Yet neither has a practical understanding of the other's field, and thus communicating requirements and solutions is difficult. 'Complex sociotechnical systems' are systems made up of numerous interacting parts, both human and non-human, operating in dynamic, ambiguous and safety critical domains. Cognitive Work Analysis (CWA) is a structured framework specifically developed for considering the development and analysis of these complex socio-technical systems. Unlike many human factors approaches, CWA does not focus on how human-system interaction should proceed (normative modelling) or how human-system interaction currently works (descriptive modelling). Instead, through a focus on constraints, it develops a model of how work can be conducted within a given work domain, without explicitly identifying specific sequences of actions (formative modelling). The framework leads the analyst to consider the environment the task takes place within, and the effect of the imposed constraints on the way work can be conducted. It provides guidance through the process of answering the questions of why the system exists, what activities can be conducted within the domain as well as how these activities can be achieved, and who can perform them. The first part of the book contains a comprehensive description of CWA, introducing it to the uninitiated. It then presents a number of applications in complex military domains to explore and develop the benefits of CWA. Unlike much of the previous literature, particular attention is placed on exploring the CWA framework in its entirety. This holistic approach focuses on the system environment, the activity that takes place within it, the strategies used to conduct this activity, the way in which the constituent parts of the system (both human and non-human) interact and the behaviour required. Each stage of this analysis identifies the constraints governing the system; it is contended that through this holistic understanding of constraints, recommendations can be made for the design of system interaction; increasing the ability of users to cope with unanticipated, unexpected situations. Driver inattention has been identified as one of the leading causes for car accidents. The problem of distraction while driving is likely to worsen, partly due to increasingly complex in-car technologies. Marine accidents can occur at any time and everywhere in the world, resulting in loss of life, property, environment and reputation of the companies involved. Preventing accidents and establishing a safer world without accidents is an important agenda for the maritime industry. Since the enforcement of the International Safety Management Code in 1998, companies have taken various kinds of measures to prevent accidents. Unfortunately, measures have been undertaken in a disorganized manner, and have not been effective. Experts of risk management, the safety management system, and accident models have each undertaken accident preventive measures within the scope of their specific fields, but have not looked beyond the realm of their own fields. This book discusses systematic accident prevention by integrating multi-disciplinary expertise based on academic research, the quality management system which has already proved its effectiveness in other fields, and findings of the author's research. In systematic accident prevention, the weaknesses of a system within which accidents and incidents have occurred are viewed by combining scientific accident investigation data based on the International Maritime Organization model and the accident model. The nature of every type of marine accident, such as collisions, groundings, occupational casualties, etc., are derived by combining the accident model and statistical data. System weaknesses are rectified by the risk reduction method of risk management, and the rectified performance is incorporated in improvement in the system by the PDCA cycle, which is the core of the Safety Management System. We can see the weakness in the system and reduce the number of accidents and incidents while utilizing limited resources optimally to prevent accidents and incidents.

The key theme of this book is organizational learning and its consequences for the field of aviation safety. Air safety rates have been improving for a long time, demonstrating the effects of a good learning model at work. However, the pace of improvement has almost come to a standstill. Why is this? Many safety improvements have been embodied in technology. New devices and procedures appear almost daily, yet the rate of air safety improvement has dragged in recent years. Improving Air Safety through Organizational Learning explains this situation as being the consequence of a development model supported chiefly by information technology being introduced as an alternative to human operators. This is not a book about the convenience of including or not including IT in aviation, but an open discussion about the adequacy and risks of some practices in the field. Two different but complementary issues emerge. Firstly, a real improvement in air safety requires a different approach, since the present one seems now to be exhausted. Secondly, the current approach has powerful economic roots, and any new approach must deal with this fact, improving safety rates without becoming financially damaging. Consequently the book is divided into two parts. Part one deals with the issue of the present learning model organizing the conclusions around accident reports that show themselves the existence of a problem: the present use of technology makes the system better at doing things already known, while at the same time it makes the whole system worse at dealing with unplanned situations. Vast global resources are ploughed into the delivery of treatment interventions ranging from diet and lifestyle advice to complex surgery. Even where the individual does engage and understand, he or she may choose not to comply. Non-compliance is estimated to cost the pharma industry US$70 billion per year. No figures exist for the cost to healthcare insurers and public health but non-compliance is undoubtedly one of the top five issues facing both drug developers and healthcare providers. During clinical trials, non-compliance undermines the accuracy of the data generated from the whole trial as well as particular aspects such as the efficacy of different dosages. The safety case and its associated reports are quickly becoming not only a mechanism for achieving safety goals, but also a valuable decision-support asset, and a vital industrial liability management tool. Recent developments in industry have led to safety cases being frequently required as contractual deliverables as part of large and complex commercial programmes. A safety case consists of a rational argument and detailed evidence to justify and demonstrate that a system or product is tolerably safe in its use, and that it has a management programme to ensure that this remains so. The safety case report is the snap-shot presentation of the arguments and evidence demonstrating the contemporary safety performance of the system and the programme that is in place.
