Among them, acute myocardial infarction is the most common type of fatal disease, caused by the progression of atherosclerosis characterized by accumulation of choles terol in vascular walls. Development of atherosclerosis is greatly enhanced by major risk factors for cardiovascular diseases such as obesity, hyperlip idemia, diabetes (hyperglycemia), and hypertension. Among those, obesity frequently initiates a metabolic change that subsequently induces hyperlip idemia, diabetes, hypertension, and eventually atherosclerotic cardiovascu lar diseases. Because obesity and its related disorders largely depend on lifestyle factors such as high calorie intake and low physical activity, a series of disorders are termed lifestyle-related diseases."

Endocrine Replacement Therapy in Clinical Practice, an update of Hormone Repla- ment Therapy published by Humana Press in 1999, aims to assist the endocrinologist, gynecologist, pediatrician, urologist, general surgeon, neurologist, neurosurgeon, p- chiatrist, generalist, and trainee in management of their patients with hormonal defici- cies or altered hormonal synthesis or responses. Many new authors have added several new chapters, and all of the previous chapters have been updated. Endocrine testing used to diagnose endocrine disorders and monitor hormone replacement therapy is reviewed. However, detailed discussion of physiology and pathophysiology is not an aim of this book, and these topics are covered in other volumes on endocrinology. Since the observation in the 19th century that an extract of the suprarenal bodies injected into the circulation caused a rise in blood pressure, the endocrine system has become a major component in our understanding of human physiology. The introduction of radioimmunoassay techniques and the ability to measure minimal amounts of hor mones (a term derived from the Greek "to excite") have shown that acute exercise causes a release of a large number of hormones and that chronic exercise may further lead to long-term alterations in endocrine homeostasis. Actually, almost every organ and system in the body is affected by physical activity and exercise, much of it through the endocrine and neuroendocrine system. Investigation ofthe effect of acute or chronic physical activity on the endocrine system is a complex matter since the stimulus called "exercise" has many components, such as mode, intensity, duration, and others. In addition, several other factors, such as age, gender, training status, body temperature, circadian rhythm, metabolic state, menstrual cycle, and various external conditions as well as psychological factors, can modify the effect of physical activity on hormonal secretion. Moreover, the physiol9gical stimulus of exercise often provokes several and parallel cascades of biochemical and endocrine changes. It is therefore often extremely difficult to distinguish between primary and secondary events and between cause and effect. These limitations will be discussed in Chapter 1."

As widespread social transformations have been paralleled by gains in health and life expectancy through public health and other improvements, a variety of new obstacles to health have emerged.
