both predictable and unpredictable pregnancy risks from genetic diseases to abnormal ultrasound, including the possibilities for laboratory evaluation, and the risks and limitations. Throughout the book, the special status of the fetus as a patient is emphasized and the concerns and questions of families are considered, making this the first fully comprehensive and authoritative volume on fetal medicine.
This is the second volume in an annual series reviewing important areas and advances in obstetric ultrasound, with wide-ranging reviews as well as chapters that look at chosen topics in more depth. This volume again draws together outstanding contributions from radiologists, obstetricians, and scientists to provide full coverage of clinical problems of particular current interest, such as extopic pregnancy, prenatal diagnosis of neural tube defects, fetal cardiac arrhythmias, and non-immune hydrops, while also covering litigations, equipment, and randomized trails of obstetric ultrasound.

Since 1945, a broad array of health care technologies have come into use, including antibiotics, anti-hypertensive drugs, oral diuretics, oral contraceptives, psycho-pharmaceuticals, corticosteroids, vaccines, open-heart surgery, genetics screening, automated clinical laboratories, renal dialysis, and cardiac pacemakers. Unquestionably, these technologies have brought benefits to millions. However, as costs of health care have risen rapidly, governments have increasingly singled out expensive technology as the culprit. The result has been changes in the methods of paying for health care in most countries to control cost rises. This has led to a slowing of technological change in some countries and increasing necessities to choose in all countries. This timely work describes how technology assessment critically evaluates the benefits, costs, and social implications of technology. The book presents an international perspective on health care technology's development and diffusion, and explains how health care technology can enlighten difficult choices faced by policy-makers, clinicians, and patients.
