Bones and joints are always under mechanical loading a key concept in understanding bone metabolism. Among the most common diseases of bones and joints in the elderly are osteoporosis and joint osteoarthritis. Dynamic changes in mechanical loading give rise to problems resulting in stenosis of the spinal column at the cervical, thoracic, and lumbar levels. Mechanical loading also accelerates joint destruction caused by inflammation from such conditions as chronic rheumatoid arthritis. An understanding of mechanical loading is essential therefore to clinicians, basic researchers, and engineers working with bones and joints. Providing up-to-date research and clinical findings, the contents of this volume are from the papers, symposia, and special lectures presented at the 12th Annual Meeting of the Orthopaedic Research Meeting of the Japanese Orthopaedic Association in Niigata, in October 1997.
Total joint arthroplasty has brought manifold benefits to patients suffering from joint diseases, but the durability of prostheses remain a persistent problem, often requiring correction through revision surgery. Among the issues remaining to be resolved are wear and periprosthetic osteolysis after total joint arthroplasty. Today's researchers are investigating the pathogenesis and reduction of osteolysis and wear, and practitioners and engineers are working together to develop new types of prostheses for total hip and knee arthroplasty. This book presents the work of eminent orthopedic surgeons, biomedical engineers, and other researchers who have devoted their efforts to the creation and implementation of more durable total joint prostheses. The latest information on new concepts and clinical trials is included, with chapters on ostcolysis, recent developments in metal-on-metal and ceramic-to-ceramic articulation, and future trends in total joint arthroplasty.

Dr. Jay A. Goldstein 's Betrayal by the Brain can be a daunting treatise to the layperson as well as a challenge for the physician who is not versed in neurology, psychiatry, immunology, and endocrinology. Here, in A Companion Volume to Dr. Jay A. Goldstein 's Betrayal by the Brain: A Guide for Patients and Their Physicians, Author Katie Courmel, a longtime sufferer of chronic fatigue syndrome and fibromyalgia, presents in layperson 's terms the salient points of the book to lead readers to an understanding of Dr. Goldstein 's theory and methods of treatment. This guide provides CFS and FMS patients with an understanding of how their brains should function as opposed to how they are now dysfunctioning. It explains the proper role and function of each brain structure and neural assembly and the neurochemicals believed to be implicated in CFS and fibromyalgia along with the dysfunctions found in CFS/FMS patients. These dysfunctions, as a whole, form the basis of Dr. Goldstein 's unified theory behind the possible causes of CFS and FMS. Patients involved in or contemplating Dr. Goldstein 's treatment can use A Companion Volume to Dr. Jay A. Goldstein 's Betrayal by the Brain to gain an explanation of the process from a firsthand perspective and can follow along to learn why they are given particular drugs and what response Dr. Goldstein hopes to elicit from their bodies. This understanding gives patients a sense of control and helps them weather the ups and downs their bodies experience as a normal response to the treatment. The strength of A Companion Volume to Dr. Jay A. Goldstein 's Betrayal by the Brain lies in its ability to simplify without sacrificing the body of information that one must understand to make sense of the multitude of seemingly unrelated manifestations of these diseases of the brain. A thorough understanding will allow sufferers of CFS/FMS to approach Dr. Goldstein 's treatment method with an open mind and improve their chances for success. Specific questions answered for readers include: how safe is this protocol? should I go alone or take a companion? what should I expect during my initial consultation? what are the effects of so many drugs? why doesn t the same drug work for everyone? if my treatment is successful, how long will it last? what happens when I leave Dr. Goldstein 's care? A Companion Volume to Dr. Jay A. Goldstein 's Betrayal by the Brain prepares readers for a unique, sometimes scary, often perplexing, hopefully miraculous, leap-of-faith experience. It helps patients deal with the very natural sense of uneasiness they may be feeling and helps them understand, in layperson 's terms, what Dr. Goldstein 's protocol can accomplish and how it can be so successful.

The term "minimally invasive spinal surgery" was coined in early 1990 following pub- cation of the first edition of this text entitled Arthroscopic Microdiscectomy: Minimal Intervention in Spinal Surgery, and subsequent establishment of the International Society for Minimal Intervention in Spinal Surgery (ISMISS) under the auspices of the International Society of Orthopaedic Surgery and Traumatology (SICOT) in April l990. The orthopedic and neurological surgeons who participated in lectures and hands-on wo- shops both in Philadelphia and abroad have witnessed the evolution of minimally invasive spinal surgery from blind nucleotomy to endoscopic fragmentectomy, decompression of l- eral recess stenosis, foraminoplasty, and spinal stabilization. In Arthroscopic and Endoscopic Spinal Surgery: Text and Atlas, Second Edition, experts describe and illustrate various techniques and approaches that are currently used in this field. In addition, the ongoing research for the betterment of spine care via minimally invasive approaches is briefly reviewed. I would like to express my sincere appreciation to so many of my colleagues who s- ported my efforts in the field of minimally invasive spinal surgery throughout the years. Many of them participated in our teaching symposiums and have provided valuable cont- butions to this text.

Minimally Invasive Surgery of the Foot and Ankle represents a novel approach to treatment of orthopedic problems in the foot and ankle. The gradual change of philosophy in the management of foot and ankle surgery means that patients require a less invasive approach to surgery and a consequent improvement in recovery time. Describing the techniques and, importantly, the indications for minimally invasive procedures for the management of foot and ankle ailments, this book will explain the management of various conditions and how they can be approached using minimally invasive techniques. However, rather than only concentrating on minimally invasive surgery of the foot and ankle, the authors will be examining the options open to surgeons operating in this area both open surgical and arthroscopic and explaining the benefits of each. Extensive radiographs, diagrams, and intra-operative pictures will illustrate the procedures described.

Bone is ubiquitous and versatile, and uniquely repairs itself without scarring. However, we rarely see bone in its living state-and even then, mostly in two-tone images that only hint at its marvels. After it serves and protects vertebrate lives, bone reveals itself in surprising ways, sometimes hundreds of millions of years later. In Bones, orthopaedic surgeon Roy Meals explores and extols this amazing material that both supports and records vertebrate life. He demystifies the biological makeup of bones; how they grow, break and heal; and how medical innovations-from the first X-rays to advanced surgical techniques-enhance our lives. With enthusiasm and humour, Meals also reveals the enduring presence of bone outside the body-as fossils, ossuaries, tools, musical instruments-and celebrates allusions to bone in history, religion and idiom. Approachable and entertaining, Bones richly illuminates our bodies' essential framework. |
