A fail-safe supply network is designed to mitigate the impact of variations and disruptions on people and corporations. This is achieved by (1) developing a network structure to mitigate the impact of disruptions that distort the network structure and (2) planning flow through the network to neutralize the effects of variations. In this monograph, we propose a framework, develop mathematical models and provide examples of fail-safe supply network design. We show that, contrary to current thinking as embodied in the supply network literature, disruption management decisions made at the strategic network design level are not independent from variation management decisions made at the operational level. Accordingly, we suggest that it is beneficial to manage disruptions and variations concurrently in supply networks. This is achieved by architecting fail-safe supply networks, which are characterized by the following elements: reliability, robustness, flexibility, structural controllability, and resilience. Organizations can use the framework presented in this monograph to manage variations and disruptions. Managers can select the best operational management strategies for their supply networks considering variations in supply and demand, and identify the best network restoration strategies including facility fortification, backup inventory, flexible production capacity, flexible inventory, and transportation route reconfiguration. The framework is generalizable to other complex engineered networks.

Since the enforcement of the International Safety Management Code in 1998, companies have taken various kinds of measures to prevent accidents. Unfortunately, measures have been undertaken in a disorganized manner, and have not been effective. Experts of risk management, the safety management system, and accident models have each undertaken accident preventive measures within the scope of their specific fields, but have not looked beyond the realm of their own fields. This book discusses systematic accident prevention by integrating multi-disciplinary expertise based on academic research, the quality management system which has already proved its effectiveness in other fields, and findings of the author's research. In systematic accident prevention, the weaknesses of a system within which accidents and incidents have occurred are viewed by combining scientific accident investigation data based on the International Maritime Organization model and the accident model. The nature of every type of marine accident, such as collisions, groundings, occupational casualties, etc., are derived by combining the accident model and statistical data. System weaknesses are rectified by the risk reduction method of risk management, and the rectified performance is incorporated in improvement in the system by the PDCA cycle, which is the core of the Safety Management System. We can see the weakness in the system and reduce the number of accidents and incidents while utilizing limited resources optimally to prevent accidents and incidents.

Packaging is ephemeral - its purpose is to be 'wasted' once we've removed the product it contains. Whilst we are encouraged to 'reduce, re-use and recycle', Designing for Re-Use proposes that domestic re-use is the 'Cinderella' of this trinity, because it is under researched and little understood. The re-use of packaging could have a significant effect on the quantity of material that enters the waste stream and the energy and consequently carbon that is expended in its production - every re-used item is another item not purchased. The authors demonstrate that we do re-use - but usually despite, rather than because of, the actions of government and designers. The book shows that by understanding the ways in which actions of this sort fit with everyday life, opportunities may be identified to enhance the potential for re-use through packaging design. Yet neither has a practical understanding of the other's field, and thus communicating requirements and solutions is difficult.
