Early in 1985, a meeting was held to discuss the organization of a work shop in honour of Professor Reinier L. Zielhuis on the occasion of his retirement on November 30,1986. Various themes for such a workshop were considered, but it was Zielhuis himself, who ultimately selected "Health surveillance of individual workers exposed to chemical agents" as the theme of the workshop. Although this topic is of the utmost im portance for occupational health practice, it is also a very difficult one, given our currently limited knowledge on the subject. The choice of this topic is characteristic of Reinier Zielhuis, a scientist and scholar who throughout his career has been deeply concerned with the health and health risks of workers. Because he regards occupational and environmental exposure - including life style - as two aspects of the total exposure to chemical and physical factors, he emphasizes the need to assess the impact of this total exposure on workers' health. The impact of total exposure probably detennines to a large extent the in dividual variability in response. This was the main reason why he selected the above-mentioned topic for the workshop.

The observations made by Paracelsus concerning the dose-effect relationship of poison are generally just as applicable to health threatening vibration. With regard to kind, intensity, and duration of the vibration, the "dose" is decisive as to whether the conse quences are detrimental, unmeaningful, or tolerable with respect to health. This law of nature determines the tasks and goals of those whose aim is to safeguard health. Researchers worldwide have been occupying themselves with this many-faceted question for some time: how mechanical vibration affects the human organism and at what point damage occurs. If prevention in occupational medicine is to succeed, it is most important that the gaps in our present knowledge be closed, for if technical preventive measures are to be effective and preventive means in occupational medicine successful, they must be based on reliable and complete findings. Whenever many independent researchers have worked in a given field for a long period, a comprehensive intermediate as sessment is appropriate to evaluate the level achieved and the direction the research is going. In the area of hand-arm vibration, this evaluation has already been carried out, and the response aroused by this particular research report had led to a demand for a comparable evaluation of the research results on whole-body vibration. This report presents clearly and exhaustively the current status of international knowledge, as well as the questions that remain to be answered.
In 1980 the Directorate-General of Labor, Ministry of Social Affairs and Employment, the Netherlands, requested the Coronel Laboratory for Occupational and Environmental Health, Faculty of Medicine, University of Amsterdam, to carry out "a critical study of literature on health risks to women from industrial and occupational exposure to chemical agents which are different from risks to male workers, or which have only been observed in female workers. " The principal investigator was Mrs. A. Stijkel, medical biologist; a part of the study was carried out by R. L. Zielhuis, physician. M. M. Verberk, physician, and Mrs. M. v. d. Poel-Bot, librarian, provided continuous assistance. The final report (in Dutch) covering the literature up to and including 1981, was submitted to the government in October 1982. A somewhat abridged and modified English text, updated to include 1982, was prepared. The Editorial Board and the publisher of the International Archives of Occupational and Environmental Health kindly made possible the publishing of this text in a special issue of the journal. The authors express their thanks to the Directorate-General of Labor, the Editorial Board and the publisher for making this publication possible. The springboard for this sixth volume in the Industry and Health Care series was a conference sponsored by the Center for Industry and Health Care of Boston University on June 9 and 10, 1978. That conference had a gradual genesis. Over a year ago we spent some time with Kevin Stokeld of Deere and Company and heard his views on self-insurance and self-administration as one device for a corporation to achieve better management control of its health benefit. More recent discussions with representatives of American Telephone and Telegraph Company and other corporations made it increasingly clear to us that management's need for data to monitor the use of employee health benefits was emerging as a critical policy issue. Subsequent meetings with executives at John Hancock Mutual Life Insurance Company in Boston and Mobil Oil Corporation in New York, among others, convinced us that simple answers would be elusive or inadequate and that there was a need for an objective and careful look at the evolving relationships between employee health benefits, claims administration, health services utilization, and corpo rate health care cost containment programs. Since self-funding and particularly self-administration represent a fun damental change in the traditional insurance relationship, the conference was convened to explore the advantages and disadvantages of self-insurance for employee health benefits, with some attention to claims production but with special emphasis on the originating question of data for effective management of an employee health benefit."
Work has never been as safe as it seems today. Safety has also never been as bureaucratized as it is today. Over the past two decades, the number of safety rules and statutes has exploded, and organizations themselves are creating ever more internal compliance requirements. At the same time, progress on safety has slowed to a crawl. Many incident- and injury rates have flatlined. Worse, excellent safety performance on low-consequence events tends to increase the risk of fatalities and disasters. Bureaucracy and compliance now seem less about managing the safety of the workers we are responsible for, and more about managing the liability of the people they work for. We make workers do a lot that does nothing to improve their success locally. Paradoxically, such tightening of safety bureaucracy robs us of exactly the source of human insight, creativity and resilience that can tell us how success is actually created, and where the next accident may well happen. It is time for Safety Anarchists: people who trust people more than process, who rely on horizontally coordinating experiences and innovations, who push back against petty rules and coercive compliance, and who help recover the dignity and expertise of human work.
Safety-I is defined as the freedom from unacceptable harm. The purpose of traditional safety management is therefore to find ways to ensure this 'freedom'. But as socio-technical systems steadily have become larger and less tractable, this has become harder to do. Resilience engineering pointed out from the very beginning that resilient performance - an organisation's ability to function as required under expected and unexpected conditions alike - required more than the prevention of incidents and accidents. This developed into a new interpretation of safety (Safety-II) and consequently a new form of safety management. Safety-II changes safety management from protective safety and a focus on how things can go wrong, to productive safety and a focus on how things can and do go well. For Safety-II, the aim is not just the elimination of hazards and the prevention of failures and malfunctions but also how best to develop an organisation's potentials for resilient performance - the way it responds, monitors, learns, and anticipates. That requires models and methods that go beyond the Safety-I toolbox. This book introduces a comprehensive approach for the management of Safety-II, called the Resilience Assessment Grid (RAG). It explains the principles of the RAG and how it can be used to develop the resilience potentials. The RAG provides four sets of diagnostic and formative questions that can be tailored to any organisation. The questions are based on the principles of resilience engineering and backed by practical experience from several domains. Safety-II in Practice is for both the safety professional and academic reader. For the professional, it presents a workable method (RAG) for the management of Safety-II, with a proven track record. For academic and student readers, the book is a concise and practical presentation of resilience engineering.
