This atlas presents a collection of richly illustrated teaching cases. It covers the fundamentals of orthopedic oncology complemented with relevant aspects that are demonstrated using individual cases. In a specialty that deals with a relatively smaller number of cases compared to tumors of other systems, this atlas prepares readers for clinical practice by combining a problem-based learning (PBL) approach, which lies on the continuum between structured and guided learning, with theory and practical insights. Through fourteen years of work as a surgeon in Erbil, Iraq, Amir Al-Dabbagh built a sense of genuine trust and friendship with his patients, the society of Erbil, his colleagues, and students in the city, a feeling that continues to inspire him to return to the city, and record his experiences in this memoir. Amir Al-Dabbagh started his medical career in 1970, graduating from Mosul and working in Iraq for six years before a period practicing medicine and surgery in the UK. After being forced into leaving Iraq in 1993, Amir continued working in the UK at Trafford General Hospital and the Manchester Royal Infirmary. His career has been one of patient care and student training, as a consultant surgeon and dean of a medical college in Erbil through peace and war. This memoir is a collection of thoughts and experiences of the medical profession through times of war and inner turmoil, fleeing as a refugee and collected wisdom on undergraduate and postgraduate training as well as good practice in patient care and surgery. The author reflects on the importance of family and friends and a genuine love of Erbil and Iraq, a beautiful country overflowing with history and welcoming people. Ultimately, nothing would have forced him to leave Erbil but the direct threat on his life. Also included is an account of Dr Waleed Khalid's practice in Baghdad, as consultant cardiothoracic and vascular surgeon during the Iraq-Iran War and the First Gulf War, providing another interesting perspective on striving to care for one's patients as a surgeon in turbulent times. Dr Waleed's perseverance to build the first cardiothoracic and vascular unit in the north of Iraq was fraught with many obstacles, which he clearly highlights in his account.
Untersuchungen iiber das Tonusproblem im besonderen der Hohl organe haben mich schon vor Jahren zum Begriff "Tonuskrankheit" gefiihrt. Seine Begriindung hat sich durch die Feststellung ergeben, daB es Krankheiten gibt, deren Wesen in einem pathologischen Ver halten des Tonussubstrates im Sarkoplasma gelegen ist. Es hat sich ferner gezeigt, daB in heterogenen Krankheiten in diesem Veranderungen vor sich gehen, die in entscheidender Weise in den Veriauf eingreifen. In zahlreichen Mitteilungen habe ich Experimentelles und Klinisches veroffentlicht, das mir geeignet schien, das Verstehen der Tonusveran. derungen, von welchen hier die Rede ist, zu unterstiitzen. Wiederholt hat man mir nahe gelegt, meine einschlagigen Arbeiten zusammenzu. fassen. In den letzten Jahren haben sich Momente ergeben, die mich bestimmen, dieser Anregung Folge zu Ieisten. Die vorliegende Schrift bringt neben einem allgemeinen Uberblick nur einen Abschnitt aus diesem Gebiet, die Tonuskrankheiten der Kreisiauforgane, darunter weitere Ergebnisse meiner Studien. Meinen Ausfiihrungen Hegen eigene Beobachtungen zugrunde und bringe ich von diesen vor, was mir geeignet erscheint, die weitere Entwicklung dieses Teiles der pathologiscben Biologie zu fordern. Eine abschlieBende Darstellung kann icb bei dem gegenwartigen Stand meiner Kenntnisse nicht bieten, doch auf nicht gelOste Fragen aufmerksam machen. Mir ist es darum zu tun, meinen Grundgedanken darzulegen und ihn den Erwagungen des Praktikers naher zu bringen. Es erscheint mir das im Interesse der Kranken und ihrer Behandlung gelegen. Diesem Bestreben entsprechend war ich bemiiht, den Umfang dieser Arbeit tunlicbst einzuengen. Trotzdem habe ich manche Wiederholung wegen der leichteren Verstandigung nicht vermieden."

Der Aufforderung des Verlages, ein gemeinverstandliches kleines Buch auf streng wissenschaftlicher Grundlage iiber das gesunde und das kranke Herz und die BlutgefaBe zu schreiben, folgte ich um so lieber, als das Wissen und Verstehen des Kranken, um das, was ihm fehIt, mir die Vorbedingung fUr eine erfolgreiche Behandlung der Herz-und GefaBleiden zu sein scheint. Der Arzt ist auf die Mitarbeit des Kranken hier bewnders angewiesen; sie kann aber nur eintreten, wenn dem Kranken zunachst die Grundlagen des Ablaufes der Herztatigkeit verstandlich sind. Auch die Verhiitung von Herzkrankheiten wird dadurch auf eine ganz andere, sicherere Grundlage gestellt. Der Gesunde wird so erkennen, wodurch er sich vor Schaden bewahren kann. Der Kranke wird sehen, daB gerade bei Herz-und GefaBleiden unter richtiger, bewuBter Lebens fiihrung ein wahrhafter Optimismus angebracht ist. Von einer schematischen FremdworterausschaItung wurde abgesehen. Selbstverstandlich muB jedes Fremdwort erklart werden. Erfahrungen in Unterricht und Vortrag haben mich indes gelehrt, daB es fiir den Laien sicher leichter ist, "Arterien" und "Venen" zu unterscheiden als "Schlag-" oder "Pulsadern" und "Blutadern." Auf die Psychologie des Lernenden muB Riicksicht genommen werden. Fremdworterausmerzung solI ja das Verstandnis heben, nicht erschweren. Wo die entgegengesetzte Wirkung eintritt, muB das Fremdwort bleiben. Man wird hier allch so einfache, aber praktisch wichtige Dinge finden wie Anleitung zum Pulsfiihlen. Sie ist in keinem anderen derartigen Buch ange geben, und dabei doch mindestens so bedeutungsvoll auch fiir den Laien wie Messen der Korperwarme. Ein besonderer Abschnitt ist der heute sehr wichtigen und aktuellen Frage der Beziehungen zwischen Sport und Herz gewidmet."

Diese Auffassung erscheint um so berechtigter, als gerade dies Gebiet in den letzten Jahren ungemein fleiBig bearbeitet und ausgebaut wurde: viele neue, zum Teil glanzende Tatsachen hat die Physiologie gelehrt, unerwartete diagnostische Mittel sind UIlS gebracht, vor aHem abel' ist del' therapeutische Apparat in einer Weise angewachsen und kompliziert, daB nicht mehr das Bessere, sondern das Viele der Feind des Guten geworden ist, wie del' kritische F.
