Integrating Population Outcomes, Biological Mechanisms and Research Methods in the Study of Human Milk and Lactation is the product of the 10th Conference of the International Society for Research on Human Milk and Lactation, held on September 15-19, 2000, in Tucson, Arizona. The presented sessions at the meeting are as diverse as the volume itself. These sessions include the impact of micronutrient deficiencies during lactation on maternal and infant health, the premature infant, developmental immunology, breastfeeding in the industrialized world, and viral transmission in milk. Whenever possible, the sessions were organized to include human population research, research showing the biological underpinnings of the effects on human health, and important methodological issues. This volume is a contemporary and influential tool for human milk biologists, breastfeeding epidemiologists, biochemists, immunologists, clinical specialists, and all professionals and researchers in the field.

We are most pleased to present Volume 3 of Developmental-Behavioral Disorders: Selected Topics, designed to serve as a companion for standard reference textbooks that address cogent issues in developmental pedi atrics. Periodic publications such as Selected Topics and theme-related articles, as well as continuing education programs, attempt to supple ment in a timely fashion the rapidly changing knowledge base in devel opmental-behavioral pediatrics. These media are important as forums for enhancing the quality of clinical practice, teaching skills, and re search activities. The need is critical for periodically disseminating and updating information about issues in developmental medicine, in as much as this field of study continues to expand at a meteoric pace. During the past several decades, developmental medicine has been recognized as a defined subspecialty in pediatrics. The spectrum of problems encompassed by this discipline is relatively broad and at times clinically overwhelming. The ultimate goal of preventing delays, disor ders, and/or dysfunctions from becoming chronic handicapping condi tions has, by volume per se, created clinical dilemmas for pediatric health care providers. There are numerous facets of providing efficient and effective care, which in the field of developmental-behavioral pediatrics are often exaggerated impediments to the delivery of services by pri mary health care specialists, e. g., time, clinical skills, need for inter disciplinary management, medical-legal responsibilities, financial reim bursements. These issues, as well as clinical problems, are still very much part of the information base to be disseminated to concerned pro fessionals."

Research into child language development is being conducted more extensively, by more people, and in more countries throughout the world than at any point in the past. We now know more than we ever did before about the linguistic, cognitive, neurobio logical, and social capacities that children bring to their language learning task, as well as the ways in which these capacities interact with a whole range of factors present in the child's environment throughout the learning process, not least the particular character istics of the language or languages to which the child is exposed. Far from inducing a sense of complacency, this new knowledge seems instead to create an even stronger impetus to know more. This is partly because of the new and unforeseen questions it raises and partly because of competition between different theories to provide the best explanation of the phenomenon, and which in turn engender different methods of enquiry. This volume addresses many of the key issues which are currently exercising the minds of child language researchers, and which are likely to motivate research for some years to come. The chapters included here-all previously unpublished-present a great deal of new data and address a range of theoretical issues from a variety of perspectives.

In most peroxisomal disorders the nervous system is severely affected which explains the clinical and community burden they represent. This is the first book to focus not only on the mutations causing these inherited illnesses, but also on mechanisms that regulate, suppress or enhance expression of genes and their products (enzymes). Indeed since the success and completion of the Human Genome Project all genes (coding DNA sequences) are known. However, of many, their function, and the role of the gene product has not been determined. An example is X-linked adrenoleukodystrophy, the most frequent peroxisomal disorder. Children are born healthy, but in more than 1 out of 3, demyelination of the brain starts unpredictably and they die in a vegetative state. The gene mutated in most families has been known for 10 years; but the true role of the encoded protein, ALDp, is still speculative; and within the same family, very severe and asymptomatic clinical histories co-exist, unexplained by the mutation.

