La ricerca neuropsicologica degli ultimi trenta anni ci ha fornito molte nuove informazioni sulla rappresentazione delle competenze semantiche nel cervello e sugli effetti di una loro compromissione conseguente a un danno cerebrale. Tali effetti si manifestano non solo nella capacita del soggetto di utilizzare il linguaggio ma anche in numerose attivita intelligenti extraverbali come l interazione con oggetti di uso quotidiano. Conoscere il funzionamento fisiologico e la compromissione patologica della memoria semantica ci ha messo in grado di costruire alcuni esercizi riabilitativi mirati e scientificamente fondati per far fronte alle esigenze del soggetto con disturbo semantico. Il presente strumento propone tre diversi esercizi riabilitativi, tutti basati sul modello componenziale delle rappresentazioni semantiche, che a nostro avviso e quello che ha goduto del maggior numero di conferme sperimentali dalla ricerca neuropsicologica. Nell esercizio Seleziona, al soggetto e richiesto di selezionare gli items target tra un certo numero di distrattori in base a un particolare tratto semantico (p.es. cerca quelli con le ali). La sclerosi multipla rappresenta una delle cause piu frequenti di disabilita cronica nei giovani adulti. Essa colpisce, soprattutto, individui tra i 20 e i 50 anni di eta, con un picco di incidenza intorno ai 30 anni. Tale malattia e caratterizzata dalla presenza di lesioni demielinizzanti di natura infiammatoria disseminate nel sistema nervoso centrale. La diversa localizzazione di tali lesioni e alla base della variabilita della sintomatologia clinica che puo' essere caratterizzata da disturbi motori, sensitivi, neurovegetativi e cognitivi. Questi ultimi riguardano circa la meta dei pazienti affetti da tale patologia e hanno una notevole incidenza sulla situazione personale, familiare e lavorativa di tali pazienti. La necessita di sottoporre i pazienti affetti da sclerosi multipla a una terapia riabilitativa degli eventuali deficit cognitivi e, pero', acquisizione recente. Il presente volume si propone di offrire una visione d'insieme degli aspetti epidemiologici, patogenetici e clinici della malattia; particolare rilievo sara dato agli aspetti cognitivi e psico - emotivi e alla loro relazione con le caratteristiche del danno anatomico evidenziato dagli esami di neuroimaging. A partire dagli anni '80, numerosi ricercatori nel campo della riabilitazione dei disturbi del linguaggio del cerebroleso adulto hanno introdotto la distinzione tra competenza linguistica e competenza comunicativa. Gli stessi autori hanno ipotizzato che l'approccio riabilitativo, ivi inclusa la valutazione del paziente, non dovrebbe essere indirizzato esclusivamente al recupero di abilita linguistiche ma contemplare anche trattamenti volti a sviluppare le capacita di interazione comunicativa in particolari contesti. Questi trattamenti, cosiddette terapie ad orientamento pragmatico-funzionale, sono stati inizialmente concepiti con carattere "complementare" o "sostitutivo" alle terapie di stimolazione del linguaggio, e hanno visto scarsa diffusione nel nostro paese nonostante la loro dimostrata efficacia. Negli anni successivi, l'acquisizione da parte dei neuropsicologi che la variabile contestuale interagisce significativamente con il "processing" linguistico del paziente cerebroleso ha comportato la formulazione di nuovi paradigmi di trattamento. Uno di questi e certamente la terapia P.A.C.E. che ha conosciuto ampia diffusione nel mondo occidentale per la sua riconosciuta efficacia ed il suo rigoroso impianto metodologico.
La parola afasia denota i deficit del linguaggio secondari a patologie cerebrali focali e diffuse e puo assumere forme diversissime, a seconda dei meccanismi linguistici colpiti o risparmiati dalla lesione cerebrale. Il disturbo afasico danneggia le capacita comunicative (e spesso anche lavorative) e non e migliorato, tranne eccezioni, dalle terapie mediche disponibili. Le possibilita di incidere su questo deficit sono dunque affidate, per il momento, alla logopedia. Il presupposto per un trattamento riabilitativo adeguato ed efficace e una diagnosi neuropsicologica accurata che stabilisca il livello di compromissione del sistema linguistico e identifichi le componenti cognitive risparmiate o compromesse dal danno cerebrale. L'esame neuropsicologico dell'afasia (E.N.P.A.) qui proposto comprende una serie di test che valutano la presenza, la gravita e le principali caratteristiche dell'afasia nel soggetto esaminato. Per semplicita e brevita l'E.N.P.A. puo essere somministrato anche a soggetti gravi e/o di livello scolare non elevato.
