Behcet's syndrome can reasonably be considered a unique entity among diseases of the immune system for several reasons: It has specific features and, uniquely among the immune system pathologies, represents a link between autoimmune diseases, systemic vasculitis, and autoinflammatory diseases. It is a pleasure to introduce this new book, Clinical and Pathological Aspects of Skin Diseases in Endocrine, Metabolic, Nutritional, and Deposition Disease, edited by two of my friends and respected colleagues, Franco Rongioletti and Bruce R. Smoller. In fact, I was very ent- siastic when I learned that they were preparing this work. Although endocrine, metabolic, w, and deposition diseases are regularly included in general textbooks of dermatology and d- matopathology, one gains the general impression that these topics receive little emphasis and are mainly included for completeness. The prospect of having all of this information in one volume, lovingly described and detailed by experts in the feld, should be of great interest not only to dermatologists, pathologists, and dermatopathologists but also to internists and en- crinologists, who may well gain a different perspective on these disorders. As a derma- pathologist, I am also pleased that this project has been organized and undertaken by two distinguished authorities in cutaneous pathology. This assures a close integration of histo- thology and other laboratory techniques with the clinical aspects of these disorders, which will make it possible - perhaps for the frst time - to view these conditions in a truly comprehensive way. The goal of this volume is to summarize the state-of-the-art in the utilization of computer vision techniques in the diagnosis of skin cancer. Malignant melanoma is one of the most rapidly increasing cancers in the world. Early diagnosis is particularly important since melanoma can be cured with a simple excision if detected early. In recent years, dermoscopy has proved valuable in visualizing the morphological structures in pigmented lesions. However, it has also been shown that dermoscopy is difficult to learn and subjective. Newer technologies such as infrared imaging, multispectral imaging, and confocal microscopy, have recently come to the forefront in providing greater diagnostic accuracy. These imaging technologies presented in this book can serve as an adjunct to physicians and provide automated skin cancer screening. Although computerized techniques cannot as yet provide a definitive diagnosis, they can be used to improve biopsy decision-making as well as early melanoma detection, especially for patients with multiple atypical nevi.
This book, besides reviewing basic and clinical aspects of Behcet's disease, covers the latest findings, including genetic studies and treatment with biologics for the disease. Although the cause of Behcet's disease is still unknown, it is well known that genetic factors, such as HLA-B51, are involved in its development. Recently, novel susceptibility loci including IL10, IL23R-IL12RB2, and endoplasmic reticulum aminopeptidase 1 have been identified, providing new insights into the pathogenesis of the disease. In addition to basic research, the beneficial efficacy of anti-TNF-alpha monoclonal antibodies has also been suggested for not only uveitis associated with the disease but also other subtypes of the disease such as entero-, vasculo-, and neuro- Behcet's disease. Adverse cutaneous drug reactions (ACDR) are among the most frequent events in patients receiving drug therapy. Cardiovascular (CV) drugs are an important group of drugs with potential risk of developing ACDR especially in elderly as marketing of more new drugs and their prescription continue to increase. However, like with most other drugs the exact incidence of cutaneous side effects from CV drugs is difficult to estimate due to sporadic reporting. Moreover, a reliable designation of a certain drug as the cause of a certain type of reaction can rarely be made. Apart from the well-known angioedema/urticaria from ACE inhibitors, lichen planus / lichenoid reaction from beta adrenergic blockers and photosensitivity from thiazid diuretics, ACDR from CV drugs might be seen in a wide spectrum extending to rare but life-threatening conditions such as erythroderma, Stevens-Johnson syndrome, toxic epidermal necrolysis or drug hypersensitivity syndrome. In this comprehensive review, the reported types of ACDR to CV drugs will be discussed according to drug class and the type of dermatologic reaction with special emphasize on cross-reactions and the role of patch testing in diagnosis.

It has been known for many years that the Malassezia yeasts are associated with a n- ber of different human diseases ranging from pityriasis versicolor to seborrhoeic d- matitis. Their fastidious growth requirements defed the initial attempts to culture these organisms on laboratory media and their true identifcation and the relationship between different species only became apparent with the application of modern molecular techniques. Pharmaceutical companies are spending increasing amounts of money on drug discovery and development. Nevertheless, attrition rates in clinical development are still very high, and up to 90% of new compounds fail in clinical phase I - III trials, which is partially due to lack of clinical efficacy.