Although breast-feeding has long been associated with lowered infant morbid- ity and mortality from infectious disease, until relatively recently little was known regarding the individual components of human milk aside from their nutritive func- tions and the presence of secretory antibodies. Over the last 40 years, and especially over the last decade, evidence has been growing that human milk contains a large number of materials that are bioactive and that are not found in artificially formu- latedinfantdiets. Disparatelinesofresearcharecurrentlyproducingsurprisinglylong listsofnewlyrecognizedhumanmilkcomponents-antimicrobialsand immunomod- ulators, includinganti-inflammatoryagents, antioxidants, cytokines, andhormones- with biological activities that relate to pathogenesis, inflammation, development, metabolic regulation, and other functions. The sum of all of these biologically active milk components may account for the strong protection that human milk affords nursing infants. Strictly speaking, most components of human milk could be considered bioac- tive, since nutrients are bioactive by definition. A major emphasis of this book, how- ever, is on defining what is known about components of human milk that inhibit common pathogens of the infant, those that have hormonal and/or cytokine activity, those that have immunomodulatory and/or anti-inflammatory activity, xenobiotics, and nutrients that are uniquely essential to early development. The topic of bioactive substances in human milk was explored in depth at the th 8 International Conference of the International Society for Research on Human Milk and Lactation (ISRHML) held at Plymouth, Massachusetts, October 25-29, 1997. This book contains the proceedings of that conference.

Osteosarcoma in Adolescents and Young Adults: New Developments and Controversies is the fifth volume in the series `Cancer Treatment, and Research' devoted to pediatric oncology. Like its immediate predecessor, Hodgkin's Disease in Children: Controversies and Current Practice, this volume also deals with the current state of affairs: where do researchers differ in opinions or firmly held beliefs? What does the future hold for research or for the patients who have benefited from research and are now cured? etc. The first part deals with preclinical and clinical issues that relate to the disease and the second part deals with the results of recent trials and in some cases ongoing trials. It could be argued that section B Pharmacology, Section D Radiotherapy and Section E Surgery of part one belong to part two; however in these sections we have tried to focus on one discipline and in part two, we have tried to focus on the results of multidisciplinary trials. A third part includes a few critiques and editorial comments. This volume was prepared and planned for both basic and clinical oncologists. It is hoped that the basic scientists will gain some insight into the problems of their clinical colleagues and that the clinicians will be willing to read to the more basic articles. It is not a text for students (or for that matter physicians) who want all things to be laid out in `black and white'. But it is a text for those students who want to know how vibrant this field of research is and are willing to stand in awe of how much research needs to be done.

This book represents the proceedings from a conference that took place in Dallas in the spring of 1999 which was entitled "Pediatric Gender Assignment - A Critical Reappraisal". Some participants rightfully argued that the conference really focused on the issue of pediatric gender assignment, and that reassignment was not applied in most cases. Their comments were reflected in the title of this monograph. This multidisciplinary meeting was sponsored by a conference grant from the National Institutes of Health, and a broad inquiry into this complex topic took place from many points of view. Basic scientists offered insight into mechanisms of sexual differentiation of the gonads, physical phenotype and imprinting of the central nervous system. Endocrinologists reviewed their experience in diagnosis and management, surgeons described traditional as well as innovative approaches, and there was strong representation from the ethical and behavioral sciences. In putting together such a panel, it was essential that we identify a cast of speakers who could address their viewpoints with strong convictions, and yet not let their passions render the meeting counter productive. We were not disappointed. While many differing points of view were firmly expressed by the panelists and audience, all viewpoints were accorded the respect they deserved. The concept behind the meeting and this book really originated in 1997 shortly after Diamond and Sigmundson published their long term follow up study of the John/Joan case.

Progress in basic and clinical immunology within the last two decades has provided profound insight into the immune system and its role in preventing endogenous and exogenous damage. In contrast, disbalances within this system can result in autoimmune disorders which may affect diverse organs and result in distinct clinical pictures. In many of these, however, the individual etiopathogenetic mechanisms are poorly understood and even more their clinical symptoms are hard to treat. The book offers insight into basic mechanisms of autoimmune disorders. It includes neurological, gastrointestinal, ophthalmological and skin diseases as well as current and future therapeutic options including immunomodulatory drugs and different vaccination strategies. By addressing diverse organ systems, both singular and shared features are elaborated. Thus an exchange of ideas is intended across research on single organ systems within a truly interdisciplinary setting.

It is estimated that the functionally significant body of knowledge for a given medical specialty changes radically every 8 years. New specialties and "sub specialization" are occurring at approximately an equal rate. Historically, estab lished journals have not been able either to absorb this increase in publishable material or to extend their readership to the new specialists. International and national meetings, symposia and seminars, workshops, and newsletters success fully bring to the attention of physicians within developing specialties what is occurring, but generally only in demonstration form without providing historical perspective, pathoanatomical correlates, or extensive discussion. Page and time limitations oblige the authors to present only the essence of their material. Pediatric neurosurgery is an example of a specialty that has developed during the past 15 years. Over this period, neurosurgeons have obtained special train ing in pediatric neurosurgery, and then dedicated themselves primarily to its practice. Centers, Chairs, and educational programs have been established as groups of neurosurgeons in different countries throughout the world organized themselves respectively into national and international societies for pediatric neurosurgery. These events were both preceded and followed by specialized courses, national and international journals, and ever-increasing clinical and investigative studies into all aspects of surgically treatable diseases of the child's nervous system."

Books on adolescence have been written for a variety of purposes. Hall's (1904) two volume Adolescence encompasses most of them: to advocate a particular theoretical approach to adolescence, to stimulate use of a particular brand of scientific methodology when studying youth, to address issues of the basic nature and importance of adolescence, and to propose recommendations on how adolescents ought to be treated and educated. In Hall's words, "It [the two volumes] constitutes the first attempt to bring together the various aspects of its vast and complex theme" (xix), a full survey of "pedagogic matter and method. " This is necessary because, "In no psychic soil, too, does seed, bad as well as good, strike such deep root, grow so rankly, or bear fruit so quickly or so surely" (xviii-xix). Mead (1928) retorted with Coming of Age in Samoa, a refutation of Hall's conclusions: "Are the disturbances which vex our adolescents due to the nature of adolescence itself or to the civilization? Under different conditions does adolescence present a different picture" (p. II). Thus, Mead wanted to correct a theoretical injustice and to promote the impact that culture has on the developing adolescent personality. Hollingshead's (1949) Elmtown's Youth was produced to further support Mead's conclusions-"Is the social behavior of an adolescent a function of physiological changes in the maturing individual or of his experiences in society?" (p. 6). His answer was clearly the latter, specifically the family's economic and social status within Elmtown's social structure.

Infection of the lower respiratory tract is a major determinant of the course of cystic fibrosis. Although numerous efforts have been made to elucidate the specific mechanisms predisposing the respiratory mucosa of cystic fibrosis patients to infection, so far no clinically relevant procedures for completely effective prevention or control of infection have resulted. Hence, in dealing with infections in cystic fibrosis, we continue to rely mainly on antimicrobials. Antiinfective measures are inseparably correlated with microbiology, and the quality of antiinfec tive therapy directly reflects the quality of microbial monitoring. Vali dated guidelines for microbiologic testing and antiinfective use need to be developed and made available to all health providers and their cystic fibrosis patients. Several years ago, the editors cochaired a symposium at the Interna tional Congress of Chemotherapy on the Global Perspectives of Micro biological and Clinical Infectious Diseases in Patients with Cystic Fibrosis. During this half-day symposium, the editors heard reports from several countries around the world with an alarming range of survival for patients with cystic fibrosis. This sent a dramatic message to us that the understanding of this disease, its diagnosis, management and prevention was different in various countries and that patients may be inconsistently served. That is how our journey began."

The book series, "Child Maltreatment: Contemporary Issues in Research and Policy." will consist of a state of the art handbook (to be revised every five years) and two to three volumes per year. The first volume in this seriesis a legacy to C. Henry Kempe. This is a timely publication because 2012 marks 50 years after the appearance of the foundational article by C. Henry Kempe and his colleagues, "The Battered-Child Syndrome." This volume capitalizes on this 50 year anniversary to stand back and assess the field from the perspective that Dr. Kempe's early contributions and ideas are still being played out in practice and policy today. The volume will be released at the next ISPCAN meeting, also in 2012. "
